Thank you for sharing this. I am a third year medical student, and just learned about obstetric cholestasis today. I like to check the internet to get the patient view of an illness, so that's why I watched your video. I want you to know that your story was memorable, and if a pregnant woman ever comes to me with itching, I'll take her seriously. I also wrote down the diagnostic criteria: Anything over 14 micromoles/L is considered OC. You were heard! Truly, future Dr. Luther

Wow thank you for sharing your story! I am 32 weeks and was diagnosed with OC 4 days after I had called my doctor complaining about itching. They were very proactive. They got me right in the next day to test my bile salts. One week later, I visited a specialist and now we are planning to induce at 37 weeks. The itching, frustration, and anxiety wondering if your baby is ok is stressful enough, I cant imagine having to go through what you went through. Even though it's a rare condition doesn't mean doctors/midwives shouldn't take it seriously or at least research for themselves. Thanks again for sharing!

This is so insane! I knew what cholestasis was and I was itching but I didn't think it was bad enough to mention, but then at my 33 week app my doctor noticed me itching and asked some questions, and he sent me in for the test just to be safe -lo and behold it was positive! We started treatment and scheduled the induction for 38 weeks. The sucky thing is that I most definitely had OC with my first pregnancy too, probably even worse, but when I mentioned it to the ob I had at the time, she said it was probably nothing. Our baby girl came at 38+4 all on her own, but I shudder to think what could have happened if she cooked for longer, the cholestasis definitely would have affected her if she hadn't known to come out on her own! Thank you for sharing your story. Doctors need to listen to women about their own bodies!!!

Hi, I am diagnosed with ICP.. I live in India, but i am currently in Canada to give birth.. I had itching to which I though it normally happens with pregnant women in third trimester, and my mum had it too in her last pregnancy with my brother and didnt take it seriously untill I contacted my gynac in India and told her my concerns as I wanted relief with itching and my mum was flying soon to see me so I could get any topical ointment or lotion to put which eases my itching.. which ended her telling me that she thinks its cholestasis and I should contact my doctor in canada asap.. then I started researching about Cholestasis and came to know about this in depth through your video.. I really really thank you for this video, it gave me a clear understanding and a course of action which needs to be followed if in such situation.. Im so grateful I came across this video.. I am in my 33 week so I had plenty of time before things could be really serious.. I tried speaking to my maternity clinic and they did not hear me out at all saying all that i was saying were not symptoms of cholestasis and that they could not write me the tests and I had to get back to my GP to get diagnosed by them first.. So my GP heard me and wrote me down the liver functions tests which came out the way they were expected and then the maternity clinic heard me and further wrote down the bile salts test as per what you suggested and the course of action has bedn followed.. I thank you immensley that through your video I got the strength and knowledge to fight through.. I cant thank you enough for this..

