I have recently been diagnosed with Spinal Stenosis. I've had MRI's done on my whole spine. I have weakness in my legs, along with numbness. My left leg is worse than my right. I have pain that radiates from my knee on the left side DOWN to my ankle and then on top of my foot. On my right leg I have numbness also but only around thee knee, some in my thigh and shin. I feel no back pain at all. I also was told I need knee replacements. But my surgeon is more concerned with the Stenosis. I have an appointment with a spinal surgeon next week. I really hope I don't need surgery. Thank you for explaining what spinal stenosis is and some of the treatments. I'm 63 years old and want to be able to function again.

I was diagnosed with Spinal Stenosis 12 years ago. Spinal Stenosis does NOT get better only worse, especially when it starts involving the sciatic nerve. I’ve had every procedure performed several times. Epidurals, back surgery, two different SCS devices, Radiofrequency ablations several times, opiates, trigger point injections, PT. Nothing works longer than a couple of months with only a 25% relief if that. Stopped all procedures and opiates five years ago because they were killing me. Medical Marijuana and Advil are the only things keeping me sane?

I'm sorry. Right now, I've had several months of physical therapy, and I'm about to go the spinal injection route. I kinda think it's not going to work for me. I'm just so used to being in pain, that I can't imagine anything working anymore. Sending up prayers for you (and me!)

Dr Armaghani seems like a credible physician with strong qualifications. I was impressed that he did not rush his patients into surgery for stenosis. The other orthopedic surgeon on KZbin, Dr. Lieberman of the Phoenix Spine & Joint, basically said you were foolish if you considered any other procedure except for surgery. Dr. Lieberman even mocked people who attempted to cure themselves through exercise/physical therapy as wasting their time.

4:00 you missed one of the most common symptoms of spinal stenosis (central stenosis, neurogenic claudication). That is lower limb muscle cramps, especially worse at night and with walking/standing++.

Thank you for the comment. I like to use the word "discomfort" as patients can experience the symptoms of neurogenic claudication in many different ways. Cramping, numbness, tingling, burning, shooting, etc.

Sitting down is worse for your back due to the increased pressure. I’m 28 and have stenosis and it makes the pain worse. Standing up or laying in bed is better. Luckily I have the surgery next week after waiting for almost a month with pain / numbness.

Thank you so much - I might add Homeopathy can DISSOLVE Bone Spurs which can also press on the Spinal Nerves so I've heard. Removing the ligament in consecutive levels rarely causes a problem long term. If you have pressure on your nerves and pain going down the legs with standing and walking, the removal has been shown to help patients with that condition.

51 year old hockey player here, I have been playing for over 30 years. I have never had a back injury or even soreness or stiffness. In addition to 3 or more hockey games a week, I also strength train sever days a week. I felt like I was in my 20s until a couple of weeks ago. It started with some mild tingling in my feet, then some pain when I stood up, increasing in mornings when I first got out of bed. After some moving and stretching, I was good to go. I would then have pain throughout the day, that would disappear while I was playing hockey, I would usually feel better after that. Bowling was good too. After about a week, I tried to do some curls, I could feel whatever was wrong in my lumbar getting worse with each rep so I stopped. I've been degrading slowly every day since then. During my hockey game the next night, I started to feel sciatic pain, I had to go to the boards a couple of times and hitting the wall put me in more pain. I told my teams I had to stop for a while. Now I'm at the point where I have pain shooting down one or both of my legs so bad when I get up in the morning, that I cannot function. I did see my doctor, got a Xray, still trying to make sense of everything she saw, had to call the office back to repeat some of it. I have Stenosis, possibly in the lumbar region. I also have some SI joint problems, which I have already started therapy for on my own while I wait to see a specialist. Motrin helps some, but I can tell I'm getting worse even with that, though the pain seems more located in the lumbar now and isn't going into my legs as much. She said I had something severe going on with my third and fourth vertebrae, but I dont remember what the medical term was, Im trying to get them to call me back to let me know, or refer me to a specialist. This gives me a little hope, I went from feeling 20 to 80 almost over night.

I have LSS as well. I totally understand your pain brother. I suggest you try to get a prescription for physical therapy. Both of my legs used to shoot in horrible pain every 10-15 minutes at a time, but strengthening my core and legs through therapy really helped me manage the pain. We cannot be cured of this but we can try our best to manage it.

Currently in my hospital room the CAT & MRI showed Spinal Stenosis I was recommended Lyrica physical therapy and possibly a Cortizone shot. I’ll do anything to improve my quality of life.

I wish you had been my doctor! The doctor I saw insisted surgery was my only option. I have regretted having the surgery ever since. I have had ablations, steroid injections, and medication, nothing helped with the pain until recently. I had a trial spinal cord stimulator and for 5 wonderful day I was pain free. Unfortunately the trial had to end but I am an excellent candidate for a permanent spinal cord implant stimulator. The surgery is next month and I can hardly wait!

