Yes, it's possible. One way is to use the cape. As you enter room 4, run to the right and just as you are about to fall off, you should be able to have enough speed to fly. Then you can fly over the wall to the right and grab the 1 Up. It is a little tricky, as the platform is just long enough for you to gain momentum on. There might be other ways as well.
