Is there any way to change the scale of the cloth without worrying about the actual topology of the cloth? If the cloth is too subdivided, you end up with super tiny wrinkles that look wrong

You can make it in Blender, this one I have purchased from Sketchfab so I think it was likely made in Marvellous designer.

Yes, don't worry about it too much. Mainly it's just another new tool that you have to learn. Just like geometry nodes, photo scanning, etc. 3D means forever leaning new skills. There's a lot of noise now about AI because it's a topic that the algorithm likes, so people jump on it. There's definitely a future in 3D, even in drawing on paper.

I haven't seen any tutorials for other 3d modelers discussing how to do cloth sim sorts of things. Is Blender the only program that supports this, or does z-brush or etc also have support for this sort of thing and I just haven't seen it?

Yes there's a relatively new (2 years ago) cloth sim feature in Zbrush 2021 and later, @askNK has a good video on it!
