Great video. i had a large subscapula tear and a torn bicep muscle from a fall. I ended up having surgery. Was in a sling for 8 long awful weeks, I was suppose to wear it 10 weeks but I was doing well. My surgeon warned me that the surgery would be more painful than the pain I was currently having. He was right and I appreciate his honesty about that. I truly do not wish this injury on anyone.

I just had a knee replacement, and let me just say it was a breeze compared to my rotator cuff surgery several years ago. The maddening thing? I was sent to P.T for my shoulder, hoping to avoid surgery…instead I ended up with 2 additional torn muscles because the therapist didn’t take me seriously when I told her she was working me too hard. After my surgery, the surgeon called the Therapist and reamed her out. It took almost 6 months of O.T to get me where I could use my arm again. Trust what your body is telling you, and be a strong advocate for your health needs.

I'm a soldier and got a tear when patrolling. We'd rested up and set of in the dark. I lifted my backpack (40kilos) over my head, but snagged it on my body armour. Then I threw the pack forwards, but instead of letting it go to the ground I stopped it part way. Couldn't sleep for two nights and after being x-rayed was told I'd need surgery. A few guys I talked to said surgery was the worst decision they made. All of them needed surgery for a second time. I decided to use physio to built up the muscles. Now I'm good.

Listening to these doctors, I think I had tear on my rotator cuff of my left shoulder. I had all the symptoms these doctors talk about. I was sleeping on my left side all the time so I laid a pillow on each side to avoid sleeping on my side. I tried not to “baby” my left arm, I would use that arm without hurting it. Lo & behold, after some weeks, my left arm feels a lot better & I am able to fold my bedsheets & stretch that left arm. I am 70 years old & I am now more careful in activities of daily living & I try to strengthen & tone my extremities & the rest of my body! I try not to run to the Dr right away & give my body a chance to heal naturally, which it does!

Had arthroscopic surgery done almost 2 decades ago. Where it was torn was from 10 o'clock to 4 o'clock. The recovery times listed were accurate for me. My advice before your surgery to help you prepare is start using your other arm for everyday activities like eating, brushing your teeth etc... that way when you are in intense post surgical pain for that first week your other arm is already somewhat trained it will make things a lot easier and quicker and less messy.

This brings back memories. I am a bodybuilder and I had a rotator cuff in both shoulders. I had my left one done and then my right one done about 2-3 years later. The right shoulder didnt hurt until about 2 years later so I knew it was my rotator cuff. I had a great ortho surgeon at Fort Wayne Ortho. He explained everything to me. X-rays, and then MRI to confirm the diagnosis. He knew since I weight trained, the only way to get back to normal was surgery. The recovery was long but worth it. I had 12 weeks of physical therapy with each shoulder. Today, I have no issues with my shoulders. So glad I had it done. I am now 52 and I don't train near as heavy as I used to but I still like to push some heavy iron when I feel up to it. :)

For starters, great video! I found it to be very informative. I learned far more from watching this video than from the visit I had with my local shoulder specialist. Since everyone else is including their specific problem, here's mine. My left shoulder has been sore for a month or two. Nothing terrible, just a moderate ache. Then on the 4th of July, our family wanted to have a campfire. I needed to chop a little wood beforehand. On the first swing of the ax, I felt a terrible pain and instant regret. Now I would describe the pain as a burning or stinging pain. It's now July 30th and the pain has not really subsided at all. I've had several visits to PT so far with no real improvements. An MRI is pending the recommendation from PT. Once again, great video, and thank you for the quality information.

Hi Dan. Thanks so much and glad we can help in any small way. So it certainly does sound like you may have rotator cuff tendonitis and possibly a small tear. If the pain is in the front, it may be related to bicipital tendonitis. The MRI will provide a lot of information that can help guide your treatment. Physio and some type of anti-inflammatory are always a good start. Best of luck. Thanks for watching and subscribing if you did.

I have a rotator cuff tear (diagnosed via x-ray) and got the injection, so I can say that (at least in my case) it wasn't really that painful, and, as they claim in this video (and in my case) it did break the pain cycle. It's been maybe a year since the injection, and I still have no pain unless I try reaching way over my head- and even then it's not too bad. At the gym, I'm finally back to bench pressing over 200 lbs (in the beginning - right after the injury I couldn't even bench the bar with no weights). They had to go in and do a replacement because the therapy stripped all the cartilage out because she had a couple huge bone spurs that no one new about because no scans were done prior to treatment.

