Thanks for this. I'm so grateful that the first surgeon I went and saw at 25ish just flat out refused to do anything to me. I asked for fillers and Botox. She said my face is well dimensioned 'you're the after photo'. Told me to wear sunscreen and be happy. Come back in thirty years for a facelift if I want. Thats what I've done. Thank you lady. She could have taken me for thousands. 🙏 she was ahead of her game in 2009.

Owing to my inherent disgust for injecting stuff into my body, even good stuff (I finally started Botox, sparingly) I have never used Fillers. Ever. Any kind. I had great skin and tone, and a youthful full face for a long time, but obviously by my late 40s I had to think about the future. I started to research things, look at photos carefully (including Ms. C Cox) and thought that surgery would be the best route. And, we are so fortunate that the surgery today is light-years ahead of what was available in the 20th century. Light-years. As a side note, I read a bio of a plastic surgeon in South Florida regarding the use and over use of radio-frequency treatments/devices on the face. He mentioned he had begun to see permanent soft tissue damage during surgeries on individuals who perhaps had done too many of these procedures. His personal thought was to be careful and accept that there is no great alternative to a top-line face lift.

My Google "handle" (Anya Amasova) is the name for the female lead character in the 1977 James Bond movie "The Spy Who Loved Me", played by the VERY beautiful Barbara Bach (aka Mrs. Ringo Starr.) Interestingly, I once saw a comment about Barbara Bach as a 60 something year old. A man, perhaps a surgeon, commented that Ringo had purchased his wife some absolute top-shelf surgery. Though Ms. Bach could pay the bill herself, I do think she has aged very well and her interventions have been the ne plus ultra.

Right!! I was talking to a plastic surgeon and he was trying to convince me to get fillers and that he would do them for free. I told him no bc you can see that it doesn’t go away and starts to sink in the face. It ended up being the end of our dating bc we disagreed so strongly. He was said that it’s proven that that it goes away. I never understood how no one could see what was happening to peoples faces over time. Thank heavens doctors are now speaking up!!!!

I had a groupon for fillers and the aesthetic doctor - laughed at me. Told me I had eye hollows and should cover them with make up as I could go blind with fillers. He told me I had asymetric cheekbones and I should keep them it was unique. He refunded me my money and told me to put it towards a vacation. Further INFO for the skeptical He ALSO told me he USUALLY adds ON more proceedures for people on top of the GROUPON and they end up spending thousands BUT they need it. HE said there was literally nothing he could add to my face that would not destroy my current looks and I should come back in another 5 years - if I had aged badly he would treat me then . But I should not obssess and enjoy my face !

I’m a therapist and I have seen a trend in the last ten years of people in their 20s getting treatments that have long-term effects to their physical and mental bodies, all to look younger. We have to stop treating aging as “something odd” or “unusual.” It’s normal. What is abnormal is for people in their 20s to ruin their skin and deeper tissues to look 17, and for people in their middle years to try to look in their 20s because deep inside they have a belief that aging is bad or ugly. It’s not and everyone is starting to look the same with all the fillers and surgeries and can’t manage a facial expression congruent with their emotions because their facial skin/muscles are surgically/chemically altered. Be yourself. Love yourself as you are; that lasts and you need no refills or follow-up surgeries. Blessings.

Sunscreen daily and retinol is the way to go! It is not easy to find a perfect retinol product or an ideal SPF for yourself, but god, it is so worth it. Long-term it is the best thing that one can do for the skin!

Having a solid skincare works wonders! I'd recommend using Retinol/Retinoid (it slows down the aging process of your skin and can slightly reduce wrinkles) and sunscreen (as sun damage is actually the primary cause of skin aging).

