The Crushing Truth Behind Carpal Tunnel Syndrome!
Are you still having carpal tunnel pain? If you're trying to avoid surgery for carpal tunnel syndrome and have tried physical therapy without success. Maybe you are missing another site that the nerve is being "crushed". The clinical name of this is "double crush syndrome". This syndrome is described as a compression to a peripheral nerve at two or more locations. Often surgeons attribute failed nerve decompressions to this syndrome. When treating people for carpal tunnel syndrome we often do a postural and cervical evaluation in addition to a brief examination of the cervical spine to see if there's other sites besides the carpal tunnel that might be contributing to the symptoms.

Over all my years of practice, I have found that almost all carpal tunnel symptoms do actually come from the neck. Correcting an irritated disc by adjusting one or more of the lower cervical vertebrae from an anterolateral direction almost always relieves numbers, tingling and pain in the fingersthe same day. I wish more chiropractors knew and implemented this adjusting technique - nothing beats it.

I developed bilateral double crush syndrome 10 years after neck manipulation during general anesthesia. Neurology defers to hand specialists. Hand specialists either want to cut right away or they defer to neurosurgeons. No one has taken the time to explain it like you did here. Thank you. It would be so nice to get some relief.

Had this for 3-4 years. Had 2 shots, then went to a specialist who refused to give another shot. Said it was double crush. Went to physical therapy and stopped using my phone in bed at night- it helped. Then went to a chiropractor, adjusted my neck and ribs and the pain DISAPPEARED INSTANTLY.

I had symptoms at night that at its worst point, felt like my arm was in boiling water. Through all kinds of PT, antiinflamatories, x-ray, MRI and so on, I finally was sent to pain management where they did an epidural injecting prednisone directly into my neck (I had compression of the nerve at C6-C7). What a life changer! It cleared up the symptoms. That relief lasted over a year and allowed me to spend time doing things to figure out some of the lesser symptoms. After an EMG & nerve conduction survey, it was determined that I also had severe carpal tunnel in my hand. 2 days ago I got the endoscopic CT release surgery. Because I wanted to watch a procedure, I just happened upon your video today as the “up next” video. OMG - you explained things so well to me and it makes total sense. Now I understand why when things were “fixed” I was still having numbness/pain in my hand. It seems I probably have more work to do to loosen my shoulder and neck so I can be back to pain free life. You have given my great hope to feel good again. Thank you for the thorough and clear explanation with imagery.

Probably the best educational video about this issue I've found. Making sense of the issues we experience is SO important for us patients. I have exactly this (aided by poor posture due to having scoliosis) and after some failed treatments, I finally found someone competent who is addressing the issue as you describe and felt almost immediate relief after the first session. I'm currently doing a combination of chiro, physio, IMS, and a bunch of exercises and stretching at home to improve my posture and strengthen my muscles. Great summary!! The way "Carpal Tunnel Syndrome" is diagnosed in the conventional medicine context is ridiculously oversimplified. Thanks so much for getting to one of the possible/likely root causes! It took me months of my own research and digging, in attempt to self-treat, to connect the dots between my chronically tight scalenes and pecs, and my recent diagnosis of carpal tunnel syndrome on BOTH arms, all compounded by bicep tendonitis (distal attachment) on one side, which I suspect is further compressing the ulnar nerve. I'm a 34 year old athlete (female, with a very "thin and long" neck) who also has a physically active profession, so this diagnosis is at a relatively young age. Reason being, the points illustrated in this video! Well done :)

Excellent presentation! What you are describing is exactly what I have..both of my wrists go numb when I go to sleep and wake up immediately. I also have a stiff right shoulder with lesser mobility which based on what you you are describing is probably coming from the neck. The surgeon recommended to do surgery on both of the wrists at the same time, but I waited for surgery and I tried putting a pillow higher in bed. I don't get the tingling any more so long I sleep on my back with a hogher pillow.Where can I go to get the treatment you are referring?I live in Miami.

