My view of the trolley problem is that if you don't pull the lever and you make the choice not to pull that lever you are still directly responsible for the deaths of those five people. You had the opportunity to save them easily and chose not to. And if you did that to absolve yourself of some kind of guilt thinking "I didn't do anything so it wasn't my fault and I didn't actually kill anyone" then that's wrong. In that case on a moral level being responsible for the death of one person is preferable over being responsible for the death of five.
