To be honest Tottoland was a pretty great place to live. Everyone got along, had food and houses for their families. Tottoland was so nice to live in the people were willing to pay rent with their lifespan. Compare that to Wano where people are starving and starving people are force fed smile fruits. Big mom is a good yonko. She runs her territories well, ensuring her citizens do not starve like the ones in Wano. She rules with fear and occasional violence, but that was part of making citizens respect and obey her. A historical example of her would be Empress Cixi, the last empress of Qing Dynasty. She kept China together without giving up a lot of territories to the Western powers, paving the way for Modern China. There was a reason why Luffy did not feel the need to liberate whole cake island compared to Wano.