I waited nearly 3 months to see a specialist and by this point i could barely walk. I was sent for an MRI and 5 days later i was told i had severe spinal cord compression. I was immediately sent for emergency surgery. I can now walk again but suffer a lot with chronic nerve pain and muscle spasms due to a damaged spinal cord. A few years ago, I began competitive golf after a layoff, and my back was incredibly tight. I figured it would go away it did not. And then completely transformed my entire fascia system, spontaneously, and intuitively over the course of about eight months. I’m quite certain no one has gone through what I’ve done. Now my spine is very mobile as well as my back and I frequently, go, numb in my fingers and my hands. I simply adjust my position and self adjust my spine a little bit and feeling comes back immediately. It’s not concerning at all. I just woke up from a nap with a thought. Carpal tunnel syndrome is a symptom. And it is absolutely caused by misalignment of the lower, cervical and upper thoracic spine. You’re welcome.

This is really the one thats happening to me,the neck, pec minor area and the my rib is elevated,i just seen it today in the mirror,i told myself how come its diff. and i got a tightness on the side of my neck,( as the other chiropactor told me) my shoulder also i know theres something wrong inside my shoulder or close there, also my rt.side i always feel cold, and i think i got a pinch nerve also that my chiro told me but i think he didnt fix it properly as im still feeling it close to the humps at the back of my neck or between that and the blade( sorry i dont know what you call that bones at the rt.back bone,im not sure if its sholder blade). I suspect my 1 massage therapist did something wrong coz he pressed a lot on my shoulder so hard that its so painful he even told me thats the first time he did it, after that im in too much pain and i got numbness mostly on my index finger and thumb, the middle finger just a little bit. My doctor gave me a teva lenoltec no.3 and cyclobenzaprine that helps me for pain and to go to sleep. Could you kindly give me an advice what to do or where to go to or who can help me? Pls. pls. kindly help me, i dont know what to do anymore, ive been doing some exercise that id seen in some video here in youtube about median nerve carpal tunnel syndrome exercises. I’m experiencing more thoracic area pain and wondered if my pins and needles in both my fingers on both hands could be coming from this area. I’m definitely going to get my neck and shoulder areas checked too because these are both stiff and sore too! Any help on exploring any other areas where this tingling might be originating from would be greatly appreciated ❤

Question: Is there such a thing as a "triple crush syndrome?" I'm a 67 y/o female & an R.N. I am 5'3" & weigh 145 lbs. I was diagnosed w/ double crush syndrome about 2 yrs ago via MRI Cervical spine & EMG/NCS (the needles were very mild feeling, esp. compared to the nerve impingement pain). I have mod. to severe pain from my neck (C5-C6 that is completely black on MRI), to both shoulders (including scalenes, pecs, & across bil. scapulas), & down both arms to fingers. I have also been diagnosed w/ bilateral cubital tunnel syndrome & bilateral carpal tunnel syndrome. The above has just been treated conservatively. The doctors have told me I need all of it surgically fixed as soon as I'm ready in order to adequately decompress the pathways. I have atrophy beginning in both upper arms & some in my thumbs. My fingers all draw up when I do chores for any length of time (e.g., hand washing dishes, raking leaves, ironing, sweeping or mopping floors, & work on my computer). I have paresthesias in hands & fingers intermittently. I have a lot of trouble sleeping due to the pain as described above, but also as a result of 3 failed lumbar back surgeries w/ hardware placement, pudendal neuropathy, & severe pain in bil. ischial spines & bil. lower buttocks. This is all complicated by frequent, severe migraines (cervical pain exacerbates the migraines), severe fibromyalgia, mild osteoarthritis, diabetes Type 2 (A1C 5.7), DJD, DDD, bil. hip enthesopathy by x-rays. I'm a mess, but I'm a well-adjusted mess psychologically & emotionally, lol! The pain is often 7-8/10 through most of the day & it wears me out. I'm taking Norco every 6 hrs w/ 600 mg gabapentin, methocarbamol 500 mg 3x/day as needed, & naproxen 2x/day. I use ice, moist heat, a TENS unit at high intensity, a home exercise program, & infrared pads 4x/day. The doctors said to let them know when the pain becomes too much for me & they will repair the cervical spine & arm/wrist impingements (I have a neurologist, an arm ortho surgeon, a shoulder ortho surgeon, & a spine ortho surgeon. Two different neurosurgeons performed 2 of my lumbar surgeries (different levels) & the last level was done by an orthopedic surgeon). I really don't want anymore surgeries, but the atrophy is very problematic & worrisome.

