Unlikely that he actually killed that women at the very end, he only ever killed those who wronged him, so that blood trail was probably symbolic that he left a trail of blood behind him by killing people throughout the movie and all that

The weird thing is that if you take a sequence from the film like this and view it in isolation, it’s amazing. But it just doesn’t mold together in any cohesive form, it’s a complete mess of a film. The not a good joker sequel, and it’s not a good musical either 🤷🏻‍♂️