First time negative, a month later 2nd time positive at 38 weeks was induced immediately. I have ICP for the fourth time. 1st time (1st pregnancy/child) minor itching started like a day before my due date. My dr took me seriously and tested my blood. I was induced at 41+0 weeks and was informed we were inducing for post dates but also I should know for future reference that I came up positive for obstetric cholestasis and it was likely to repeat with subsequent pregnancies. 2nd time (2nd pregnancy/child) itching started at around 35 weeks. But this time, the itching wasn't a minor inconvenience this time, it got worse with each passing day. My midwives saw my history and immediately tested and a week later the results were positive for obstetric cholestasis. They referred me to an OB/GYN (Canada) who was really nice and he schedule and induction for 37 weeks. Within about 48 hours of meeting the OB/GYN, my itching ramped up, I was taking scalding hot showers and freezing cold baths to numb my skin. I wasn't able to function. My husband called the midwives. We got a call back from the OB/GYN to come in straight away. The Dr had me get the blood tests but didn't wait for the results, he started an induction (36+6 weeks) and was nice enough to let me stick with my midwives for the delivery. Birth went well, but my son wasn't okay. He spent weeks in the NICU and was traumatic watching him. Results came in and I was informed my bile acids were over 100. 3rd time (3rd pregnancy/child) I tried dietary control from the beginning of the pregnancy but also timed the pregnancy for the summer because obstetric cholestasis is less common in the summer than winter. Itching started at 39 weeks and induction was scheduled for 40 weeks before the results came in. Results came in the day before scheduled induction and it was only barely positive for obstetric cholestasis. I went into labour naturally the same day and had a home birth. It was such a more positive experience than my second child having a summer baby. March baby was the hardest. My third child died of FLHCC at 12 years old (liver cancer). Unknown if connected to obstetric cholestasis, his cancer was a lot rarer than ICP. With our older 2 heading off to college soon, we decided to try for another baby. We had low expectations but luckily I'm still fertile in my early 40's. 4th time (4th pregnancy, twins) and little itching started at 24 weeks. I'm going to a hospital that specialises in multiple pregnancies and they see obstetric cholestasis all the time because it's more common in twin pregnancies. But also with my track record, they said from the beginning ICP was probably unavoidable. My first blood tests showed elevated numbers but below cholestasis diagnosis. I was given a prescription for Ursodiol and was asked to hold off on starting it until my itching got worse and to let them know when I start it so I could get blood tests. 28 weeks the itching got worse, I called to let them know, Maternal Fetal Medicine Specialist advised to start the prescription and to come in to get bloodwork. Sure enough, acid bile had risen high enough for obstetric cholestasis. My MFM has said current research shows under 40 is lower risk. Over 100 is high risk and induction by 34 weeks. Between 40-100 induce 37 weeks. We're trying to keep my numbers below 40 with prescription and diet. I go in every 2 weeks for ultrasound and my bloodwork is being done. Love that I can get my bloodwork results same day at this hospital. I'm 31+6 weeks, twins are in 65th percentile and I'm still below 40. We'll see if we can keep those numbers below 40. Aside from my age, twins and ICP, I haven't had other risk factors come up. My blood pressure is good. Passed the glucose tests. Cervix holding solid. We're doing great so far! No commitment on induction schedule so far. If I shoot up over 100 again, we'll be looking at an induction by 34 weeks. If I go over 40, we'll be inducing at 37 weeks. If I can keep my numbers below 40, we might be lucky enough to go to 38 weeks. I feel like my midwives and dr's have been listening to me over the years and what they've said has always matched current research/data available. So sorry to hear that wasn't your experience.

I'm so thankful for people sharing their stories like yours. Thankfully, my medical team was way more responsive. I was just 38 weeks when I started itching, so at least I was full term. I called on a Sunday when the itching started and the on call doctor immediately ordered the labs for liver and Bile Acids. However, the lab didn't open til Monday. Then the bloodwork was supposed to take a few days, so I had an appt Thursday. Half the bloodwork came in by Tuesday. The Bile Acids were not back yet, but liver was looking not so normal. So I called and asked if they could move up my appt. My medical team thankfully opened something up that day. Even without the Bile Acids, the liver function and symptoms were concerning enough, they scheduled a c-section for Friday (I had two previous csections. I really wanted a VBAC, but with the previous csections, they couldn't use relaxants for induction and my cervix was too closed for a balloon. I just wanted baby out like you, so I sucked it up and said do the csection). Baby was born safely on Friday. I only had to wait 5 days and it was torture. I feel so bad you had to wait so long. I got by Bile acids back the day before the c-section and they were 15, so I am glad my team was proactive a scheduled it. Did you continue itching at all post partum? I know they say it goes back to normal. I am itching less, but still have short episodes. Perhaps it is the pain meds, but just curious if you had any lingering symptoms postpartum. Glad your babies are safe. It is terrifying knowing within hours, your womb can become toxic and having to wait days or weeks.

Your story really hits home for me, even though there are stark differences from my own. While i was lucky enough to have an OBGYN who had knowledge of ICP, I did have a number of other conditions that, all together, made induction at 37 weeks to the day pertinent. I had hyperemesis gravidarum, borderline gestational diabetes, the ICP, and my son was considered to have IUGR. Luckily he only spent a few hours in the NICU and was a generally healthy 4 pound 10 ounce baby. Otto was very slow to grow and catch up developmentally with other babies his age, but at 10 months old he has finally caught up and I couldn't be happier.