I have lumbar spinal stenosis along with degenerative disc disease at L4-S1. My orthopedic surgeon has suggested posterior lumbar fusion (with bolts and screws) to allow more spacing. I also have an upcoming appt with a neurosurgeon to see if he is a better selection for ensure relief from the nerve pain I have been experiencing in my back, buttocks, legs, and feet, after being on Gabapentin for 10 months, having PT, acupuncture, trying steroid injections, all the things I can. Two questions: 1) I feel most pain when sitting, and less when walking or standing. This seems opposite of regular symptoms. Is that unusual? 2) Is a neurosurgeon or orthopedic surgeon better when performing disc and nerve relief surgery? Thanks!

Thank you sir. I have been having radiating pain down my legs since 2011. At that time it was determined to be a mix of few things. A minor fracture on the L5 and a grade-2 slippage. Losing weight and core training helped a lot. The pain comes and goes but certain position, for example bending forward to do work (tiling a floor) causes back stiffness after a minute or so. The radiating pain now happens a lot less. BUT the pain has shifted to the lower back itself and it tends to radiate to the hip bones on the side and front. The past year has been pretty bad. Waking up it hurts and super stiff. Few months ago it was so bad for 2-3 weeks that I could rate the pain as 7/10. It takes a good hour for things to go to back to a bit of normalcy when I sit or walk but breathing in can hurt. I met a surgeon and we did an MRI. My bottom 3 disks are not health, but the one in the L5-L1 is in a very bad shape. They all started to lose height but the one at the L5-S1 is very thing and it is causing sever nerve root flattening. My surgeon said this is not something that can heal on its own. However if I am not getting numbness, weakness or sciatica pain, the surgery itself might not make any pain resolution. He was just honest and wanted me to make an informed decision and I appreciate that. Basically a disk replacement with fusion on that level. Now my only question for you is related to the nerve damage. Do you recommend surgery to avoid further damage to the nerve once symptoms are not that bad? I mean I have tons of symptoms but the good thing is that I am not getting a lot of radiating pain down the legs, but it seems like once you have a lot of pain, it might be too late to fix it. So the structure is fixed, but the nerves are done for good. Thoughts? I know it differs for each case. The other two disks above the L5-S1 have herniations.

kiwiswat, I can greatly relate to your conditions. I have similar and more. In addition to the L5-S1 Spondylioethesis , I have Spondylosis of the entire spine to the top of my head. Variuos sites of Stenosis (L2, L3, L5-S1; C4-C7 ), many disc bulges and Dextroscoliosis of 25 Degrees. have your SI Joints examined too. A Rheumatologist told me some years ago 9I do have SI Joint chronic Inflammation and OA in the left Hip) is the Ground ero to many spinal problems. My right leg is 1 inch shorter than my left as a result of fracturing my right femur imn 1966 , age 15 playing HS Football. That "Trauma" left unaddressed for many years, has yielded many problems in my spine . hips, knees, etc. This doctor lists many things, unfortunately, I'm The Sub-Set for many of the issues. 6 Surgeon consultations all said No Surgery...Too much Arthritis. keep in Mind, under current strict protocols. Spine / Neuro Surgeomns will NOT operate if the success rate isn't at least 80% (expectancy). It reflects on them and the associated Hospital... I've tried 6 different Chiropractors which did help a little, Acupuncture (45X - very little help), PT at least 7X over the years, but not offering the relief I need from the Chronic Pain and Stiffness. Like you, I need about an hour each morning to find some relief. When there are no real answers ... this SUX Big Time.You should try certain supplements to help with Inflammation (which is Public enemy No. 1): Turmeric, ginger, cinnamon, prolyoetic enzymes, MSM, Hylauronic acid, and many more for the battle. But everybody is different with differrent issues.

Thank you for the great video. My mom is having insane back pain problems lately and shes 64. She had some back surgery over a decade ago and now past few years have been pretty bad for her with sharp pain in the lower back. She went to see the doctors in Finland and they said she has Spinal Stenosis. She was also prescribed pills (I do not know what kind) but she refuses to take pills.. I am trying to reason with her to take the pills but she is stubborn like a child and then cries how bad her back is.. I am so sad and angry.. I dont want her to get another back surgery, she has tried physio etc.. she is also telling her doctor she has taken the pills... I am at a loss.. finally I told her to take the goddamn pills, ask doctor to presribe new ones and tell him that she did not take them although she said she did.. I hope it will help but I cant feed her the pills because she lives in another country most of the time.. she says she is afraid of pills because they will kill her.. Im just at a huge loss of words at this point..

If those pills are opiates then she’s smarter than most! If she has access to medical marijuana she should look into the edibles. When I told my pain management doctor I was going to start using medical marijuana, he said “good, because these things (opiates) will kill you” . I got off those things five years ago and will never, ever take them again!

I have this exact condition (RL4/5 stenosis with idiopathoc scolitosis after my career as a dental hygienist). Kudos to those still working in that capacity past 40... superwomen! Wow! The injection helped the numbness and leg symptoms, but the axial pain is still there. I did rest after my L4/5 injection. I think my husband... "The spine guy" does not see me as a patient... I'm the "compaining wife"! ☺️ I need to come here to find "real doctor" suggestions.... lol! Cold compresses while resting and wearing a flexible brace during necessary cleaning and chores are great ideas. Thank you! What is your opinion on nerve ablasions? I've done 1000's of hours of PT, 2 facet injections... yet continue to have this super annoying axial pain... Ce la vie.. Btw.. I love how you are exactly calibrated with my husband! Less surgery, give it time and possibly more PT! Wish there were more docs like you guys out there!