I’m not a doctor, or a medical practitioner and my clients would ask me my opinion on fillers as a beauty professional. Just my personal opinion, I told them that laxity and aging skin should not be filled, but instead tightened. Filling does not treat the sagging/aging and long term, you will look puffy and older instead of younger. It’s amazing to me that the medical aesthetic industry is just leaning into this… I had no experience with fillers or Botox and was able to come to this conclusion by just thinking for 5 mins. 😒

Plastic surgeons and med spa workers aren't beauticians, beauty professionals, modeling agents or artists. So they do not really understand what is beautiful, facial aesthetics or facial harmony, unfortunately. They just understand the medical aspects of plastic surgery and injectables. And besides that I definitely will never go for fillers but just likeneith tattoos, I'd rather pay more to get things done right or not at all lol. So appreciate your opinion. I’m an RN. I’ve worked with plastic surgeons in Palm Beach. IMHO less is more. Filler face or over toned is distressing & odd. I haven’t started my nip journey. That's good..Plastic surgery in general is ruining people in a variety of ways. I get that in the world of plastic surgery and cosmetic procedures, there are some that are better at their job than others. But please, plastic surgery is a sin before God. I totally understand people wanting to look and feel their best, stay young, etc..But it's a serious thing and shouldn't be done. There are natural ways to age gracefully. Plastic surgery doesn't last. People fall apart from these procedures and it just becomes an endless cycle of more. It goes against what God wants for people. We need to repent, Jesus Christ is real and the afterlife/spirit world is real. Repentance is truly a gift from God and has nothing to do with "religion". Sin really does separate the soul from God if you don't repent and get right with Him before death. God bless

I've never been "religious" and was raised in a home where we really didn't talk about God/Jesus, etc. Then things started happening in my life, it's like things became so clear and more things were revealed over time about the reality of this world. I'm so glad you're bringing this up. I think the filler option is an easy $ for practitioners and clients. I used botox around the eyes and filler at the cheekbone one time, and my undereyes started to hollow and skin was crepey/ saggy. Prior to that I didn't have those 2 issues. I was there for something different and got talked into the filler/botox.

I got a pea-sized granuloma from filler that I had to have removed by a plastic surgeon. Now I have a scar! He had to make a fairly large incision because it was more complicated to get out than he originally thought. Thank you so much for this video! I am sharing with my daughter and daughter in law. I am telling them to save their money for a proper face lift when the time comes. Every time they want to plunk down $1000 on filler or something, just put it into their facelift account.

I create their websites and know the many promises and lies that are told day after day to many clients. You are for me a true pioneer in this industry full of money-grubbing doctors who constantly use ineffective methods to make money year after year. I have always always always believed since I was19 years old that something that is suppose to disintegrate in your body after 6 months needs to be looked at. I was an esthetician student where a women had bulgey chunks in her face, she was 70 and had lots of fillers over the years. It came out that fillers can be blocking the abilities of collegen and can linger there much longer. I truly believe if you can get away with more natural ways, or lifts, cuts, machinery type of rejuvenation with creams or prescription, do it. Try to go as natural as possible in order to love your aging! That’s why I follow this fella here .

I have an appointment for Botox this morning and maybe a little amount of lip filler. Just last night before bed I was in the mirror considering under eyes/cheek filler (I’ve had it in the past) But I woke to my coffee and this video and what a fortunate coincidence! When I was a teenager many moons ago, a dermatologist told me that because I have oily skin, it will take forever to wrinkle. Now, years later, thankfully he was correct. Oily skin keeps you looking dewy and youthful. I've never injected anything into my face and never will. No sun and starting skincare young is key. It boggles my mind the unhealthy obsession with these fillers, especially in twenty somethings. They look ridiculous and won't age well later. I have no regrets about aging natural 😊

It just didn't last past one month. 4 months after, I reached out and while they said no refund they offered the redo with the nurses from one of the supplier companies showing techniques. Now my cheeks are lumpy and hurt . I feel like my eyes are being squished and I think/see my face looks swollen. I got covid a week after so I havent been back for them to follow-up but I'm going in today. I think I have a sinus infection. I regret doing this and spending the money.

Here's my question - when filler is injected is ALWAYS looks like the injection is VERY superficial. I have never seen a needle go really deep. So are you saying that the filler settles down into the lower layers of the face past the tissue, collagen and muscle? This video is GREAT! I'm a fat transfer gal - no more filler in my face.

