Now and then, we've all thought we heard someone calling our name, or noticed a strange coincidence. But for people with schizophrenia, these can take on a much more nefarious quality. Dr. Vikaas Sohal walks us through what it feels like to be inside a schizophrenic brain.
----------------------------------------------------------------------------------
VIKAAS SOHAL:
Dr. Vikaas Sohal is Assistant Professor of Psychiatry at UC San Francisco. His research has focused for years on cognition and functioning. He has written extensively on aging in schizophrenia, functional impairments in severe mental illness, the cognitive effects of typical and atypical antipsychotics, as well as studying the effects of cognitive enhancing agents in various conditions, including schizophrenia, dementia, affective disorders, and traumatic brain injury. Dr. Sohal is also a board certified psychiatrist and continues to see outpatients approximately one half day each week. He directs an annual conference on cognition that is an official satellite of the International Congress on Schizophrenia Research and the Schizophrenia International Research Society.
Dr. Sohal earned his A.B. in Applied Mathematics from Harvard University, his M.A.St. in Mathematics from Cambridge, and his MD, PhD from Stanford University. He completed his graduate work in the lab of John Huguenard, then stayed at Stanford to complete his psychiatry residency and a postdoctoral fellowship in the lab of Karl Deisseroth.
----------------------------------------------------------------------------------
TRANSCRIPT:
Vikaas Sohal: Many of us often have the experience of thinking we heard someone calling our name and then realizing that wasn't really the case, and we just move on, and we forget about it. Sometimes we notice coincidences, like, "Oh, there were a couple of cars on the street," and again, we pay no attention to it. But for someone with schizophrenia, these experiences take on a vastly different kind of feeling. So you might notice, "Oh, there were three red cars on the street," and instead of just forgetting about it, you start thinking, "Well, why were there three red cars on the street? Maybe it has something to do with me. Maybe these people are actually coming to monitor me or do something that would harm me," and you start working through some kind of plot or conspiracy related to that coincidence that you noticed.
The symptoms that people are most familiar with in schizophrenia are often hearing voices or having paranoid delusions and paranoid thoughts. We now know that, in addition to those kinds of symptoms, many people with schizophrenia have trouble focusing, paying attention, and remembering things. Many people talk about hearing voices and mean different things. Sometimes people will say, "Oh, yeah, when I'm thinking about what to do, I hear a voice in my head that tells me this would be the right thing to do, or this would be the wrong thing to do, or that gives me an idea." And when individuals with schizophrenia talk about hearing voices, they describe it very, very differently. It's really a voice, that they can't tell the difference between that voice and a voice coming from a person sitting right next to them in a room. And it sounds absolutely real. It sounds loud. Sometimes it's so loud that they can't stop paying attention to it. And sometimes the voices are just calling their name, but sometimes the voices are saying much more complicated things. Sometimes they're giving them commands, telling them what to do. Sometimes they're commenting on what they're doing, often in a negative or derogatory way.
It comes as no surprise that the brain is a complicated place. There's information flying all over the brain, and the brain has to get that information to the right place and decide what to do with it. We think that, perhaps, in schizophrenia some of the problems come because information isn't getting to the right place, or, sometimes when information does get to the right place, the brain doesn't know what it should pay attention to and what it should ignore. This is really important. In our everyday lives, we might hear a car alarm in the background, and we have to know not to pay attention to that, but we might also hear a baby crying in the background, and that probably is something that we need to pay attention to. And so, if your brain's not able to act as an executive and say, "This is what's important and this is what's not important," it becomes very difficult to sort .......

Living with someone that most of the time can't diferenciate reality from what's on your mind makes you think a lot of questions related to life itself. My father has schizophrenia and in all of those years i've never seen such a video describing how the person feels. Extremely accurate.

My mum currently has voices calling her a pedophile, child abuser and accusing her of raping people. She works for the Police so her line of work is often dealing with brutal cases and her voices manifest in the form of crimes. She is terrified and in the psych ward currently. This is a heartbreaking diagnosis to live with and I hope everyone with this disorder recovers.

