I literally romanced Shadowheart, got to the end of act 1, then restarted my whole playthrough so I could recruit and romance Karlach. The thing is, Shadowheart would be a great romance option in any other video game. But it's hard to compete with a firey devil lady with the personality of a golden retriever.

She is a stand out character among a treasure trove of companions. I had started the game with romancing Shadowheart, but decided to finish that one later and started over so my avatar could be with Karlach. She’s not complicated, full of life and love and that’s just what I needed right now. When ever I get bogged down in the game and get frustrated (I hate how dark act ii is) I go back to a save of the morning after they first sleep together. Watching her express her loving being with the mc more than she’s loved anything and that even just this morning is incredible sets my heart thrumming with warmth. I saw a comment on Reddit that said, “Karlach is life.” And I have to agree.