Even before this video I was worried about trying filler just from seeing blister-like spots on lips of people I know that got it, and filler outside the lip line that didn't look natural

Thank you for being honest and bringing awareness to this issue. For those who have done a lot of filler, what are some solutions? You said the patient you were giving a lift did not have tissues strong enough to pull back due to the oozing filler causing damage to her tissues. What is the solution to repair your tissues? How can the filler be removed? Can it be removed? I heard from other specialists it cannot be removed completely, even from hyaluronadaze (sp.?) Are those who did too much filler completely screwed?

This part: Not only can a filler be stored in tissues for up to 20 yrs but it also destroys you tissues too ! That is very scary. I’m so happy you talk about it so openly. Thank you for being honest! My M.D. does fillers also, I threw away the card. I've noticed that many of my favorite movie stars, men & women look like lions, huge jaws, unrealistic cheekbones. I'm 67 & I've never had fillers, and I'd rather have a facelift. I have a nice face, maybe I'll forget the facelift, and go on a vacation!

This is a great video and I'm glad you explained this so thoroughly. I always imagined that adding fillers to the skin to plump up cheeks for example, would actually stretch the skin and once the visual effects wore off I figured the skin would be even more saggy as a result.

As an academic artist - never wanted fillers cause aesthetically they don’t make your lips prettier or natural looking(the look of everted lips is just not what’s looking attractive) Good point doctor about bigger and puffier lips/other face parts - don’t make you younger 👏🏻 hopefully I will see less people with fillers around 😅🙌🏼 yay!

I am71..I get filler once a year..1 syringe almost on top of my cheek bones..no where else..all it does is give me a more chiseled upper face look..that is all I have done for 15 years..and I must tell you it still looks beautiful..and I have strengthened my jawline by execise gadgets alone..so if it is a little and NOT on the mid to lower face I think you never have that overfilled or odd looking face!

@@kristinab1078 Of course I will share..even my Derm's staff bought these gadgets they thought I had a lower lift LOL..I am 71..I have been using the first one listed for many years BUT!!!I noticed a droop recently and bought a newer gadget and it is magic..also I collagen load ..I take twice as much 5 days a week..BIG help..I am a holistic health coach so I learned a ton when I went back to school for it 10 years ago.. OK here goes..First cheap gadget...both you can get on Amazon.."Neckline slimmer" A french man invented it..I think it is around 10$..newest is "FACEFIT" around 22$..this is amazing..I bought both strengths but please start with the lower resistance it is a workout..I never do facial yoga..I think it stretches the skin..Just my thoughts..Let me know how you do..!!

valid point! Volume does not play a role in making someone look younger . I have seen many people in their 20s with folds, and hollows, although look their age. And people in their 50s with overfilled faces with fillers to look younger but still look their age or worse, older. The problem is the clinics are offering discount rates on the more amount of filler you have, the cost is expensive alone. Doctors don’t like administrating 1ml filler to be distributed overall in the face. It is not always necessary to be overfilled in a particular area. The amount of time I have heard 0.5 will not be enough or will wear off in matters of months. When in fact, fillers hold on to water that’s why it can always seem one’s had filler again when their face is hydrated. Facial yoga can help face shapes and I’m also a huge believer in bio-stimulators, and feel they will play a huge role in the industry going forward.

Exactly HA attracts water it's terrible especially in dynamic areas of the face. Raidesse and Sculptra gives much better and more natural results and are actually regenrative, meaning they promote tissue healing and collagen/elastin synthesis. I prefer Radiesse as it's biocompatible and doesn't work through inflamation like Sculptra and you can dilute it as much as needed. Either way volume DOES play a role in aging, anyone who had sudden fat loss due to energy treatments or HIV treatments knows it, that's why Radiesse and Sculptra are FDA approved for fat loss in HIV patients. When you face looks like a deflated baloon all of a sudde, it makes no sense to think all of the tissue started to sag, if you lift them up you can definitely improve but you will still lack volume, cheekbone projection and that toned, scaffold volume underneath. Every doctor will try to sell you his perfect overpriced recipe but in reality an holistic apporach is always better.

Are they unable to “save” filler? Ex. You have to use whole syringe. What if you want to only try half in one spot. I understand there isn’t much in one syringe but just wonder if they are allowed to do that.

For someone with no fatty tissue on the face, we very much want the filler to remain for ever. I started filler at 24, I'm 32 and I just have no fat or volume to even sag. I've never wanted to look younger, I just want to have facial balance and volume.