@@louielouiepks These doctors, like we're seeing so much with particularly this last year and a half publicly, are being controlled by bureaucrats within medicine, so even though there's a basis in science, a lot of physicians aren't being mentored to use the basis in science, but to rely instead on clinical measures alone. you don't prescribe "therapy" without knowing what your rehabbing, that's 180° backwards, to arbitrarily have somebody come- in and move they're arms ambiguously for half an hour, One direction, or another. you design (rehabilitation) based on where the flaws (damage) is and you only know that, if you perform certain lab tests first, MRIs,: for whatever reason I have never been able to get a straight answer why doctors are so reluctant to order an MRI! ? what goes on with the procedure of an MRI, that makes it so costly ? I've never been able to research and find a straight answer to that. I've learned the hard way and it's becoming more apparent systemically that you shouldn't be in "AWE" of someone, just because they have an M.D after their name. they do not know everything, they're limited to what their specialty they've been practicing has been and they're further limited by how they were trained and where they are working currently within the bureaucracies (administrators) involved, ESPECIALLY within H,M,Os The average lay person who has a pain problem and goes to a doctor for answers doesn't have enough of an understanding of terminologies to be able to "unpack" what they're hearing, to understand whether they're getting good treatment or bad treatment. they don't know literally whether that doctor is dazzling the patient with brilliance or completely confusing and baffling the patient with bullshit simply because it's all like hearing "Greek" to the average lay person. they don't know what a clinically "diagnosed" term even means and how there is no science if something's clinically "diagnosed", it's an opinion, that's unverified, without any things like x-rays or MRIs or a biopsy, or blood work, It used to be routine practice that a doctor in their office- examining you would suspect something, and then order lab work to verify that doctor's theory, less and less that second phase where they're ordering lab work to verify the clinical theory is not happening, thanks to all the bureaucracies & red tape and money changing and bean counting that's going on with so many medical plans, particularly the HMOs.

Love your video's - You both do a great job explaining in layman's terms what is going on. Last year at this time I was just beginning the recovery on my second left knee replacement, and it has gone excellent, wish I had it done sooner. So now on to my next problem - My dominant right shoulder 3 prior Rotator Cuff Repairs with the last being in 2005. I had an MRI in April 2021 and the doctor local) said that I have the following - Trace glenohumeral and subacromial so deltoid bursal fluid, Moderate osteoarthritis, Supraspinatus tendinopathy and full-thickness partial width tear involving the anterior insertional fibers measuring 4 mm in AP dimension. Low-grade partial-thickness intrasubstance tear at the supraspinatus myotendinous junction. Subscapularis tendinopathy and low-grade partial-thickness intrasubstance tear of the superior insertional fibers. Long head of biceps tendon: Intra-articular tendinopathy. Labrum: Anterior labral tear, Glenohumeral cartilage: Low-grade partial-thickness cartilage loss, Severe infraspinatus and teres minor muscular atrophy. The doctor said to do PT for 6 weeks 2x weekly. Well it is no better now, and I am not able to throw underhand now. I decided to see the same doctor in December 2021, who did my surgery back in 2005, who is based out of Mass. General. With this being said I was thinking that the doctor may say to do a shoulder replacement, however now I am thinking maybe a reverse shoulder replacement based on the fact that I have already had 3 repairs done. I was just wondering what your thoughts would be, and why. Thanks Rick.
@TalkingWithDocs3 жыл бұрын
Hi Rick. Fair question. The decision for reverse vs TSA really depends on the strength and quality of the rotator cuff. I hurt my shoulder ten years ago lifting something heavy. I immediately felt the tare. I went to the doctor and did a series of PT. A year later at work it was made worst by a student pulling on me. I had a full workup and it is a tare, a spur, and arthritis. I have had a few shots, taking ibuprofen 800 for a period of time along with rest, ha ha! The last shot was a month ago to buy time. I asked the doc to reduce the dose so I wouldn't be so agitated by the medication. I treat it at home with heat and ice almost daily. Because it became a work comp issue and has continued so long I need to have it fixed soon for it to be covered. I am waiting for my recent THR to completely heal before embarking on this next journey. I have heard that this is a more difficult process than the hip. Boy, I have to really gear up! Here's a couple of suggestions as I've been through it already. Buy a pair of sturdy SLIP ON SHOES- NOT SLIPPERS! You can't tie your shoes at all. I would strongly suggest a pair of slip on sneakers (gym shoes) that as it will help you get in and out of your house with ease. Don't wear house slippers or loose fitting shoes as they are a trip hazzard waiting to happen.. when you're walking up and down stairs you have no ability to stop your fall if you misstep. Don't make light of a daily activity you've been doing since you were a toddler. Your sense of balance might be slightly off with an immobilizer on. Get a bath sponge that is on a long handle. This will help at bath time to get to parts you can't reach with one hand. Buy pants that don't require a zipper for on/off. Athletic wear is the best but avoid over the head shirts AND buttons! Snaps work better unless you have family members to help!

