My mom had a THR after she was misdiagnosed with back pain for years. Before the THR, she was in chronic pain and could barely walk across a room. After the THR, she has been pain-free and walks faster than I do! I am so grateful for this procedure...it gave her a new lease on life. Thank you to all the surgeons who perform this surgery well!! Wow, same here. Misdiagnosed. Treated my spinal stenosis for 3 years even put in a neurostim implant...extreme pain, constantly.. found out on MRI to check my implant . At the bottom of the report it was noted that had severe degeneration of my hip. Instant relief with thr. Just had my other one replaced...I'm feeling bionic. Lol

I’ve had both & I’d NEVER allow another surgeon anywhere near me that only does the PLA. I’d just keep my old totally destroyed w OA hip before I’d endure another PLA, for a THR! My first THR was done with the PLA & it was just wretched unexplainable horrible pain for months afterwards. Way worse than the pre-op bone on bone pain I’d had for many years. My now deceased husband needed me on my feet & taking care of him a whole lot more than I needed my destroyed hips replaced. So that’s exactly what I did! The second hip, I just had done on 30 JAN 24, exactly 18 months to the day after my first one. (& I have to say I was dreading it worse than my by then really messed up entire upper femur), from continued use long after it should have been done. It had “Stove Piped” is how my surgeon described it. So much so that doc told me afterwards he had to use a LOT more bone cement to get the stem to adhere due to having to remove way more bone than the CT Scan (which was done less than 60 days prior to surgery indicated!). And this OS used the DAA w MACO CT scan and robotic assist. As a FULL disclosure I’d like to correct a comment from the video, the robot does NOT do any of the cutting on anything. At least NOT here nor in my case! The robotic assist does the grunt work of moving the patients body into the necessary positions. Ergo releasing the surgeon & other staff from manually having to move, (for me), what equated to 140 lbs of dead weight around. And I had exactly ZERO issues with that & actually appreciate the use of that type of equipment to save wear & tear on the entire OR staff! Now let me tell you the DAA vs the PLA, was like the difference of night & day, the ocean vs the desert, post op pain wise. With the DAA, I was off the bed & full weight bearing walking within 30 minutes out of the OR. (No PACU here in my lil hometown w just 39 total beds in the whole hospital. PACU is basically done in the room I was assigned, but spent very little time in.). My BP went way too low & rather than have them just start pouring meds into me needlessly, I asked to get up & walk first. Which did exactly what we needed & no BP meds were needed. Post op I drove myself to a PCP appointment on day 6 & to my OS office to get my wound pump removed on day 7. That wound pump was worth it’s weight in 24Kt gold, btw, as I had absolutely NO swelling what so ever from the DAA THR. Never even needed an ice pack! Whereas with the PLA 1st THR (I’d never even heard of a wound pump, least of all had it used.), & I was so swollen for months I had to get bigger britches as mine wouldn’t fit over that swollen hip! Now @ 2 years & 8 days later, I still have some swelling on that side! On day 10 for follow up appointment w my awesome OS doc with this last one I drove myself, had ditched my cane AND I wore my boots with 2&1/2” high heels for the first time in years & had not one drop of hip 40:37 pain from wearing them! I had so little post op pain w the DAA & robotic assist that I can 100% honestly say I’ve had worse pain from a sliver! It was amazing the difference in the 2 approaches. Further I have less than 2 MILLIMETERS difference in leg length, certainly NOT NO 1/2 INCH & have to wear a shoe lift or limp the rest of my life! Thanks but um NO THANKS. To wrap this up, my first surgeon had 37 years @ the same facility doing exclusively THR & Total Knee Replacements. He was a very kind hearted doc & very spiritual. However continuing to use the PLA for THR’s is IMO, BARBARIC in comparison to the more up to date DAA. My last OS is about the same age as my youngest son, mid 30’s & has only been fully OS Licensed w FOAOS, for less than 3 years & he does all ortho procedures not just mill operation type THR’s to get 5-6 cases done each day he has the OR like the first OS I had, plus I was 2,000 miles further SE & had the first THR done at one of the major med facilities in Chattanooga, TN. Just the ortho floor has more beds than our entire rural 39 total bed facility has.

Had my THR Anterolateral on the right side 3 week's ago. It has been a very good experience for me. Immediate relief from the arthritis I have been suffering from for years. I am walking 1.5 miles a day. I am scheduled for the left side in 9 weeks.

I had my left THR end of April and my right THR a few days ago. You are correct immediate relief from the osteoarthritis pain in the hip. Recovery for me for the first one was very easy. So far the recovery for second one has been a little more pain but recovery is still going very well.

What a comprehensive explanation. I had a TKR courtesy of UK NHS hospital Rugby 8 months ago. The most painful surgery I ever had with many analgesics used in the waiting time prior to surgery, so many opiates, NS-AIDS, NEFOPAM, paracetamol, blood thinners during post surgical hospital recovery. During and up to 5 weeks after surgery strong pain was constant.... And then the pain dropped away and I was able to eliminate opiates and all the other painkillers over the next 4 months. 3 months after I drove alone from England to Southern Portugal, whilst still taking Etoricoxhib and Nefopam. A distance of 1600 miles with a stick shift manual car, using my new knee all the way continuously. Once here I stopped all painkillers to discover that they had masked deterioration in my right hip which I now know needs THR in the coming months. Bummer😂 I am very grateful to this video for explaing so clearly all the pros, cons, choice and risk factors involved. Now better informed, I feel much more confident to handle what is to come.