I had three epidural injections 4 weeks ago, and the bilateral sciatic pain has gone since, but the axial pain is excruciating at times. I've been a dentist for almost 20 years. Had to stop working a week ago. Are you able to practice? I'm devastated ATM. By the way, my spinal stenosis is congenital, first symptoms December, MRI scan end of Jan., March, I'm out of my job. Thank you for this video doctor. I was diagnosed with Secondary Central Canal Stenosis. MRI says c5-c6 disk mildly impinging the cord hence causing it. However no obvious features of compressibe myelopathy. I'm 22 and I've been taking the medicines and the pain has improved a lot. My question is will it get worse with age? Cause I'm only 22. This was an excellent presentation covering Western medicine. I am curious what studies you derived these statistics from. I’d like to add that there are also some complementary adjunctive therapies that may help patients improve. There is a mind body connection holistic approach of personalized biopsychosocial add ons including weight management, nutrition, nutricuiticals, psychological therapy for the challenges, of dealing with pain and certain types of meditation or prayer disciplines such as mindfulness meditation. Thank you for the comment. There was a series of level 1 data covering thousands of patients starting in the early 2000s that looked at these specific diagnoses: lumbar disk herniation, stenosis, and spondylolisthesis. It's the best data we have to give general statistics to patients. If you are interested it is called the SPORT trial and all the articles are free to read. I certainly agree with you that complementary adjunctive therapies can go a very far way in terms of helping patients both before surgery and after but also sometimes eliminating the need totally for surgery. The great thing about most of those is that they almost certainly cannot hurt the patient and can only help.

I am only 47 and have diagnosed with SS recently, my doctor mentioned naproxen ( Advil) but I don't want to take anti inflammatory drugs if it's just going to cone back when I stop, should or could the anti inflammatory drugs actually make a permanent physical change if I take the course thanks.

Fantastic question. Pain (in general) is caused by 2 things. Physical compression and the body's reaction to physical compression which is inflammation. Naproxen really helps with inflammation type pain. Sometimes patient's pain may be a mix of both physical compression and inflammation. If there is more inflammation causing pain compared to physical compression, things like epidural injections, naproxen, and other things like physical therapy can certainly help. Sometimes patient's pain is coming all from physical compression because none of the above things help them.

@@ArmaghaniSpine For me, NSAIDS work immediately. But my pinched nerve which exits L5/S1(?) keeps getting reinflamed every time I drive to or from work, which is only 15 minutes. Sitting is my enemy. I've been suffering like this for 4 years. Did the PT, and did several injections with no significant help. Finally the doctor just prescribed NSAIDS and Gabapebten and wished me the best. Gaba is no help and I'm afraid at my age to live on NSAIDS. I'm going to a new doctor next week to ask for his thoughts on a foraminotomy. My physical life sucks.

I have LSS. I used to take a ton of naproxen, but I tried to switch to a more natural pain relief through physical therapy. It really helps you strengthen your muscles and help you manage your pain.

Am approaching 84 with lumbar stenosis, and have tried conservative and epidural but pain still slowly increasing. Would surgery be unwise at my age, even minimally invasive? Also a medial branch block has been suggested. Is that likely to help? Would be very grateful for your comments.

All excellent questions, Henry. Age truly is just a number and there are 84 year olds who are as healthy as some of my 64 year olds. Some of the things we consider is what is the amount of surgery necessary to maximize outcomes and minimize risk. At your age, in general, smaller, single level, minimally invasive surgeries give the best mix of maximizing outcome and minimizing risk. Sometimes this isn't always possible though. As for the pain procedures, medial branch blocks will only really help with back pain, not pain going down the legs. For patients of your age, or any patient for that matter, if you have much more back pain than leg(s) pain, I highly discourage surgery.

There have been all sorts of things tried to close the hole after a microdiskectomy but none have shown to work. That's why we limit activity for 6 weeks and allow the body to scar over that hole and then lift restrictions. Lifetime chance for a reherniation is about 10%.

Great questions. In general, there is an 85% patient satisfaction rate with surgeries to treat this condition, meaning that 1 year after surgery, 85% of patients are happy they went through with the surgery and would do it again. That does leave about 15% who aren't satisfied and we haven't quite figured out how to predict who those patients are. I always suggest getting a couple opinions and weighing your options closely. I find that an educated patient almost always makes the correct decision regarding their care. Good luck!

I definitely have spinal stenosis omg the pain is unbearable, and yes I often several times a day I have to lean over to try to relieve the pain and having 7 herniated disc makes it even worse, it's been going on since 2008 when my ex-husband kicked me in my lower back and in 2006 he pushed my legs apart to far and caused me to have a pinched nerve and damage to my right hip wich he kept hitting on over the course of many years,and he hit me so hard it caused herniated disc in my csevicL spine