@@aaronjennings8385 sadly I don’t think people realise that it’s a very wide spectrum. Some people may hear sinister voices, others may hear voices that they become familiar with and don’t cause them too much distress. Sadly most people associate schizophrenia with the sinister end of the spectrum. This leads to so much stigma and a fear from the general population of those who suffer with schizophrenia. Videos like this will hopefully help to tackle that stigma

I remember a horrible nightmare I had as a kid a couple of times. There was an endless screaming/roaring. It was deafening. Luckily I have always been able to wake my self up from any dream, as I usually realize I am dreaming which is a really cool gift. I have used this ability to fly in my dreams, and to control them to some extent. I usually realize I am dreaming and then forget that I am aware again, so fall into the dream story again.

+MMR13 Schizophrenia is based mostly on environment and social constructs. Mostly everyone that says he hear voices of God,demons, or angels were involved in some form of religious belief. Whether they were in it or heard enough about it . There are some people who in fact do hear good voices but is rare. Hearing anything to much can be annoying and cause paranoia. .You're not having hallucinations or crazy. You're actually hearing the voices of dead people, angels, demons and/or elementals. Modern medicine which is only 300 years old, rejects the concept of a spirit world that the Bibles Old Testament, which is more than 3,000 years old preached. Well, if there is a God, then there are also angels, demons and other spirit entities. Since most people (about 90%) don't have a third eye/ear, they think people who can see/hear spirits are just crazy. Spirits communicate on a certain wavelength that ONLY you can hear/see, while others can't. The wavelenght you get to acess depends on your LEVEL of spirituality. If youre a good & decent person, you get to access angels and saints. And vice versa for bad people - usually demons. Some times your third eye/ear opens after a traumatic event or near death experience. Spirits can also imitate peoples voices like those of your parents, friends even enemies. They can also create noises like screaming or crying people, footsteps, knocking on doors, police sirens, dog barks etc. Like people some spirits are good while some are bad. Some are bright, others are dumb. Most bad spirits want to isolate you by making people think you're crazy. There are also spirits who play tricks on you. The sooner you realize this the better you can cope with it. If a problem is spiritual, the solution is also SPIRITUAL. BTW this is NOT a substitute for not taking your meds. Take them if it helps. I've dealt with this problem for 25 years now.

@@napoleonaquino3829 I can believe that. I honestly thought I was possessed by spirits. I even saw one come out of my wall, she took the form of this blue holographic image... she walked closed to me and was trying to comfort me when I kept having horrible visions of me dying. She sat next to my bed and carressed my face and told me in a soft tone "I don't ever want you thinking about that again" and started to fade, but her voice remained. I am convinced I am involved in a telepathic situation, but nobody cares. It would really make a difference if someone wanted and was qualified to scientifically look at what I'm going through but people get licenses to practice their profession from the same folks that want you to keep quiet and remain silent. I've injected methamphetamine for the past 100 days now and want to quit.

Sophisticated White Man you know the problem is for alot of people it begins and starts up for the first time at work. there are laws in place to protect people's jobs if they are fired for an actual illness or disorder. firing that person could make the illness worse. everybody needs to work. getting state aid for these types of mental problems takes alot of time, documentation and witnesses writing testimonies in order to help. just because a person has mental disorders does not mean they are dangerous to themselves or anyone else. that person just needs help. examples: temporary part time, temporary light duty, positive reinforcement in the work that they are honestly trying thier best to do, an extra break or two to help them come to thier senses. they can get healthy again, it just takes time and patience

Our downstairs neighbour has schizophrenia. As a result he watches us from the window all the time, leaves the building door open all the time, has stolen post and parts of my bike and is following and asking our guests what they're doing here many times. One time he was hiding between the bins when I went to throw out the the rubbish and he scared the life out of me. Any advice on how to deal with him? We want to be sympathetic towards the fact that he has a mental illness but on the other hand this behaviour is annoying, dangerous (one time he picked on the bike breaks and I only realised after I had started cycling, now I carry the bike up and down my second floor flat every day) and borderline criminal when he steals things. We have thought of calling the police but I don't see how that would help or if they would make matters worse... Advice?

