Would this work on 2005 Honda pilot with power chair lift? It has a backup camera installed but all I see is the power chair. Can you help?

Ugh that's terrible. Yes, this backup cam will work and you won't need the video extension cable. But first see if you can move the existing camera to a better location.

Does the rear view camera show video when driving down the road or only in reverse?

No the rear view mirror does not normally show video when driving forward. However you can tap the display and it will show the rear camera video for a few seconds.
