@@GiliGulu1969 Could be! In the Aubrey and Maturin series, upon which this movie was based, the Doctor dueled with his shirt off for that reason. He only meant to wound his opponent (Canning). Because the Doctor did not wish to be attended by military surgeons and, as an act of penance, he performed the surgery on himself.

In this case it was even more so, Maturin wasn't just a surgeon, he was a Physician, a true Doctor. At that time real Doctors in the Navy were RARE, only the wealthiest of Captains could afford them, most ships had to make due with a sawbones of Loblolly Boy (Medical Assistant or Corpsman in todays military). As clumsy as Maturin was on a ships deck he was only slightly less admired by the crew as the Captain

I love how Jack starts to leave and Stephen says that a spare pair of steady hands would be helpful and Jack starts getting ready. I also love how at 2:10 Jack was starting to look a bit queasy and Stephen asked him if he was all right. I also love how after Stephen finishes his surgery and falls asleep, Padeen goes outside the tent to tell everybody how it went, and Captain Howard is the first one that walks up. Padeen doesn't speak and never spoke throughout the whole movie, but the way he nods his head and the way Captain Howard takes his hat off and looks relieved and takes a brief look at the doctor before settling in for the evening was very well done. Captain Howard was probably feeling very guilty about having shot the doctor earlier. He probably fell asleep right away after he reached his tent.

In this era, it was not uncommon for someone to sniff an abdominal wound. If it smelled like dung, the wound had perforated the intestine and it was septic. The only treatment was a kind-hearted comrade putting one out of their misery with a blade or a bullet. It saved you a week or ten days of dying by degrees, screaming every step of the way.

That is always a risk, yes, but part of why they were doing this is because fibers from his shirt had gotten in the wound, which was known at the time to cause infection, or "festering", so they wanted to get it out as quickly as possible. I believe the doctor also mentioned something to the effect of using alcohol as a disinfectant previously in the film, so it's not like they weren't taking ANY precautions against infection.