I feel like this is me! I've had Carpel Tunnel for years. I've also had 2 cervical disc fusions. The electricity from my disc compression is different from the carpal tunnel. The electricity would radiate down both arms. As soon as I had the surgery on my neck the symptoms were gone! The carpal tunnel that I have is a different feeling. I've suffered for 28 years on and off. Just this past October I finally found a doctor that was willing to do something surgical and not just give me a shot and send me on my way. I've had the surgery on both hands with in a month. It's tough at times but I know I'm going to see a brighter future and be pain free carpal tunnel wise. I've got more surgery in the future because I have 4 more discs that have herniated in my neck. Thank you to the drs that listen to their patients and not just treat them and street them. I'm so grateful ❣️

@@hmmmm798 Hi there! I did not have endoscopic surgery on my hands and I was awake! They numbed the area with lidocaine and I didn't feel a thing! I have about a inch and a half incision on my palms just above the wrist. I have about 4-5 stitches in my palm. Your doctor will tell you that after your wound is fully healed use vitamin E or Bio Oil to massage the area to break up the scare tissue. I do believe that after you have healed and done your therapy I believe you'll be able to throw a baseball. Everyone is different and if you don't have any nerve damage I think you're going to be just fine. Don't get me wrong you're going to have about a week of some serious pain. I had both hands done within about 3 weeks of each other. As soon as the stitches were out of the right hand I waited for a week and did the left hand. Good luck to you if you choose to have the surgery. 🙏

I have had couple tunnel back in the early 80s and started having problems after I broke my finger from a line drive in Softball in 2021. That is when my nightmare began. First of all it took 30 days before I was able to have anybody look at my hand. Then it healed up but in 2022 I went back and told them I was still having problems with my hand. They gave me some type of test and showed that it was fine. So I went to another neurologist, and he did the same test, but it was not a warm test. It was a cold test on the hand. Told me that I have carpal tunnel in both hands. also that I had to pinch nerves in my neck . I do have a plate and eight screws in my neck from when I got hurt playing softball . I just had to put that in because it will relate to it a little bit later in the story . So I went back to the hand doctor told him this and he saw the Tests and performed the surgery on it. A year later my hand is still swollen and it’s worse than it was before. Below the palm of my hand are purple veins sticking out and when I take a warm shower, it’s like a purple belt all the way around my wrist. It was getting worse. So now at this point I went back to the other doctor that performed the test to tell me I did have carpal tunnel and he says well you still have carpal tunnel in both hands so he took an MRI of my lumber thoracic cervical to see what was going on. The lumbar and cervical are fused so he sees in the image from my cervical that I have now a few pinched nerves and a bulging disc and he said something about C6 C7 . So now I have a surgeon that said that he was going to do a release on my elbow and I think he said the ulnar nerve, but he needs to talk to his partner about the findings of the MRI of the cervical. So that’s where I’m at right now. Thank you for the video. Now I have questions I can ask both of my surgeons.

