The toilet is permanently stained because you never clean it. Try filling it with vinegar for a couple of hours and cover the other parts with vinegar soaked toilet paper. Coke (not the light or zero versions) works too.

For difficult stains on porcelain (toilets) get a pumice stick, it's a stick of rock you grind on the stains and it will scrape them off. If it really is just hard water stains vinegar should help too
