Honestly, I think the way to go with the avatar for maximum cash is to get three to four avatars, a moon, and a ton of suns. Use clear skies + essence, rusty gear + essence, and anything that buffs suns to consistently multiply the amount of cash the suns make

No, I dont think you got the money from the cleaver trigger. I also noticed in my own run that the dog wasn't giving the +1 money with the cultist. I think the mod has several money triggers that aren't activating or are bugged. Edit: you should have gotten at least 16 from the woodpeckers, and you only got 2.
