This is a terrific video. I have suspected that I may have an APT. I was a gymnast for many years when I was younger and back then we didn't have springboard floors to tumble on. We tumbled on a two inch mat on concrete. I was told I have some arthritis at the L5 S1 junction not surprising. I have had back issues for a long time and I'm wondering if this is at least part of the issue but I've never been seen by anyone who knows what they're doing or what they're talking about unfortunately. Every channel on youtube tells you something else and it's hard to know who is right. I can see you know what you're doing and I appreciate that and that is refreshing. Thanks for these videos.

ITs a pleasure, glad you found it helpful, best thing to do is just get it properly measured :) then you can move forwards with a degree of certainty :)

I surely have an apt L4 l5 disc is very thin it is bulging but not too much My feel a constant pain in lower back most of the time i can bend forward and backwards without instant pain But i can't walk more than 5 min without the pain increases. Even when laying i feel pain. By the mri result i have hypertrophy ligaments. Doctors just gives me pills and tells me to swim and physio here make me do movements like full extention cobra. My question is this a facet joint problem?

If you already have an issue with the disc thinning it is likely that that segment is the issue, and more probable that you have a poster pelvic tilt you have to understand that if the disc is thin the facets have moved together too so the wear will be on the segment as a whole, it's never really one thing. Focusing on rebuilding strength and control is a sensible way to go to begin with: here's a link to the degenerative podcast which is relevant: backinshapeprogram.com/2023/07/degenerative-disc-disease-in-the-low-back-the-essential-guide/ additionally, you can check out the video on our channel homepage for a little more help too. and we have the full program if you want support in getting this resolved - backinshapeprogram.com

I don't think it is posterior pelvic tilt I can place 4 hands behind my lower back if I am standing by the wall and my belly is way forward and i am skinny

@@mustaphaassi58 It's best to check on load bearing X-ray, there are plenty of instances where the surface can create illusions, the spine is rather deep inside and it's better to be certain than to mess things up more.

@@BackInShapeProgram Is it possible to get a load bearing X-ray done on the NHS? I am super skinny, I can't stand still on my feet for long before I get immense pain in my pelvis and lower back so intense it drains me of colour and makes me feel like I'm going to pass out. Zapain helped, but they won't prescribe me that anymore. I never overused or ran out before was acceptable. Was issued something like 48 tablets of them in 3 foil sleeves. They lasted me over 18 months. They won't prescribe any more to me claiming I could become addicted to them. I point out to them that they lasted me over 18 months. They don't listen and I have said I should take codeine and paracetamol instead short term but obviously I can't take codeine and paracetamol short term because the pain has worsened and whilst I don't take more than four tablets in 24 hrs and I don't take them every day I do need them in my bag I'll buy my side in bed because lying down creates the same pain as standing up still. I end up rocking from side to side on my hips in bed. I can walk, but doing so creates the same pain, but not as quickly as when standing still. When standing still my body kind of goes into shock mode incredibly cold, an incredible excruciating pain almost as if I've got hypothermia, when this happens I have to get off my feet immediately. It makes me feel pain in my legs also which is not sciatic pain because I already know what sciatic pain feels like and it's a different type of pain to that. My upper back is very straight, though when lying down straight on the floor my pelvis is on the floor as is my upper back, but my lower back not above the tummy button is off the floor curved and strain is on my pelvis. I cannot lie my whole back on the floor straight. The only way I can get my whole back straight on the floor and get rid of that lower arch/curve and get rid of the pain somewhat is by pressing on my stomach muscles just below my tummy button and also lifting my pelvis up off the floor in a balled up hedgehog position as if I am going to roll my lower body over my head roly-poly style though without actually rolling my legs and lower body completely over my head. A yoga move as if I'm cycling in the air almost, my pelvis and lower back off the ground, my hips hoisted up by hands. I can stand and touch my toes without any issues or pain, I can wrap both my legs around my neck without any issues or pain. It is the load bearing issue of me being on my feet that causes the excruciating noring draining pain in my pelvis hips and lower back. When lying flat in bed on my back before I fall to sleep I need lots of pillow support of my pelvis hips and of the arch in the lower back. I also have significant pain in the back of my neck. I previously had x-rays done nothing was found. The pain and walking issues I have increased since I was in a coma in 2018. I sleep on my left side and on my right side with a pillow between my knees. The pain is norringly excruciating I've seen a neurologist about my walking issues and the pain it was them I believe who prescribed the zapain. After my coma I had to relearn how to walk, I achieved that fairly quickly with the help of a zimmer frame after initially not being able to walk at all. However this pain I experienced was a pre-existing issue prior to my coma. I have recently seen a podiatrist who has made insoles for my shoes to try and correct my walking issues but I know those insoles are going to do nothing absolutely useless because the issues stem from my pelvis, hips and lower back which in turn affect my feet and walking too. I have had nerve conduction studies done those returned normal (apparently) though I never got to see those results. I've had MRI done of my brain those apparently come back normal, though I've never seen the results of those either. I've had the electrodes on my head, again results apparently return normal I never saw the results. I've had scans done of my lower back, apparently those results came back normal and I never saw the results of those either. I feel like nobody is really listening or really looking and all I know is that when I have to stand for any amount of time or walk after an amount of time I am in excruciating noring pain. And at times it feels like my spine is grating on the inside of my pelvis and that my spine is going to shether or snap leaving me in a wheelchair get these so-called medical professionals can't find the cause and this has been going on for years and years, and is not getting any better. A lot of the time I feel like I need somebody to grab my hips from behind and with their foot on my pelvis they need to pull towards them to realign my pelvis and hips. They were pondering after my coma if I had dystonia but they settled upon essential tremor, ataxia and a walking gait. I have not been given any physiotherapy in my coma I suffered extensive muscle wastage and because I was already super skinny that wastage has created muscle weakness issues. Though I feel the strongest part of my body is my back and in particular my upper back, but my pelvis/hips and area just right under tummy button (waist) is problematic. Pains in my legs and feet which are not sciatic pains. I also have some bladder issues embarrassingly. I don't understand after all these test scans everything that they've done they can find nothing at all and despite them knowing there are problems with my walking and pain and pelvis they offer no physiotherapy. I can't afford to pay for physiotherapy it has to be on the NHS. Have you any suggestions?

The big issue here will be a degree of lack of conditioning and body control. Building strength and spinal stability will be the best way forwards to sure up the pelvic stability and safeguard the L5/S1 region too! if you need help with this check out the program backinshapeprogram.com/premium/