+Vassiliki Sin talking to him with might help decrease his paranoid because he is familiar with you. If he trust you he might not be as paranoid . Only call the police if he commits a crime and tell the dispatcher he has a mental illness. This would send him to a mental hospital instead of a jail. He might just need help or he might be a criminal.

What he described in the beginning is so true. And it is really hard to steer them to a more rational place when they're so convinced everything and everyone has a dark or negative agenda, including their doctor. "they're much harder to deal with... have problems with memory..." This. I really hope a cure can be found for them because they become so terribly angry and unhappy that it's sad. They're in that mind frame all the time no matter what family or friends try to say or do :(

Schizophrenia is just a catch-all term for forms of mental behaviour that we don’t understand. In the 19th century, there was a term “melancholia”, which we would now call bipolar depression, so forth and so on. But all forms of sadness, unhappiness, maladaptation, so forth and so on, were poured into this label “melancholia”. We have no tradition of shamanism. We have no tradition of journeying into these mental worlds. We are terrified of madness. We fear it because the Western mind is a house of cards, and the people who built that house of cards know that, and they are terrified of madness. Tim Leary once said - or I gave him credit for saying; he later told me he never said it - but whoever said it, this was a brilliant statement; someone once said, “LSD is a psychedelic substance which occasionally causes psychotic behaviour in people who have not taken it.” - right? And I would bet you that more people have exhibited psychotic behaviour from not taking LSD, but just thinking about it, than ever exhibited it from taking it - certainly in my family. I watched my parents both go psychotic from the mere fact that LSD existed; they would never have taken it. There is a great phobia about the mind: the Western mind is very queasy when first principles are questioned. Rarer than corpses in this society are the untreated mad, because we can’t come to terms with that. A shaman is someone who swims in the same ocean as the schizophrenic, but the shaman has thousands and thousands of years of sanctioned technique and tradition to draw upon. In a traditional society, if you exhibited “schizophrenic” tendencies, you are immediately drawn out of the pack and put under the care and tutelage of master shamans. You are told: “You are special. Your abilities are very central to the health of our society. You will cure. You will prophesy. You will guide our society in its most fundamental decisions.” Contrast this with what a person exhibiting schizophrenic activity in our society is told. They’re told: “You don’t fit in. You are becoming a problem. You don’t pull your own weight. You are not of equal worth to the rest of us. You are sick. You have to go to the hospital. You have to be locked up.” - You are on a par with prisoners and lost dogs in our society. So that treatment of schizophrenia makes it incurable.

The derogatory remarks below prove stigma is very high with this horrific brain disease that effects one percent of the population. It has been discovered that most of these individuals also have anosognosia. Brain disease in frontal lobe, it shows on autopsies, it is lack of self awareness. They have no idea they are sick, until medicated. We will continue to see these folks in full psychosis on our streets as long as schizophrenia and bi-polar are seen as behavioural problems and not the brain disease that it is, just like Alzheimers and Parkinsons.

.You're not having hallucinations or crazy.

I didn't think it was Tourettes because I'm able to force myself to shut up or at least lower my volume when others are around. Alone, with the defenses down, I'll be cursing and yelling. So what if you have paranoid delusions with Asperger syndrome, which can overlap with schizophrenia? I have Asperger Syndrome with depression, anxiety, marked paranoia. If I don't get sufficient sleep, I get very paranoid about my surroundings, I get delusions, paranoia, hallucinations. Like I may see something or someone thinking its there because of me, but this can be regarded as anxiety! I get so confused with it all just trying to comprehend all this information in my mind, I just lost in these thoughts and there are branches that stem from this into more delusions of paranoia, anxiety. Hmmm.. welcome to Mental health services huh.

I am diagnosed with high functioning ASD. There are times, like you mention, where stress, anxiety, and lack of sleep can induce hallucinations of various kinds. From my experience, these often occur during an autism 'meltdown'. For me these experiences are like having every nerve and receptor in my body turned on all at once. I think these meltdowns are due to the lack of filters people with autism tend to have. These don't just filter what's going out, but also what's coming in. It's completely, bang your head against the wall, overwhelming. It's during these times that I experience voices, shadow figures, and music that isn't there. Fortunately, I can tell the difference currently. If you can't you really need to speak with a doctor about this.

