For a total hip replacement, it’s very important to know, and abide by, your precautions after surgery. Listen to your surgeon and physical therapist and do what they tell you to do because they are the most familiar with your particular situation and know the things you should do to recover as quickly as possible. There are three big precautions for a total hip replacement with a posterior approach. These three precautions are don’t bend past 90 degrees of hip flexion. So that means no bending down to tie your shoes or pulling your knee up towards your chest to put on socks. The next one is no hip adduction. This is crossing your leg across your body, like crossing your leg over your knee. The last one is no hip internal rotation, which is turning your foot inward. All of these put a lot of pressure on where the surgery site is and where the muscles are very weak, so they are important to follow.
Now I will show you some isometric exercises you can do very soon after your surgery. The first exercises are quad sets. Sit in long sitting with your legs straight out in front of you. If you want, put a rolled towel underneath your knee to give yourself a target. Then squeeze your knee down into the roll towards the ground. Hold it for 3 seconds, and do ten of them.
Now is a hamstring set. Bend up your knees in a hooklying position. Push your heel down into the ground and hold it for 3 seconds, do this 10 times.
Then you will do glut or butt squeezes. You are basically trying to tighten your buttocks muscles, almost like if you have to go to the bathroom, and you are trying to hold it. Start off with 10 of these with a 2-3 second hold.
The next two are hip adductor and abductor isometric exercises. First take a ball or pillow folded in half, and put it between your knees. Squeeze into the ball and hold for 3 seconds, repeat 10 times. Then take a belt and wrap it around your thighs just above your knees. Push outwards toward the belt like your legs are opening up like a clamshell. Hold for 3 seconds, and repeat 10 times.
Then you will do an ankle pump. You can prop your ankle up on something if you want, but keep your heel free to move around. I am using a roll, but you can just hang your foot off of the bed or stool. You are going to pull your toes up at your ankle, and then push down like you are pushing on a pedal. This is dorsiflexion and plantarflexion. It works as a natural pump to get the fluid and inflammation out of the leg, so you can do this several times a day. It can also help prevent DVTs, so this is very important.
Finally, you want to focus on the way you walk. Getting back to a normal gait pattern is very important. When we walk normally, our heel hits first, we roll through our foot, and then we push off with our toes. If we don’t get the heel strike in the beginning of the step, we tend to limp. If you are still limping, then you still need an assistive device like a walker or cane. If you walk with a limp, then your muscles will get in the habit of doing it, even when your muscles are strong again.

A follow up. I'm 2.5 years post op. I can do everything I could do before OA took it slowly away from me. I got a trainer at a gym and a pt and did regular follow ups and I do strength training, balance and spin bike almost daily. I can hike up a steep mountain trail with my dogs too! Trekking poles were a great addition to my recovery protocol.

G'day Dr Joe I am 63 yo and I have had a left hip replacement. I have also had both knees replaced I get a bit nervous that if I move a certain way that it will "pop" out of the socket at the moment it is very bruised and swollen down to my knee, I am really glad that I found you on KZbin as you make the exercises easy to understand. Thank you for showing how you do it after a posterior replacement kind regards John

Excellent teaching techniques the. Dr. Jo. It helps remind me every day to my exercises. Any good suggestions for those of us who have had their knees replaced and now have our first hip done. I know that both my knees have gotten stiff because just before surgery I was unable to walk. A month earlier the ball in my hip collapsed 3 days after having a hernia repair. Man I’m a mess! My hardest time is getting in and out of bed. It was the same in the hospital. The beds are too soft and scooting isn’t in my vocabulary anymore. I can do it to the side but not backwards and forwards. Help. What can I do to prevent hurting myself. I do have a grab bar at the left side of my bed. Same side as my staples all down my thigh and same side I’ve been getting out of bed! I have osteoporosis.

Very good video. The best I've come across so far. Thank you! I can't seem to find any information on the time span one needs to be careful on non-crossing your legs and the 90° angle restriction. I really would love to reach my toes again. I couldn't touch them for at least three months before the operation!