I've had both hips replaced.... First was two years ago. That was my left, done Posterior method..... Was back to work in six weeks. Just had my Right side done, Anterior and going into week five. I will not be ready for at least another three weeks to go back to work. If I could do it all over again I would opt the Posterior. In my opinion, the Anterior is way more risk of nerve and muscle tissue to recover from surgery. Right thy is still numb .

People will always say whatever method they chose was the "best". The simple truth is after a 4-6 month total recovery period neither has any documented proven better results than the other. And anterior "may" have less of a chance of dislocation but with a 1-3% chance with a posterior thats unlikely anyway. You'd have to fall or do something very stupid. For me, I chose posterior because I am in excellent physical shape and didn't want to risk the possibility of any nerve damage from the anterior approach. I didn't need any pain meds, was walking with a cane 3 days later and got out of bed myself. Everybody is different.

Iv had trh done posterior ...wondered why my recovery was so long after investigating I had some nerve damage and developed tendonopathy...4 years on and after a lot of rehab to build up that area on my part I'm in better place but have to say the rear in quite devastating and when I need the other would opt for side approach..

Anterior and Southern approach can be debated for infinity. Posterior is just best. There are way too many problems with anterior approach. I've operated on THR over 600 over since I've been a Orthopeadic surgeon. Never had any dislocation in my life. In my point of view, I think for hip placement, anterior approach should be banned. Period. Use it only for other procedures.

I have definitely been in severe pain with the anterior surgery. The femoral part is so painful. It’s been three and a half weeks and I’m still in pain and using a walker. I was sent home the same day. Family stayed with me for five days. After that, it was very difficult to care for myself. I walked inside over one mile every day. It was the only way to get some relief from the pain. They had me on two 500 mg Tylenol, one 800 mg ibuprofen every eight hours and one baby aspirin a day. My primary doctor told me not to take baby aspirin because of my age, 63. It could cause a brain bleed. Doing my own research, I discovered that ibuprofen causes you to swell. My right leg was twice the size of the unoperated hip. After I went down to 200 mg of ibuprofen and no Tylenol, the swelling decreased drastically. I took a very good quality B complex vitamin and a full regimen of other vitamins. The B complex greatly enabled me to walk better and with less pain. The wound was red right after surgery. My physical therapist got the doctor to prescribe an antibiotic which cleared it up. I feel as though I was put in harms way by being sent home the same day. It is also necessary to have someone with you for at least two weeks. It’s been sheer hell but I’m on the mend. I’m not sure how they expect someone to safely walk with a Walker when they are on high doses of Tylenol and ibuprofen. Getting out of bed was a nightmare.

I had the anterior surgery 10 days ago.. recovering is so easy.. Lots of soreness around the incision as you would expect. I have not taken any narcotics for pain. I am now off any walking aids and driving. Best of luck to you...definitely say go with anterior, I did a lot of research before having it done.

If you had your hip replacement on the right side you should hold your cane in the left hand. Sleeping the first couple of months is challanging. As a side sleeper I used a pillow between my legs. Now in my 4th month and am playing golf. Only problem in getting my stocking on but I manage. I am 89 years old.

I personally HIGHLY disagree with there being no difference as far as recovery for PTL vs DA approach. I have had both. Exactly 18 months apart. The first was the PTL & first off the numbing block did NOT work one bit. I woke up in the most horrible pain I can imagine. (I had both of my sons natural birth without meds, blocks, etc & I’d gladly have triplets every week the same way for weeks on end than to endure the horrendous pain I had post op with the PL approach. I thought the post op pain was NEVER going to end & it was horrific for weeks on end. Having the DA Approach on 30 Jan 24, Due to my BP dropping far too low, I got off the bed & was walking FULL WEIGHT BARING, approximately 30 mins after leaving the OR, (not the PACU as our 39 bed facility does NOT have a PACU.) I walked a full flight of stairs up & down & could have done several more, within 4 hours of coming out of the OR. I walked out of the hospital, (I’d already parked the walker & have never needed it again.). With the first PLA I was 3 solid weeks before I could do full weight baring & 4 weeks on the walker! With the last DAA, I used a cane for the first 4 days & then simply carried it for a safety issue, such as getting off balance. I would definitely have either one of my hips redone if it became necessary. But if it wasn’t with a surgeon using the DAA I’d just have to pass & park myself in a wheel chair for whatever time I have left in this human suit. I will NEVER UNDER ANY CIRCUMSTANCE have another PLA hip THR! YES DOC as the recipient of both of the most common approaches I am bring 100% honest in my comments. If you sir; were to need a THR I certainly hope you find a qualified OS that will save you weeks of suffering and do a DAA for a THR!
