1:58:27 Baretha is absolutely terrifying - I had so much trouble with this fight. I also did not realize it was possible to make a melee attack miss vs a held target.

The miss chance is due to Reeling (from Lae'zel's shield, Tav's armor, and Astarion's crossbow), so it's more about debuffing the attacker's hit chance. Regarding the difficulty of the fights, if they lasted more than 2-3 rounds, it would be much harder to sustain their damage. These NPCs are built more like main characters than typical NPCs.