Thanks, and you're welcome! Usually your surgeon will clear you of the precautions after the first or second follow up as long as there are no complications. Each surgeon is different, but it's usually anywhere from 6-12 weeks, but sometimes a little longer.

@@julianolucca4202 Hi Juliano, thanks for asking. I had problems reaching my toes/shoes many months before the operation. I mostly wear sandals, and the occasional pair of shoes, when it rains, are slip-ons. I also have a shoehorn with a long handle. An equivalent question would be when I finally could cut my toenails without problems. I think it was around 6-8 weeks after the operation. My main worry was hip dislocation after the operation. I've heard horror stories about that. But I think the only worry is having a fall, resulting in a dislocation. Now, one year after the operation I feel 100% again. It gradually got better over the months.

This is one of best with a very clear explanation for each exercise . I am on the end of the week 4 post operation. I still need a crutch to walk. Then I find it’s very helpful to follow the gait practice. I can manage to prevent typical limping after a few minutes practice !!!❤ How important it is to squeeze the hip to walk on. Thank you again

Love you Dr Joe. Just had surgery in NZ, and there is a lot more information here than I was given by the hospital. Do you have any recommendations for very tight psoas? I am 10 days post surgery, walking w normal gait and 1 crutch, mostly for balance, maintaining all exercise w no difficulties.

I really appreciate the expertise and advice you have for building strength and gaining proper gait after surgery. I am going into my second hip surgery in a few months. My first was anterior (my surgeon retired) and my second will be posterior. I'm nervous because I am a competitive rower. Is there still, in 2022, a huge percentage difference in dislocations with the posterior approach? I need this surgery to be highly successful, as my winter/spring training is crucial for another successful rowing season.

Thank you dr. Joe for clarifying that after hip replacement surgery that it is okay to cross you knee over to the outside. I have a total knee replacementon the opposite side and really need to do your pirformis exercise for my back. I also have severe disk degenative disease in a couple of disks.

You're welcome. You surgeon will need to clear you of those precautions, but after a certain period of time, it is possible.

Really good, thank you. I do a lot of ankle pumps as well as a routine of other exercises & I've had no ankle swelling at all. It defiantly works well. I'm at week 4 post op and doing well. It's good to what you Dr Jo as a reminder to do my physio the right way.

I had a knee op and the foot on that side leans in towards the instep so it makes me still limp. When I saw the heel to toe step I immediately tried it. Fantastic. I'm gonna tell my physio about it as she said I may need a built in insole because my foot isn't level. I'm incorporating that step into my exercises I've been given. Thank you😙

My wife's physio is shut down because of COVID-19 a week & a half post surgery for several weeks. She does not want to let up on the steps of physio & fall behind. What are the stages in time for these exercises (day 7-10 day 11-17 day 18-26 etc). Great video by the way. Nice that there is no silly music like most others...

These are very basic ones that she should already have (hopefully). Unfortunately, I don't know what specific stage they would be in since I can't evaluate her and everyone is different depending on how well they are healing and what all their surgeon did. Hopefully her physio will be able to call her and help her on her progression.

Thanks Dr Joe for the info ! I had posterior total hip replacement on my right side. I think everything is prob going well considering my surgery, it's been 17 days today...it kinda feels as if my incision site is tight maybe lil numb. I'm sure it's just my muscles trying to stretch again .,

You're welcome! If your incision site is completely healed with no more scabbing, you can start scar tissue mobilizations to help with that.

Actually I may be misleading you....I meant I have swelling about three inches diameter with my scar in middle. Does that still classify as "harden scar" ?

How soon after surgery can you start doing these exercises? I assume we don't do any 'floor work' ;-) But what also can I do for other parts of my body? Are there stretches to for upper body, lower back and arms/calves etc? The body is pretty tight and sore all over...

My understanding, is that I have to have right hip replacement and I'm not having hip replacement until after the New Year on top of that I have sciatica pain on that same side. My question is will the sciatica subside after my hip replacement?