Thank you for your video, it was very informative. I have a question that nobody I have been to can directly answer and am interested in your opinion. Backstory: Worked as a beautician in a very busy salon 40+ hrs a week (was my first full time job as a beauty therapist after a career change from corporate). After a few months, I started experiencing discomfort in my right hand and rapidly led to extreme night pain and wearing a brace at night which helped. At the same time as this was occurring, I was experiencing [what I thought was] muscular pain under my right scapula. I'd get massages, but NO relief came - I never considered it could be connected with my neck and never reported my “muscle pain” to work or doctor! It didn't take long for me to get a NCS done, for the pain in my hand, and determined I had Carpal Tunnel Syndrome ("CTS") in both hands but right hand needed surgery and so I resigned from work, under Workers Compensation accepted as a work-related injury (I live in Australia so the laws could be quite different in the USA). I had CTD surgery in March 2018. Since surgery, the pins and needles reduced, but have never completely receded, but at the same time, the pain under my scapula continued to worsen and come on more frequently. Massages and acupuncture did nothing to relieve the pain. Two months ago, the pain under my scapula got so intense that I ended up in the Emergency Department on two occasions, lost feeling in my right thumb, index & middle finger, half my hand, forearm and upper arm, and couldn't even drive a car or even shower myself. I had an MRI that showed that I have compressions from C4-C7, with moderate disc protrusion at C5-C6 and a large herniation (cervical radiculopathy) at C6-C7 pressing nerve root against spinal cord and told by a neurosurgeon that I should have surgery ASAP to avoid ending up in a wheelchair! Work Cover won't consider that it is connected with my CTS or work, however I am convinced that the position of my neck in a downward position for up to 9 hours a day performing facials and massages contributed to the pressure placed upon my discs leading to the protrusions ... which I wouldn't have known that I had, other than the pain in my scapula and there's no way I considered that was connected with my neck at the time. I'm 48 years young and the MRI indicates: - facets at C5-C6 have minor degenerative changes with moderate sized left paracentral disc protrusion abutting cervical cord at the site of the left C6 nerve root; and - facets at C6-C7 have a normal appearance with “shallow disc osteophyte complex" and large superimposed central and right paracentral disc protrusion with disc abutting and mildly rotating cervical cord posteriorly by 10% and markedly compressing existing C7 nerve root. I'm just trying to understand how my neck ended up in this state and whether they overlooked the main cause of the CTS. Surely not every female ends up in my condition without there being some contributing factor or event. NOTE: To keep the pain levels down, I’m taking 200mg of Lyrica daily and 10/5mg of Targin (OxyContin/Naloxone) and been told by the neurosurgeon to remain sedentary until I have surgery (Anterior Cervical Discectomy & Fusion). Thank you for your time.

Hi Trina. I was just reading your comment. I'm sure you were a bit disappointed that Apex Orthopedic Rehabilitation clearly did not read your comment/question! It's been a year since your comment. I am very curious how everything has eventuated! I wonder if you were misdiagnosed initially too. I had a nerve conduction study done as well and also had carpal tunnel release surgery 5 years ago in both hands. Did your nerve conduction study involve needles like he mentioned in the video?? Mine didn't. I live in Australia too. I'm struggling with significant numbness and tingling again...mainly in my right hand. I am very sore and stiff in my neck, shoulder, ribs and arm on the right too. I'm happy I stumbled onto this video as I was preparing to book for surgery again. I'm going to find a therapist today to address my muscle issues first!!

@@miadafoenour8927 Hi Mia. I just stumbled across your message, thank you for responding. I absolutely was misdiagnosed. All the research I've done indicates that the referred pain I was getting under my right shoulder blade was indeed cervical radiculopathy and was connected with my carpal tunnel syndrome. Like you, I just had a standard NCS, not with the needles. I can totally relate to your symptoms and am happy to report that I did end up having surgery in May 2019 at Royal Brisbane & Women's Hospital ... I had an Axiomed arthroplasty prosthetic disc @ C5-C6 and ACDF @ C6-C7. The surgery took 3.5 hrs and was able to slowly take myself off the toxic medication after about 4-6 months and although recovery took at least 6-12 months with six months of regular physio, I feel the surgery has made a huge difference to my pain levels and now, 16 months on, I only get mild pain and discomfort if I do too much which triggers that sharp/burning nerve pain ... I take panadol osteo, use muscle relief cream for my stiff neck muscles and use heat packs (so much better than before). I still have numbness and tingling in my hands and fingers (mostly my right hand) but was told, by the neurosurgeon a year on, that probably won't ever fully recover as the compression on the nerve was there for too long, thus causing irreversible damage, but I can live with that compared with the pain I was in before surgery. I don't have full range of movement of my neck, but it's definitely better than if I had a double fusion ... the prosthetic disc gives me a lot more movement and puts less strain on the discs/vertebrae above. Have you seen a neurosurgeon? What do you think you will do? I can say I absolutely do not regret it, as scary as it was it was worth it.