Not to be rude or to offend...but a thin face usually goes hand in hand with weight...I notice when I'm fuller figured my cheeks are plump too with a healthy glow...I'm over 50 and my only issue is my lips. I know everyone is different but good nutrition helps including proteins.

I'm 43 and have never had anything done and have only recently started to consider having any fillers or work done. I think I'm still pretty happy with my face. The only treatment I think I'd do is a little botox in between my eyebrows and that's because I've heard it helps with migraines. The cosmetic aspect would just be a side benefit.

Dr. Karam, I am an esthetician and I really appreciate your videos. I often refer clients to your channel to help them understand the basics about plastic surgery. The surgeon to whom I refer in the Bay Area is very PRO fat transfer and will do fillers but only very conservatively. Personally, I cannot get any longevity out of fat transfers even with PRP. I have done the temples maybe 3 times. It simply does not last so I have turned to filler. I am a slender woman, I like to exercise and at age 54 volume loss is a real issue. This video gives me pause about continuing down this path. I had my first lift at 52 and that addressed the lax tissues but the fat transfer part of that procedure did not last. Might you speak to this is a video?

I’m 75 and have been getting fillers for 2 years. I refuse to have a facelift because I don’t want to have surgery unless it is medically necessary. I realize fillers are temporary but I am fine with that. I like the way they make me look and I will continue to get them.

Thanks for the education. I have been following the Australian doctor who was the first to talk about this on KZbin. The message is everything is moderation - not everyone can afford a facelift of nearly a hundred grand and not everyone needs a facelift . So we should normalise ageing grateful if filler is not the answer. Also get an ethical doctor, not all of them will want to fill you to the brim. It’s hard getting old, just take care of yourself please❤.

Interesting! My doctor uses fillers sparingly in the areas you mentioned maybe once a year. I have never done heavy fillers on my face. Thankfully I have good genetics and good bone structure.

Great information, Dr A is a good teacher. I just accept what my body is doing as I age. If my face will make a difference in my acceptance by those around me I'm willing to let those people go on their own way. If I need surgery to be appealing to someone I will find others to associate with instead. It would not be me anyway, that they'd admire, but a surgeon's skill. I'll choose to be a person rather than a painting.

Personally I disliked the feeling of filler. It was being utilized to fill horizontal bilateral drape lines from a 25 yr old chin implant. My body broke down the filler in less than 6 months. It wasn’t worth the pain and expense. I will clarify that it was done by a double board certified cosmetic surgeon. The final solution was 12 months of CIT & BBL treatments. The bilateral area involved is barely visible a year later.

Glad to know this about the cheeks as I was thinking of having filler done in this area as well because of excellent results having had 0.5 ml filler under each eye to get rid of the hollows. It’s the best thing I ever did when it comes to fillers. But that’s all I’ve done. Having listened to this video I think face lift is the way to go if one is thinking of it of course. In the US I heard about the EVE lift. Very non invasive. Has anyone had this procedure?

I knew fillers were no good and have been saying it for YEARS. Ever since I first laid eyes on pillow faced celebrities. These celebs have access to the so called best cosmetics doctors yet they are running around with these massive faces looking weird AF and convincing themselves that they look good. I remember reading a while back that fillers shifts the natural fat pads in the face over time, making you look older than you are. This video pretty much painted the full picture of the damage it can do. I am all for cosmetics procedures, but GOOD ones that won't have you looking strange over time. I am 40, luckily no wrinkle issues, but I feel like before I turn 50 I might just want to have a mini cheek lift and I will be good to go for a few decades if not the rest of my life.

I had lip filler 3 years ago and I still have it. When I want a little "umph"... I get a botox lip flip which lasts 1 month. I will get more refilled when I actually think I need it ..but I'm basing it on how it looks or feels and not just cse "1 year passed"

I'm 61 and have only had filler 1 time in my life, last March. I loved the way I looked but don't think I would every do again. My lips were great, My labial folds awesome, but the lumps from the filler that formed close to my eyes scared me to death. Took about 3 weeks for them to go away. The person that did it said keep using ice and rubbing them outward.