+aspektx Hi, I've since been able to come to realise now that I have been confusing my anxiety disorder with Paranoia, and with the crossover between Aspergers and Schizophrenia. This is because I can't tell the difference because of my Asperger's unable to pick up signals and read them correctly. I struggle to understand what anxiety actually is in my head. Although I am aware I get paranoid when I can't sleep and I agree with you when you say it is caused by sleep induce. Though my SSRI medication has helped to some degree, I don't think it is working as effectively anymore as I've gone back to my old state of mind with anxieties getting out of control. Maybe I need to try a Benzo or SNRI?

Wether they boo you, cheer you, or whatever they have to say, you are still the main attraction. They are and always will be fans and onlookers on the other side of the barracade. My internal monologue rarely stops, even with meditation. I hear distorted sounds but not external voices. The sounds appear to be made by a human voice but not words, at least not a language I know. I say it sounds like Charlie Brown’s teacher through a thin wall. Usually only happens when I’m alone somewhere like the woods. I seriously have a lot of schizophrenic type thoughts like that. The paranoia of people with elaborate plots against me, or that the car that just turned behind me is now following me home, or more extreme like I'm living in a computer simulation or that everyone I know are actors or not conscious people and the entire universe only exists in to me. Or perhaps things like certain things that I experienced happened in order to give me a message, just typical magical and superstitious thinking. I've never had any sort of psychotic reaction to them. The thoughts surface for a few seconds and I realize I'm being ridiculous and I just end that thought sequence. These thoughts always seem to surface when I'm not paying attention to my thinking and I'm just on autopilot. When I'm having more conscious thought, I would never think things like this. I don't get worried or confused or anything. I just end the thought and forget about it. Is this normal? I assume schizophrenics would get these thoughts and not have the ability to tell that they are making extreme errors in judgement. They would be jumping to conclusions over and over again and, because they can't control their run away thoughts, they become extremely perceptive and paranoid of innocuous coincidences that give credence to their delusion.

+Doik I know the thalamus is also the central hub of the senses and thought. Would schizophrenia be a problem with a malfunctioning thalamus? The reason I think this is because I've studied psychedelic drugs and I've read about the thalamus's function. I've also done psychedelics before and almost had a full blown schizophrenic paranoia attack. I was convinced aliens and the government were monitoring my every move. I also experienced ego death when I hallucinated some kind of central hub network routing incoming data and I recognized it as myself. Kind of sent me over board and I felt like I had a seizure. After the loss of all sense of self I just, poof, snapped out of all the delusion and paranoia and went back to having a normal fun trip.

+Doik aaaaaaaaand I should specify that that "paranoia attack" was on 4 doses of strong LSD, on my 3rd time ever doing psychedelics. I've had probably about 20 trips since that and have never had another bout of paranoia that intense. I should also specify that I've always had the paranoid schizophrenic thoughts that I first described, as long as I can remember anyway. I'm not advocating the use of psychedelics but I do feel that they have certainly helped me gain control of my mind and allow me to cut out the garbage thoughts (thoughts that are logically fallible or adverse to established scientific knowledge).