What would you consider the earliest point to introduce the active (no resistance) side lying clam shell post THA (posterior/lateral approach)? Likewise at what point post THA would you use the theraband resistance during hook lying clam shells (same approach)? 6 weeks? Thanks

I think it can be earlier than that (even just a week or two out), but it really depends on how much strength and control they have. I would start with a manual resistance in hooklying first. If you feel like they have good control without pain, you could start with the lightest resistive band. Once they can get to 20-25 reps of a green or blue, then you could do sidelying clamshells, but start off the same with manual resistance or no resistance to make sure they are doing it correctly and activating their gluts (med and max).

Hi Doctor Jo I just had hip replacement on 9/16/20. I've been in bodybuilding for 40 years so I have a lot of muscle. I'm having a problem getting up after sitting. My hip is stiff. What is the best exercise I should do for this? Thank you for all your videos!!! They are wonderful!!!

You're welcome, and thanks! That's pretty normal that close from your surgery. Sometimes, something as simple as heel slides, heel taps, or kicking out your leg a few times might help loosen it up before standing. As far as exercises, if it's a posterior approach, the above video might help.

I appreciate the gait training as walking with a bad hip sure creates a bad movement from shoulder to heel! How often are you supposed to do these exercises daily? All of them everyday? How many times a day?

It depends on how far out from the surgery, if they are still on precautions, and a number of other things.

Hi Dr. Jo! Love your videos! I am 48 years old 5 weeks post-op posterior approach. I have a tilted pelvis post-op which makes my operated side feel longer. My doctor said I don’t need PT to just walk. Any exercises you can suggest that could even out my pelvis that doesn’t break my 90 degree restrictions??

Thanks! I hate to hear your doctor would say that. Just walking really doesn't give you the best results. The above video should definitely help. Do you feel like the tilt is from hip flexor tightness? I have a question , is adduction activities precaution in both posterior and anterrior approaches​ or just in posterior ?

Not usually with anterior unless the surgeon had to do something different during the surgery.

It's not clear whether forward bends and crossing legs, etc. are okay after 6 months post-op. If the doctor says you can do what you want without pain, is it then okay to do any movements including forward bends and yoga, etc.

It's usually 6 weeks, but yes, it's definitely up to your doctor. Precautions are there for a reason. I have seen people in the clinic re-dislocate their hip because they were breaking precautions. I really think it only happens if there is something wrong to start with, but in my opinion, it's better to be safe than sorry. Good luck.

anyone know if these can be done at home? or are you required to be at a rehab place with a physical therapist for a month or two? cuz i'm thinking, at a rehab place you may get an hour training per day but at home you can do them more frequently at your own time. what's the truth? am i wrong?

Hi guys, I'm 22 and had a total hip replacement exactly a week ago ( last Tuesday ). I'm a stomach sleeper, does this mean I can NEVER sleep on my stomach again?? Or is this just for the recovery stage? Also, I feel as though my operated leg is much longer, advice?? Loved the video

I had my replacement done slightly more than a year ago. My doctor still says I am not allowed to do all the actions that you have mentioned in your video. My question is when can I do all these actions?

Hi Dr Jo. Your video is extremely helpful. I only wish I had seen it sooner! I was 2 was post surgery ( posterior) & was able to do more weight baring & use a cane. Unfortunately, a few days after my dr visit, I went to pick up a Xmas ornament from a lower table & had a hip displacement. Ouch! The ER was able to get it back in, but obviously I regressed in my recovery. There is not much literature on what to do after a displacement. I have, thus, been fearful to move much. It is 4 weeks since displacement & it seems that I should be doing your recommendations. I am worried that the displacement has negatively impacted on my recovery. I was not referred to PT. I am an active person, but don’t want to overdo now that I feel better & want to follow exercises that will strengthen my gait. I would appreciate any advice from you! Thank you so much!