Very recent cortisone injection for this issue. While there was indeed some mild pain the couple of days after the procedure, the PA who administered the injection was so good that I literally had no idea when she was inserting the needle except for her telling me step by step what she was doing. (The whole thing is done from behind, which was just fine with me...) And in my case it has meant a significant pain reduction, at least for now. Hope that might help someone facing this with some misgivings.

Thanks so much for your very informative videos. I'd like to request a comparison video of when and why to choose a "partial" knee replacement rather than a Full knee replacement. I had a partial done and this surgeon used the robotic method and I'm totally happy with it. It did take longer to heal that I thought it would, like a whole year, but after that it's been great, Full motion and no pain. I can even kneel in the yard to pull weeds. On the other hand I have a family member that had a full knee replacement and it took 18 months to heal and still can not kneel on it. I'd just like to see something that discusses when and why a person should request partial over full or to agree to full and not partial. Thanks so much for considering.
@TalkingWithDocs5 жыл бұрын
Hi Casual2020. Thanks for your question. So the main reason to consider a partial knee replacement is arthritis in only one of the three compartments and minimal deformity. Not all patients are candidates. This is a fair question to discuss with your surgeon. We tell all of our patients that it takes 1-2 years to full recover. Continued good luck and thanks for watching.

If you've injured your shoulder be sure to keep moving it as much as possible to prevent your shoulder from becoming frozen. I know it hurts like hell but if the surgeon has to bust your shoulder out of its frozen position during surgery, your healing time will be increased. I had a machine that worked my shoulder everyday after surgery. Do all the PT and you will be pain free and get your mobility back. I still do the exercises to keep my shoulder strong. Initially I don't think my Dr. believed me about my level of pain before surgery but after surgery he admitted my shoulder was really bad. He had to remove my muscle, trimmed it and re attached it because it was so frayed and damaged. Biggest thing, do your physical therapy. Before and after.

I’m coming up to nine months since my tear. It always hurts with movement, sort of like letting you know it’s always there. but nothing like the first two months when I had to use my left arm to move my right arm. Trying to wash my self Amy brush my daughters hair was a struggle and super painful. But over time, you adapt. At that time afterwards I had a lot of other stuff going on, I went to book in for a MIR and surgery but my kids and husband come first. Crazy family health problems all at the wrong time. So, you learn to deal with the pain. It actually lessens a bit over time. I still have to chop wood, mow the lawn, cut back the trees and attend to the animals living areas. And go to work at an early education centre. It’s at night that it aches and let’s you know how much you have pushed it. I have to lay on my stomach and place a pillow under the injured right shoulder so I can sleep after a couple of Panadol. if I have used hard equipment, Panadol doesn’t do much. There are times when I swear I did more damage while still needing to do everyday work. Sitting here now, it lets me know it’s there. But when you have jobs to do to keep a rural household going. Family comes first, mum will be ok.

Having had my left shoulder repaired twice I can tell you that it is 4-6 months after surgery before you are allowed to return to work. My first surgery was an inferior-capsular shift repair, which was an open repair. That scar is on the front of my chest just in from the shoulder joint. The second surgery was a bankart repair through a scope. That time they had to repair the labrum again as well as fix an impingement, rotator cuff, and re-attach the supraspinatus tendon to the humerus. I can tell you all that the original injury was from when I was 17 years old so no it is not just old people who have these injuries.

I think it's hugely important to point out that sometimes the cure can be worse than the affliction. My recovery from rotator cuff injury was delayed for at least a month, during which time no pain medication worked, I got no sleep, and thought I was losing my mind because there was no relief. Through research I found that the acetaminophen I was taking along with other NSAIDS were actually causing my tendons to begin detaching from my bones. No doctor told me about or warned me of this. After 2 months of unending pain, I finally rid myself of all medications and slowly began to heal. The entire arm affected had atrophied to half its size and strength was reduced by at least 75%. Now 6 months after that life changing event, I am almost back to normal, and will never take another NSAID. S/N I went to three different doctors and all of them were guessing when they couldn't see something specific on an X-ray. I got diagnosed with everything from nerve damage, potentially needing surgery, to arthritis in the shoulder. I am pain free now and on zero medication after healing myself with physical therapy utilizing a rubber band, which is the only thing that helped after riding myself of all medication.