Hi I was recently hospitalized for some terrible neurological attacks. Complete weakness of my entire body. They did every test on me, including the nerve conduction study. They ended up diagnosing three things. A cervical myelopathy at C5 and C6. Rheumatoid arthritis, and severe carpal tunnel. I'm beginning to think however, that's the biggest part of my problem is a my neck. You pointed out that this is exactly where the nerve to the hands leaves the spine. They did not recommend surgery at this time. I'm beginning to try the exercises you showed. Can you give me any recommendations? I am a musician and this is very bad. Need to check entire upper thoracic spine, & upper ribs all around, as well as jugular notch, & inside upper part of clavicle, too. Hold ALL the tender spots - right spot, at the right pressure for the right amount of time. In addition, isometrically maintaining erect posture while stretching neck ("pushing" head away from body) helps, as well.

Did you do any follow up videos for the neck and pec minor stretches? I was interested in doing stretches for this double crush syndrome. I have a very tight neck my scalenes are very tender and my left collarbone has a lot of tightness under and around the top. I would also say my traps are very tight too I think I have TOS for sure a lot of nerve pain pins and needles as well when I mess around in that triangle between the neck trap and collarbone. I know when I raise my arms they get that referred pain and numbness/tingling on the left side. I know for sure I have bad posture I try to shit back and not lean forward anymore but the damage is done I need to start loosening things up. I have some pain around where my neck meets the back I guess t7 is what I heard it is. I'll check your playlist later on see if I can find any I always add new stuff to try and help myself get better.

For years I had carpal tunnel syndrome coming and going due to overuse (gaming), over the years it pulled up to my shoulder several times. This winter I really overdid it, silly me. It started in the wrist as always and then over a few days the pain extended up the arm to the shoulder. Usually rest and exercises makes this go away after a month or two, and while my wrist is better this time the shoulder refuses to let up. Now 6 months later I still have problems using my computer and my fingers/top of my hand still tingles and goes cold when I do and ofc this puts strain on my hand and wrist again. I also wake up at night because of the pain in the shoulder and sleeping fingers. I been to physio therapy and the doctors without improvements, I have a time for the nerve test in a couple of months as my doctor thinks I need surgery in my wrist although the last nerve test showed no problems in my wrist. But watching your video made so much sense to me, after watching this I am positive the problem is in my shoulder. I have to mention this when I go in for the test. Hopefully they will check my shoulder and neck this time. Thank you for the explanation.

So, a few things going on with me and my hand. I started to have numbness in the tips of my middle fingers on my right hand that suddenly occurred a few months ago right after i received a massage. More recently within the past month, when i bend my right wrist forward, i feel a "zing " or shock like sensation through my hand. I've had the bouts of waking up with hand "falling asleep " while I slept and bothersome enough to wake me up. Another thing that causes that tingling sensation in hand is when I sit down, which tell me it may have something do with my spine? Today i had the EMG and NCV done. A little annoying sensation but not bad at all. I've sat through 4 hours tattoo sessions so I'm more tolerable to pain I guess, According to the findings, doctor said I have carpal tunnel. The First treatment would be a splint to wear at night. He did insert a needle in the back by my neck and it came out a negative for pinched nerve. I first went to a chiropractor when I first had the numbness in finger issue. He suggested "intensive "therapy where I went to him 2x a week for a month and no difference. So that where I'm at. So new these tests That i haven't even gotten a splint yet. Oh and another thing I am a pastry s chef, where for a several week period i was using a piping bag but I've been doing this for over 30 years and never experience this before. Thank you for posting this, it is great information and something I’ve been wondering for the past few weeks. On and off I get numbness in the tip of my thumb, and it seems to be directly related to my posture and head position. Ive been working long hours the last several months, and have definitely got tight pec muscles with rounded shoulders going. I notice if I straighten my posture and bend my head to the side, the bend in my neck causes the numbness to run down the nerve my arm into my thumb immediately. Sounds like this could be an instance where the issues are centered around the tightness my neck. Will look into some stretches to loosen it up and exercises to try to better the posture long term... My thumb moves on its own and it’s like a zap. I think it’s due to me sleeping on my shoulder/arm and it’s getting pinched there! My wrist is not getting compressed so this video is very helpful as most people are saying the wrist is where it’s happening.