I know this video is from two years ago, but I am going through something similar to your second story right now and I feel like I’m going crazy! This is my third pregnancy. I had ICP with both of my previous pregnancies. Early onset with the second. They caught it at 24 weeks. This time around I started itching in my hands and feet some during the first trimester. During my second trimester, I started having gallbladder pain and attacks, on top of still having the itching (which had started to get worse) especially at night while trying to sleep. I was digging my skin raw in spots, almost to the point of bleeding. They kept doing blood tests but kept saying that my bile acids were within the normal range. I’ve had an ultrasound on my right upper quadrant which showed gallstones and bile/sludge backed up in my gallbladder. They did a liver function test last week, which showed that my alkaline phosphates are elevated, another marker of ICP. And my bile acids are 13.1. But they don’t consider it above normal until 14. It’s just making me crazy and terrified because I know my body, I KNOW the itch. Like you said, it’s SO different than a basic itch! And I have the history and every other symptom. It scares me so much because I know that my bile acids can go dangerously high pretty much over night, but the blood tests take about a week to come back. So our son could potentially go into distress one night while I’m sleeping, and I wouldn’t notice his lack of movement until it’s too late, all because they are waiting to get THEIR confirmation of what I already know I have. Hello, I found you through my cholestasis of pregnancy research. I have not been diagnosed yet but have been itching for 3 days and being a nurse and doing tons of research I know this is cholestasis. I go to my doctor on Monday to run the liver function tests and bile acids and I just know this will be the outcome. Nothing is worse than the torture this itching is. I felt silly telling my mom about it but it really is taking over my life and so incredibly difficult not to feel depressed about all this. At least my midwife has taken my complaints seriously and are getting the tests they need to diagnose me. I’m only 25wks so more than 2 months of this. I’m sorry you went through such an awful time with your healthcare team.

I'm going to see my Midwife next week I will mention to her. do you think I should tell her earlier?

It should be okay to wait for your appointment but I'd definitely ask for liver function and bile salts tests. How did you get on?
@jesslee83588 жыл бұрын
Hey there. I saw my Midwife. She's not convinced that it is ICP. Because my itch is not on my palms or soles of feet. Its all over my body and my fingers and around my toes, soles of feet and heels. My urine is not dark and poo is not pale but how do u tell if u r on multivitamins and iron tablets?? Lol. Itch has been abit better. Some days are worse than others. Some nights I wake up with itch and can't get a good sleep. I'm not 100% certain of I have ICP but if I missed it and something happened, who's ti blame? Took that approach my midwife agreed to do LFT for me but was very reluctant to do bile salt level for me. Not sure if its because its more expensive or what. But I did push it and she gave in. But I need ti wait till 36 weeks to do them, along with my CBC and antibodies test.. as long as I know the result before 37 weeks I guess would be OK. But can be quite nerve breaking everyday especially today my baby movement has been sluggish and not as strong :( its funny how if you are not diligent, they say you don't care or not being a good pregnant woman. But on the other hand , when you are being too careful they call you a crazy pregnant woman, isn't it?
@AThousandWords8 жыл бұрын
I hope you are having the tests anyway!! I was told I didn't have it and I actually did. Outward symptoms are not the full picture, and not everyone has the symptoms your midwife mentioned. I'm so glad you pushed to get the tests run!! No idea why she is making you wait to have the tests run ?! So weird. If you are concerned about your baby's movements definitely push for getting checked out!!! DO NOT let anyone bully you or say you are being crazy, you know what is normal and they should take you seriously if you mention reduced fetal movement.

