Welcome to Dr. Matthew Boes’s Rotator Cuff Repair Rehabilitation video series. This video is designed to help maximize your recovery following rotator cuff repair/surgery. The goal of our Phase 4 program for rotator cuff rehabilitation is to regain normal strength and pain-free function of the shoulder. Remember the following as you engage in Phase 4 rotator cuff exercises:
Continue warm-up stretch if motion is not full
Progress to light dumbbells (2-5lbs) as strength improves; some exercises are easier to do with Therabands; it is okay to continue with bands if they are more comfortable than dumbbells
Strengthening should continue 3 times per week for up to 12 months to maximize positive outcomes
Conduct prone scapular muscle exercises [15-18 below] with no weight at first; however, as strength improves add light weight (if comfortable)
The following rotator cuff strengthening exercises, which are demonstrated in this video, should be conducted during phase 4 of your rotator cuff rehab:
WARM UP STRETCH - WALL SLIDES
WARM UP STRETCH - EXTERNAL ROTATION
WARM UP STRETCH - CROSS BODY
WARM UP STRETCH - BEHIND BACK
DUMBELL - ELEVATION AND ABDUCTION
DUMBELL - EXTERNAL ROTATION
DUMBBELL - INTERNAL ROTATION
DUMBELL - BENT ROW
DUMBELL - POSTERIOR RAISE
BAND - DIAGONALS
DUMBBELL DIAGONALS
BAND - PUNCHES
BAND - EXTERNAL ROTATION @ 90 DEGREES
BAND - INTERNAL ROTATION @ 90 DEGREES
PRONE ABDUCTION @ 90 DEGREES
PRONE ABDUCTION @ 110 DEGREES
PRONE ABDUCTION @ 90 DEGREES WITH ROTATION
PRONE EXTENSION
Icing is very important after each therapy session, so be sure to place an ice pack or cooling pad on the affected shoulder for 10 minutes to help limit any irritation from exercise. If the affected joint is stiff, try using moist heat for 10 minutes prior to exercise to help loosen things up.
This video should not be used as a substitute for regular physical therapy visits and guidance from your physician. Only progress from one phase to the next based on recommendation from your physician. Most therapy regimens, particularly after rotator cuff repair, are based on a healing timeline. Progressing too quickly, even though it may feel okay at first, may put you at risk for injury. Regular physical therapy visits are valuable for giving you feedback on your progress and helping to ensure you are doing exercises properly. Retired 60 yo firefighter paramedic spent the majority of that time as a wanna be bodybuilder. At age 40 did 7 years of kick boxing. Age 55 started jujitsu. At 58 & 60 bilateral complete supraspinatus tendon tears. First surgery I was Back on mat in 3 month however, second surgery I’m 4 months post surgery and unable to do a single push-up. I’ve watched countless shoulder videos and been through months of professional PT. This is 1 of the most comprehensive shoulder rehab videos I’ve come across. Be patient but most of all be David goggins dedicated to rehab daily. Daily !!!

Summary: 1. Warmup 1.1. Wall slide up 1.2. External rotation (turn body, not arm) Waist height 1.3. External rotation (turn body, not arm) Shoulder height 1.4. Cross body 1.5. Behind back arm raise (strap/cane)e 1.6. 2. Exercises 2.1. IVT with weights 2.2. External rotation with weights 2.3. Internal rotation with weights 2.4. Bent row 2.5. Posterior raise (bent over, raise arms to side) 2.6. Diagonal raise (across body) 2.7. Punches 2.8. External rotation 90 degrees 2.9. Internal rotation 90 degrees 2.10. Prone abduction (on stomach, arms straight, out at 90 degrees). Lift up 2.11. Prone abduction (on stomach, arms straight out at 110 degrees) Lift up 2.12. Prone abduction (on stomach, elbows at 90 degrees, arms out 90 degrees, ) Lift up 2.13. Prone externsion (on stomach, arms straight, aligned with legs). Lift up, pinching should blades

I did these earlier in PT, phase 2 or 3. As I progressed I did not have to do them anymore.

I'm 22 weeks post op and still really stiff. This is crazy. Should i still be so stiff?

If your Dr has cleared U U can be more aggressive with your stretches. Your physical therapist should have already been doing this. Unless your Dr. says u not ready

@@ClayLud I'm about 22 weeks post 6 week total immobilization in a sling which is what I opted for over surgery as natural healing has better outcome than surgery most often. I too am very stiff still but still working at these therapy phase 1-4 videos in this playlist for about 20 min a day and seeing gradual improvements since day one. I think I could have progressed faster if I were willing to do a few hours a day but I got other stuff to do and don't mind a slower recovery. Better slow and steady than fast and risk reinjuring it!

artbyrobot I do not believe this is true for massive retracted tears. Those can cause major issues if not surgically repaired. Minor or small tears yes, conservative measures are appropriate.

I’m 11 months post op on my second surgery still having an issue with one spot. 1st shoulder wasn’t nearly as bad as this one. It depends on how bad your rotator cuff was and how much repair had to be done.

I can tell you one thing for curtain the guy doing these exercises isn't actively recovering from rotator cuff surgery

I'm a physical therapist and I do appreciate this video, but some of the excercises are, by no means, intended for phase two! Patients are being confused and not satisfied with their results if they see what they "are supposed to be able to do" at this point. Please, edit some of the excercises out.
