I think what some are missing is that activation of the depressors is not a full contraction. When activating the depressors (esp the Lat), it also allows for humeral head depression during flex & Abd, reducing humeral rub on the superspinatus. It is not a full restriction of motion, but more of an inhibition of over active elevation. Eccentric training. Hence, re-balancing the force couple and accessory mms. When I was going to school, we called it Glenohumeral Rhythm.

I feel like I would define many of those terms differently. For one, there is no "depression" of the humeral head, although there is inferior glide. This is not something I would attempt to cue. Depression in this video refers to depression of the scapula, and does not include or need to include the latissimus dorsi. Contractions cannot be partial, although they may not occur through full range of motion - in this video we are cuing depression to prevent excessive elevation, although there will be upward movement of the acromion, this should be related to upward rotation. Glenohumeral rhythm is actually limited to the osteokinematic motions that are correlated between the humerus and scapula, i.e. upward rotation and abduction and would not include inferior or superior glide. I hope that clarifies how we are defining some of these terms. Dr. B2 Dr. B2

Just curious: Is excessive elevation something you would try to correct in an upper crossed syndrome situation? Wondering what would give a personal trainer an idea to have a client do this. I’m a NASM CPT at an Anytime Fitness, and am strongly considering the CES program, bcz of the benefits I feel it would have. TY.

Great tips Brent. I have a female client that she can't do a McKenzie press up. When I put her in this position her arms must be far away in front of her and she can raise her torso about 30 degrees. Do you think it's a structural issue, a muscular issue, a issue of the fascia, a bonny issue ? She says to me that she feels like a wood. So stiff. Do you have any recommendations ? Take care

Hey George... hard to tell without seeing her. I would need to do some other assessments and maybe some manual techniques to see if I could identify the restriction. Even then, you may need to send for imaging to see if there is in fact a structural issue. Wish I could have been of more help, Dr. B2

why would you want to keep the scapula depressed when scapular upward rotation is necessary for full flexion/abduction movements?

I think he's cueing to avoid elevation since excessive elevation is a common problem. I don't know, just a guess

well this is an activation drill, to stimulates the trap, keeping scapula depressed will help the maintain tension of the lower trap through the range of motion

You can depress and upwardly rotate at the same time, those are two separate joint actions. What we are trying to avoid is elevation combined with downward rotation and anterior tipping, which often occurs as a compensation during exercises like this. Dr. B2

if depression is the inferior scapular angle moving in a caudal direction, and upward rotation has the inferior angle moving in an abducted ceph direction, how can you do both together??

If the movement causes pain or exacerbate symptoms it should be replaced by an exercise that does not cause pain or exacerbate symptoms. Obviously pregnant woman are going to be uncomfortable in prone on a ball. In summary, use common sense. If it hurts, don't do it. Dr. B2
