Im an intern at a gastroenterology station in Germany working under the nurses (but funnily enough, I come from Philly and study medicine!), I watched this to figure out what the doctors at my station spend their day doing, but I believe they have quite a different schedule so I still feel quite in the dark ^^ Covid makes it all confusing too! Dear Dr Chiang, Thank you so much for sharing your professional life with us as it inspires us to be a good doctor. I have got one question for you. I was wondering whether it’s possible to secure a GI fellowship in the US without doing the Residency in the US (provided someone has a very strong CV like someone is Upper and Lower GI endoscopy competent and have significant publications) ?? Many Thanks

Hopefully you get to see this comment and give me some advice. I have been having flare ups almost every day since early April of this year. It started out of nowhere a healthy person with no known medical conditions. I feel nauseous every night I wake up with a lot of acid reflux tasting a metallic taste in my mouth a lot of dry heaving and feeling like I’m going to throw up and nothing ever comes out. I have been depending on tumms and although they help these episodes typically last for about twenty minutes these flare ups mess with my sleep. I also have this feeling to poop every time I have these episodes and feel super bloated I find myself rubbing my stomach especially where my spleen and liver are. That’s where the pain is at and the feeling of being nauseous is the worst I find myself talking and saying why am I feeling so nauseous it feels like my stomach is dirty and I just want whatever it is to leave me alone and to go away. I was on pills for acid reflux and none of them helped so now I don’t take medication and I feel like my symptoms have gotten worse. They diagnosed me with gastritis and fatty liver. But they said my blood work is fine but I have no stomach pain. My pain starts from the throat down to my liver. Please help.

Hello. I don't have a doctor and I don't know who to ask. Did the spit test, resulted POSITIVE. Did the urine lab-test 3 days after, and resulted NEGATIVE. Did spit test again, Positive again. Do I have stomach candida? I do have a yeast infection on my left arm though.

Do you have any idea of what gastrointestinal problem I may have? I completely lost my appetite, I feel a stuck and heaviness feeling on my left side, and when I suck in and out my stomach, it makes these piglike noises. When I pass stool, it feels exactly the same and no relief. I could go all day and won’t feel the need to eat because I don’t get hunger signals anymore. Also, i don’t eat big meals just small ones. I’ve massaged my stomach and it feels like I have trapped gas bubbles. Do you think it’s trapped gas, a bezoar, or something else? My GI doctor says I have IBS which I really doubt (I don’t get diareah, nausea, just occasional constipation). They’ve done an ultrasound and everything was fine.

Samiya Mohamed I’ve interned at a non-profit where I communicated a lot with residents, fellows and practicing doctors in GI/Hepatology. After 3 years of internal medicine residency, you complete a 3 year fellowship in Gastroenterology (some programs in the US have a combined GI and Hepatology fellowship program). If you want to do transplant Hepatology, that’s an extra year of fellowship after your GI fellowship ends. I’m not a doctor but this is what I learned from interning at the non-profit for 3 years. Hope that helps!

I have had some symptoms for a long time now and I was 100 percent it was a hiatal hernia. I was telling everyone I know I had a hernia just because of the chronic belching and back pain, neck pain, shortness of breath, bloating, I could not bend over or pick up anything and always feel weak and dizzy all the time and heart papitations like I cant do anything. Today I had a endoscopy and they told me I did not have a hiatal hernia and I am shocked because for the last 4 years I was certain I did because I had every symptom and more. Is it possible it could have been missed on my upper Gi ? The doctor diagnosed me gastritis . Would that be doing all this to me? I burp near about 24/7 Every time I swallow, breathe , or talk I burp. Its so embarrassing. I have to hold the burping in at work and let it out quietly because its so chronic. Would a barium swallow test show a hiatal hernia? I feel so hopeless with this and just really need some anwsers about what might be going on with me. Its hard to talk because i am always so out of breath even when I am not doing anything.

I have a lot of the same symptoms but they have yet to make a real diagnosis other than colitis. I get constipated and have a hard time breathing when I eat. Can’t bend over cause I have lower abdominal pain and I too feel dizzy and feel like I’m going to faint. I have gotten anxiety attacks as well from it. I thought it was my heart but they ok that. It sucks feeling like this when I used to feel like I was indestructible before. Have they gotten a good diagnosis on you or just some bs cause they don’t really know?
