Just like the great before him, John Bradshaw and Daniel Pink removing the shame component is tough to let go. Shame is the ghost in the machine of the human mind. It can implant itself in the psyche before the first word is spoken, even before the first thought has formed. In his groundbreaking book, The Science of Shame and Its Treatment, psychotherapist and author Gerald Loren Fishkin, Ph.D., addresses the genesis of shame and self-talk from an empirical analysis of their core elements, its insidious ingress into conscious thought, and the havoc it inflicts on a person’s self-worth and behavior.
Through his empirical analysis and understanding of toxic shame, Dr. Fishkin has identified multiple effective clinical approaches for its treatment and addressing shame-based behaviors. He clearly outlines why contemporary treatment approaches, including cognitive behavioral therapy, do not treat core shame wounds and most often cause individuals to terminate the therapeutic process prematurely.
This book is a must-read for clinicians, addiction specialists, teachers, students of human behavior, counselors, social workers, patients in treatment.
Book is available online, B&N, Target, amazon.com and at drgeraldfishkin.com

26:20: "When a person is in the middle of their addiction, they don't understand; they can't. ... their brain is being controlled by the drugs or the chemicals they're addicted to ... so you have to get through that first...in order to begin the process of healthy therapeutic treatment." How do you get to that "clearing in the woods" with food addiction, eating disorders?

I think the therapy world does a great disservice to those with addiction disorders when they keep instilling this derogatory idea that those struggling with these disorders need to be “clean and sober” before they can even address the trauma that may be causing the disorder in the first place! I mean look at the language even ‘clean’ as in what, you’re dirty? What a way to throw more shame, this antiquated notion of looking at the person as they ARE their addiction and can’t get any mental health help if they don’t do it the system’s way or the highway needs to stop! It just drives the stigma and makes it harder for those who are trying to get help if they don’t fit into a certain box that society enforced on them. Ridiculous.

Fully agree that’s toxic shaming anyway, it’s reenforcing the belief that there is something wrong with them, it’s judgment which just leads to an even deeper downward spiral. I’m a bum, I’m a drunk, I’m a loser, why am I so pathetic, I can’t quit this and get straight ect. Stupid!

I hear that. What it sounded like to me is that when someone is actively using - whatever the addictive substance is - brain function may be altered to the extent that the person can’t perceive, accept, or successfully do the work involved. But I think it varies from person to person, and their approach may discourage people from trying because they don’t feel that they can give up the substance at this point.

I am so glad someone else said this somewhere. I was pressured to recently leave my PTSD support group because they wanted to override my psych's guidance (moderate until abstinent -- with medication) by trying to force me to be completely abstinent in order to stay in my support group which was helping me in a variety of ways, including helping me curb my drinking to cope. Even though my consumption dropped dramatically, it wasn't good enough for them after 8 weeks. I withdrew and now I have no support group whatsoever; I have no friends or family and am largely isolated. I did not like AA when I tried it previously and did not find focusing on the punishment/shame-peer pressure-"powerlessness" thing very healing (or empowering). Now I'm isolated once again and not convinced I'm much safer nor do I feel that I should feel ashamed and punish myself for being a failure. Doctors or staff in groups that do this to people should be ashamed of being the manipulative out-of-date charlatans they are, and they do many people who hope to recover from trauma and/or alcoholism a disservice.

@@curiousone6435 I feel your pain my friend. I spent a lot of painful years in addiction. I could never find the right support to help me break free. One day it happened. Right time, right place, right people (just in time as I was getting close to suicide). I finally found people that got me and gave me the time and love that I so much needed. Never give up. Hope is always there. It’s just sometimes we can’t see it. I wish you all the best

I’m so glad some have commented on this! Ugh! If some hear this; they won’t even consider treatment. I understand that sometimes, when the addictions have got some into some pretty dire situations; such as losing job, spouse, health complications; and they go into rehab they have to be clean and sober; some just have to. It can and often does work; with possibility of relapse. I guess it depends on the situation and the person. I agree; while using; even a little; it does affect the way they think. However; some aren’t in dire situations; they are maybe just starting to see life unravel and reducing consumption is the only way they may be willing. It’s at least a start. I know of a couple people who have done it this way and have been successful. It’s not for everyone but it can work. Ultimately; they need to want it. Therapy with compassion; as the Doctor said; is huge in keeping the addict going; not fleeing from shame; which is what they need! They need to know who they are It’s a process. I think the Doctor is brilliant, just the absolutes with clean and sober is a tough one. Many won’t go if that’s what they hear. Find the self, deal with the shame, and trauma, learning new tools on how to feel, and manage the emotions with healthy coping strategies, and some find they don’t need the unhealthy behaviors, or dependencies anymore. While in some cases; in patient rehab , completely clean and sober is necessary; but some won’t even consider it. Many relapse anyways. It’s a long process. It is a whole new way of doing life.

24:00 I don't think most drug addicts have shame isolated to their actions to get drugs, deeper rooted. Just let the doctor talk

Shame, and being *convicted*, recognizing destructive responses, and working to heal are TOTALLY different. Shame is ugly and drives behavior underground. Conviction is recognition and the beginning of healing. 💚