It is scary! I had Cholestasis as well with my first and only (to date =D) pregnancy. I started getting very itchy in my third trimester maybe around 34-35 weeks I dint think this was anything to worry about I thought it was due to my dry skin or laundry detergent. I was itchy mostly in my feet and legs, super annoying nothing relieves it. On my next appointment I mentioned it to the doctor ( I had been seeing midwives all along, but on that day I saw the doctor) I didnt think it was a concern because the itching was not extreme, the doc asked me how I was doing and I said ok then she started listing symptoms I said no then she mentioned itching and I said yes! she then sent me to get my blood tested but she never said what I could possibly have. They were supposed to call me in the next week with results but they didnt. The itching became worse each day especially at night, the night before my last appt I couldnt sleep I was itchy in my legs, feet, hands and vagina still I didnt know what it was and never thought it was a real condiition as no one had ever mentioned it. Day of my appt I saw a different doctor, mine was on vacation, she checked the baby, asked me if they had given me the results for my itching I said no then she goes to find them and says "your bile...? is abnormal you have cholestasis. did they give you medicine?" I say no I dont even know what that means. and she says "you are 37 weeks already you have to be induced today or you risk a stillbirth, I'll call the hospital get a room ready for you" at this point im so confused, ignorant of the condition, i dont know what is going on I just focused on "stillbirth" "induce today" shes rrushing to get me a room set up, calling the hospital etc while im sitting there trying to process whats going on. I ask what it is I have again and ask her to explain the situation she then half explains using medical terms I dont understand tellls me my blood is being poisoned pretty much and theres a high risk for stillbirth and I need to be induced ASAP. so leave scared, confused and totally unprepared for labor, instead of going to the hospital I go home tell my husband the situation twice, call my parents and rush to get a backpack of clothes before going to the hospital. It was so scary! Thankfully my son was born perfectly and labor went well. Later I researched Cholestasis came to know the condition and have tried to tell other pregnant women about it to educate them on it since many women dont know about this. My mom had 7 babies and had never heard of this. Im glad your babies are good and its a bummer theres a high probability of recurrence =( as we are planning on another baby!

I felt absolutely ridiculous going into the er saying "my hands and feet itch" but thankfully they sent me up to labor and delivery. 7 hours later, 2 needle pokes, and 10 viles of blood, and they told me to take benadryl until my results come back until 3 days 😩 obviously I hope I don't have cholestasis, but the other part of me is scared everything will come back "normal" and then I won't have answers for why I'm having this itch that is driving me insane.

This was so different from my experience. I told them my symptoms, they ordered several tests including bile acid and salts for that day, I got the results the next day, we immediately set up fetal monitoring, ultrasounds, high risk ob appointments, and further bile tests for later, and began planning my induction for 37 weeks. My midwife and the staff immediately had taken me and my cholestasis research seriously. My story happened in the US though... and I wonder if socialized vs. non-socialized medical care was a factor in how my experience was different? I also though... wasn't just in the US, I was in Utah where we have a significantly higher birth rate than other states. I wish your experience had gone as smoothly as mine, and I hope this becomes a routine check for pregnant and well women exams. I think I may have even had cholestasis before my pregnancy. But no dermatologist mentioned cholestasis as a possibility. I don't believe blood work was even ordered (can't remember). Thank you for helping get word out there!

@AThousandWords8 жыл бұрын
If you're desperate and can get the blood tests run by going into hospital now then go for it. Beyond the blood tests unfortunately there's not much they can do for you immediately. The top priority is getting the tests run, so I'd recommend going into hospital if you can get them run that way, or making a doctor's appointment with a different doctor to ask for them, or waiting for your next appointment. Hi, thanks for sharing your story. I've been itching severely at night for 2 weeks now. It started on top and edges of feet and hands, and then slowly wrapped around to soles and palms and random places throughout the body. I insisted that my doctor send an order for blood test and I did it two days ago but everything apparently came back "normal" and the bile acids are 4. I'm 36 weeks today and they want to induce it anyway for 38 because of diabetes and hypertension, but the itching keeps getting worse. I totally agree with you on insisting to get taken seriously because I had to ask my doctor like 3 times for this. They look at you like you're being crazy or dramatic, but if your baby ends up not making it, all you'll hear from them is "I'm sorry" and you get stuck with the pain of loss and guilt forever. Not worth it. Babies only have us mothers to fight for them.
@LeftieLoosie3 жыл бұрын
There are other reasons for itchyness in pregnancy.... my bile acids were at 55 for my first test. Anything under 10 is "normal" and not cholestasis.... I see this was 3 months ago, so I hope everything turned out well for you and you got some itchy relief.

I'm so sorry you're struggling with this. I'm really not qualified to be giving anyone medical advice; I would advise you to talk to your doctor and to seek a second opinion with a different doctor if yours is not receptive to your worries. I wish you all the best.
