I'm gonna say this is a result of improper fastener tightening, either by torque or stretch measurements. Proper parts, proper procedure, proper tools. It's all got to be right and done right to do well and last.

Amen about using Molybden lube on threads and contact surfaces! But even with Molybden lube, torque VS internal bolt stress can vary + - 25% in high torque applications. Most accurate way is to measure the stretch of the stud while tightening. Love the videos, keep it up!

What's the name of that allen head bolt at the front of the motor or the recommended torque spec for it?

Someone didn’t follow the procedures on the rod cap bolts. Looks like as simple as forgetting to tighten to spec. I’d be surprised if the arp bolts failed, more likely after seeing the lack of moly lube or conventional oil on the head studs. I’d question who assembled this motor before parts failure personally. The bearings look like the wrong clearance as well.