Hi Scott. Sorry to hear you had a difficult post op course but glad to hear you are ok now. There is a lot of vague literature about NSAIDs affecting the body's ability to make bone but these are mostly from spinal fusion studies. There was a recent article in 2020 from the Journal of Shoulder and Elbow Surgery International that did say there was a trend toward less intact rotator cuff repairs in a group that received celebrex (50%) rather than placebo (70%) but it did say that the results were not statistically significant. If you dig deepre there are a lot of issues with this study. There were only 79 total patients in this study and 21 were lost to followup, 6 did not have cuff repair, 5 were revised and 2 declined so only 40 had an MRI. This found that 10/20 of the celebrex group had intact repairs and 14/20 of the placebo group were intact. This study is grossly underpowered to make any conclusions. Rotator cuff tendon surgery is certainly not universally successful and there are many reasons for failure but it would be hard to specifically blame the NSAIDs. Nevertheless, glad you are ok now. Thanks for watching, subscribing and sharing with someone if you did.

Hello, doctor, doctor, doctor. I’m pretty sure I have a small super-? tear. I’m getting up there in age, my body is telling me to slow down, ugh...I don’t want to. I guess it will totally tear at some point. Thanks for the information, now I know what to say to my ortho doctor.

Try to ingest CBD oil to reduce pain levels and inflammation. I have two failed rotator surgeries due to infection. Listen to your physio and use his/her acupuncture treatments. Sleep will come.

@@theGonemadagain Amen to that! I started using CBD a month ago out of curiosity to see if it would help my knee which was injured 30 years ago. Since the initial ACL injury, I’ve lived with a constant reminder (sometimes agonizingly) that my knee was “just not right”. Since starting CBD, the constant aches and pain have gone and the knee feels normal! Remember too, to strengthen and stretch the muscles that are injured - I cannot stress the importance of exercise in maintaining good physical health. Highly recommend it for pain and inflammation.

Great video! I just tore my rotator cuff tendon completely as well as my biceps. No choice but to have surgery but I have a much better understanding of it now. You guys should sell your video to other orthopedic docs to play for their patients because most docs do not explain so clearly. Thanks guys!

So with my shoulder I’ve had pain for months. I have full range of motion , however it hurts when doing pushing movements. Also when I do the thumb down test my shoulder pops each time
@TalkingWithDocs4 жыл бұрын
Hi T Doh. High chance of rotator cuff pathology and possibly biceps tendonitis or labral pathology. Start with pain control and physio. May need an ultrasound or MRI if it gets severe. Best of luck to you. Thanks for watching and subscribing if you did.

Being a transplanted born and raised downtown Torontonian, it's nice to see something this clear and concise for some great info as I have a left rotator tear along with degenerative Osteoarthritis, hence Cortisone shots in both shoulders every 3 months. From experience I don't trust 99% of Physiotherapists so this kind of info is perfect with my Personal Training background to give me insight as to coming up with my own rehabilitation. Thx so much guys for such an Amazing and informative video as well as the others I have seen!!

I recently tried to lift my 64 pound dog and heard a loud pop and lots of pain. Went to doc next day. Xrays showed widening of the joint but not dislocated. HISTORY of dislocation as teen. She put me in a sling for a week and referral made to ortho. What does widening of joint mean?
@TalkingWithDocs4 жыл бұрын
Hi Bobbie. Sometimes after a shoulder injury the muscles (particularly the deltoid) can become weak and the humeral head (the ball) sags a bit off the socket. Maybe that is what they are talking about. The only other thing we could think of is if you have an AC separation and there would be some widening between the clavicle and the acromion. Best of luck to you. Sorry for the delayed response. Thanks for watching and subscribing if you did.

I think you're right. Treatment often depends on who you see first. General Practitioners will prescribe pills/shots, Physical Therapists will prescribe PT, Chiropractors will prescribe "adjustments", and Surgeons will cut. Unless you have socialized medicine, then you get the cheapest treatment possible.

"Best Way To Know if You Have a Rotator Cuff Tear" - is it just me, or did they NEVER ACTUALLY say how you'd know this? If I take this entire video at face value, pain seems to be the only indication. I've been dealing with sometimes debilitating pain for over 3 years now. Been to physical therapy, accupuncture, had injections, etc. I'm still searching for a solution. This video wasn't that.
@cecil51523 жыл бұрын
Over the years I had multiple x-rays, sent to a few physical therapies, but nothing worked. I was even told that I was just getting old. Finally told a different Doc it hurt more than it really did so he ok'd me to get an MRI, after that they were able to see the tear. This was through military doctors so its a matter of money. MRIs cost more than x-rays, from my experience, the best way to know is get an MRI. My thoughts, military docs are on budgets and will do everything cheaper before spending on an MRI.

Thanks Cecil! I have an MRI scheduled two weeks from Friday. Hope this helps sort it out.

@@Adam_K_W I hope you get good news. Surgery will suck, hopefully it's not a tear and the doc can see the problem and is able to prescribe some kind of therapy that will strengthen/fix the problem.