Thanks! So sorry to hear you had a dislocation. Usually you can start gentle stretches and exercises right away, but it really depends on your hardware and how stable it is now. I would call and ask for a PT referral. I think it's really important, especially with the setback. Good luck.

AskDoctorJo Thank you for your reply. What would need to be done to check on “my hardware”. It doesn’t feel as tight as it did prior to displacement. Would physical therapy help bring it back to where it was? And how long would you recommend I wait prior to seeking an evaluation from an ortho surgeon. I don’t want to settle & be worse off than I was when I started! Could you tell me where I could get additional information regarding displacements? I’m so happy to have found you, as I feel at a loss in where to go from here since I have not received direction or detailed info from my surgeon’s office. I’m essentially looking for reassurance that this can still heal properly & willing to do whatever it takes. Thank you so much!

Have you not seen your surgeon since the dislocation? I definitely think PT would get you back to where you want to be for sure. I think the exercises in the video should be fine, but honestly, I don't know for sure without getting to evaluate you. You can definitely still heal properly, and get back to doing the things you want, but I really think you should get a PT evaluation so they can make sure everything is good, and ready for you to continue with your rehab.

Bless you DoctorJo! Thank you for your words of encouragement!! No, I have not seen my surgeon since displacement. That is a bit of a sore subject..... After my 2 wk f/u, I was given a 6 wk f/u appt. The displacement occurred 2 days after my first appt. It took me 4 days after that to get a return phone call to let me know his review of my ER visit. At that point, he told me it was put in correctly & would see me at the reg scheduled appt., despite my request to be seen sooner. So, for 4 weeks, I have just been “winging it”, keeping the restrictions, but uncertain & quite frankly afraid to do much of anything. I was not even sure if I could try to use cane & after seeing your video, I realized I have been walking wrong & bending my legs in bed wrong. I’m so happy to hear that PT may help me get back to where I was! As I previously said, I wish I had seen your video sooner & written you. You would have alleviated my 4 wks of anguish! I will request the PT referral for sure! Thank you again!! 🤗

Very informative video. I was interested to hear about the associated knee pain. I had a knee replacement op 3 years ago and 6 weeks ago I had a THR op on the same leg and have been suffering a lot of pain in that knee. I am hoping this is going to get better with time! Is this something you can reassure me will happen and will these exercises help? I have religiously been doing the exercises shown me at the hospital but the pain doesn't seem to abate.

They are specifically for posterior, but usually there are not any precautions for an anterior approach because they don't have to cut through major muscles, so they should be fine to do.

How long after the surgery can we try walking without the cane? Is it just a matter of having the muscles ready and holding the weight as you’ve shown? Of there is any precaution we need to have before, like not putting to much weight on the replaced hip...?

It depends on a few different things. The main one is if it was an anterior or posterior approach. Are you going to physical therapy for it because the physical therapist should help you progress off the cane. My rule of thumb is, you need to have a pretty normal walking pattern without the cane. If you are still limping a lot without it, you are not ready to walk without it.

Many thanks for your prompt response! :-) Yes, I'm going to physical therapy, and the protocol they are following is very similar to your video. I did the posterior approach... so I know the rotators were cut and stiched back in place... hence why all the precautions... But until 1y before the surgery I was very active... cycling about 200km per week with long rides on weekends. Always practiced sports, and I'm young for this surgery (i'm 41y). Today I completed 12 days after the surgery and I'm feeling amazingly well. No swelling, no pain, and walking very well with one cane. I can do all the exercises in physical therapy, and even some with weights... I have the feeling that doctors in Brazil are overly conservative with the recovery protocol... he asks to walk with two canes for 4 weeks, and one cane for another 4 weeks, before walking freely... I can notice I'm still not fit enough to leave the cane (I'm limping, not a lot though, not like you described in the video), but I'm more than well to use only one. And I can feel that in another week or so, I'll be able to walk without limping (I can feel by the improvement during the physical therapy). So my question was more to understand if there was some "no no" for trying to walk without the cane if I was feeling well... or if I could try and see day by day how may strength was improving and if I could leave the cane... You're welcome! Usually it's about 3-4 weeks for most people to start weaning off the cane. I wouldn't push it too much even being that you are young and healthy. Sometimes you just have to let biology do the work, and let your body heal. You are probably okay to try short distances, like around your home, but I would wait until at least 3 weeks before you start longer distances without one. Good luck!