Been Schizophrenic since I was 12. Before all that fuzz I had straight A's, did martial arts, played sports and made friends easily. When I use fall asleep I will close my eyes and I am in my room yet theres shadow people around. Doors opening and closing. People grabbing me, choking me. I will wake up from my day dream and I am in my room yet this time no one is there. Yet i feel/ hear what I just went thru. Now that I am older it has calmed down a lot. The worst is when it happens mid day. One time I was in my apartment just finished washing dishes. WIDE AWAKE! My other roommates were gone. I start to walk to my room and before i made it to the hallway. I was attacked and thought maybe one of my roommates was trying to rape me. I fought best I could. I struggled for maybe 30 seconds. . I broke their guard and turned around so fucking fast trying to grapple at the same time. When I broke free no one was there. My pants were on yet I was bleeding from my anus. I knocked on my roommates door and checked the garage port. NO ONE was in my apartment expect for me. Doors/windows/sliding balcony door was all locked. That happened to me in my early 20s. At that moment I knew I wasn't Crazy or Schizophrenic. I knew I had some connection to another awareness or dimension or perception. I quit going to the doctors and stopped taking medication. I may only have 1 or 2 "episodes"a year now. I think the medication I was on was actually making things worse. No one would believe me when I told them it wasn't working. I gained a lot of weight, lost hair, my pee turned neon green for 3 days. I felt like a lab rat. I started to do my own research and found out schizophrenia and other like disorders have only been taken seriously and properly investigated for about 150 years. If I were born at any other time I would of probably been burned at the stake or lobotomized.

Idk if it's schizophrenia, but I sometimes hear this creepy man mumble something but I won't know what he said, it happens during my sleep too, like when I'm asleep I have dreams with that voice in them, and I always stair at something without blinking and I swear I can see it breathing or moving and i could see a dolls eyes moving and sometimes I would lip sync or dance to music that's not even playing

I think I was misdiagnosed with major depression because my hallucinations and delusions are not only focused on depressed things, I think we are being controlled like dolls or like we are in. A matrix or simulation, I see photos moving until I look, I see garden nomes move or black shadow figures, today I saw a fox and the kids outside didn’t say anything about it, the other day I thought a bird was dying in our attic and My dog didn’t react to it, my dreams seem so real that I wake up checking my body and surroundings, my panic attacks have caused me to hallucinate objects turning into humans, a wasp on my leg, I hear ringing and the shower curtain made noise as if a bug was stuck in it but I just had moved it and then went downstairs for 3 minutes, I can’t sleep with my closet door opened, I’ll also hear tapping, the voices are in my head so to listen to music so I can sing along to quiet the voices, and I’ve thought I had a ghost in my house with long hair brown eyes and brown hair, wearing a white dress and things in the attic

.You're not having hallucinations or crazy.

Half way through it i regained control of myself and realized what was going on, i punched the guy straight in his nose and broke it. If you hear voices, and are not sure if it's your thoughts, or voices, first, LOOK AROUND AND SEE IF THERE IS ANYONE ELSE IN THE ROOM...IF THERE IS NOT, THEN THINK ABOUT WHAT WAS SAID TO YOU...WOULD THE COMMENT YOU HEARD BE ANYTHING OF INTEREST TO YOU, WOULD WHAT YOU HEARD BE SOMETHING WORTH THINKING ABOUT THAT WOULD HAVE BEEN AN INTEREST TO YOU? OR A QUESTION YOU MAY HAVE ASKED YOURSELF IN THE PAST WANTED TO KNOW OR WAS CURIOUS OF? I TELL MY SON WHO HEARS VOICES, IF HE HERE'S SOMEONE LOUD AND NEXT TO HIM SAY SOMETHING.....TO LOOK AROUND THE ROOM!! IS THERE ANYONE THERE IN THE ROOM WITH YOU? IF THERE ARE PEOPLE IN THE ROOM WHEN YOU HEAR THE VOICES, DID THEIR LIPS MOVE AS YOU HEARD THE VOICE SPEAK? IF THE ANSWER IS NO TO BOTH QUESTIONS, Then you may want to visit your local Dr., or walk in clinic, to discuss hearing voices that are not real, before your disorder or whatever your prognosis is becomes a problem in you everyday life! And if it has already become a bit of a problem in everyday life, PLEASE SEE A DOCTOR IMMEDIATELY TO HALT ANY FURTHER PROGRESS!!!

The part that doesn't line up for me is when i describe voices. i can still identify them as internal thought processes. i do this because i relate certain changes in air pressure to relate with real sounds. i'm not trying to say that some people, or even most people with this condition do not experience them this way. What i'm interested in is if it really is like that for everyone with schizophrenia, and if it is do i really have schizophrenia or am i being misdiagnosed.