Thanks for the video the one thing I did not want was a limo unfortunately I had a prosthetic fracture in my inner femor bone 1 week after THR surgery recovery has been hell and it cause meniscus tears in my knee now I have so much weakness I that leg when I put pressure on out out hurts causing more to limp..My hips don't hurt anymore but what can I do about the tears and recovery and strengthen that area. I believe I would be walking fine of out were not for that any advice will give insight... Thank You

Hi I had a double hip replacement 4 weeks ago and I was wondering what your take on walking at this stage, ie amount of times a day?, I'm currently walking with my crutches once a day for about 25 mins, I'm Now ready to get rid of my crutches and begin to walk in aided, my Physio has said if I feel ok then go for it, what do you think?

As to this video i'm hoping to get an anterior approach, would these exercises be appropriate as well? Or are there better ones? Sx in about 6 to 8 weeks. I've been dealing with this pain since 2009 but really bad for the past 3 to 4 years, I finally went to a surgeon and the x ray shows that my ball is very flat and i've lost 1/2" on my left leg. I've tried a cane sporadically but was doing it wrong. My question is, with the poor muscle use as well as underused will using the cane properly help undo some of the damage i may've already done?

Getting stronger might help reduce some of the pain, but it won't fix the damage already done unfortunately. I would strongly recommend going to physical therapy before AND after your surgery. They can help you prepare for everything you will need to know and ways to modify things at home after your surgery.

The precautions would be different, but early on the exercises would be similar. Usually with a labral repair you are only toe touch weight bearing for the first few weeks.

Oddly, I was told crutches for a month but on my follow up on day 6 the Doc said no crutches after 2 weeks. And I'm getting incomplete info on exercises... Love the precautions here.

Sorry to hear. Some protocols are different depending on the doctor who did it, but I would think maybe your doctor was confused on how far out you are. Most of the time, you are on crutches for 3-4 weeks, and you can progress weight bearing and go down to 1 crutch at the 3 week mark if you are doing well. Also, for the first 2 weeks, you should not bend your hip past 90 degrees or externally rotate your hip (turn your foot outward). These will not injure anything, but it would possibly irritate the surgery site. Are you in formal physical therapy now?

The only ones you shouldn't do are the ones I mentioned. No hip flexion past 90 degrees (bringing the leg up past your waist), no adduction (crossing the leg across the body), and internal rotation (turning the foot inward). Unless your doctor/surgeon has told you otherwise, you can do all other movements. Usually the precautions are only for a few months until the muscles get strong to protect the joint again, but your doctor should clear you of them when you go back for a follow up. Good luck.

Hip replacements are the easiest joint replacement to recover from (in my opinion). So as long as you are consistent with your physical therapy before and after and have no complications, you should do great and feel better after you recover than you did before your surgery. Good luck.

It really depends on how much motion you had before the surgery, and if you were able to do it before. In general, people usually have enough motion back, but it's very different for everyone.

Great video thank you! My surgeon told me to do most of these exercises but I'm eternally thankful for what you said about concentrating on heel - toe approach when walking and to keep using your cane . I'm at the 2 week mark now and I thought I was doing great by not using my cane and limp / wobble around! NOPE! Thanks for the tip , I'll stick with the cane and concentrate on a heel toe approach when walking.

As long as you follow your precautions until your doctor says you can stop them, you should be fine.

Either way, being consistent with physical therapy is the key to a quick recovery.

@@AskDoctorJo I'm starting already with pre-op physio! I just wish it didn't hurt so damn much. NSAIDS are destroying my gut and the pain is driving me crazy
