I know someone who is extremely restricted in internal rotation, has a positive result on fabers, passive hip flexion sends the knee way out of line away from the body, very externally rotated foot. Could this be something to do with the capsule? Glutes not firing correctly, quads tight, as are adductors.

Sounds like a hip pathology to me, if they are older then maybe OA - otherwise look for a labral tear of a FAI....regards JG

So what does one do when they have a tear ? I've been told I can't have a repair due to arthritis. I want to continue exercising but I get pain in the groin & also electric shock sensations in the glute. I was strengthening my glutes before but cant say it's making much difference & my hip flexors are constantly short & tight no matter what I do

Emma claire hi Emma, you can try a guided ultrasound steroid injection first and see if that helps reduce the pain and take it from there...good luck, if you come to oxford I can have a look and let you know what I think. regards JG

Meee tooo. I had FAI surgery 7 years ago... Thinking it's time for a hip replacement or at least a revision. I'm only 35 tho so trying to avoid a THR.

So if the Faber and Fair tests are positive, are the solutions mainly surgical, or potentially, can extensive work on improving the ROM (through MET, stretch, muscle training...) work?

Tricky one to answer as a lot of my patients with a Labral tear / FAI are normally women with mobile hips so stretching I believe is not really recommended - if they train their Glutes then they normally say that they struggle to engage these muscles - this is because the hip pathology is neurologically switching off the Gmax and maybe Gmed so the muscles becomes inhibited and normally the psoas is facilitated as protecting the hip so back pain follows. I suggest a U/S guided steroid injection into the hip before surgery is indicated as a lot of the time it will settle down after the injection. Hope that helps, regards JG

can one of the reason that hip joint restriction of external or internal rotation is about anteversion or retroversion of femur to? i mean problem of centralization femur in acetabulum. so i think we should do craig s test to. what do you think?

I agree that if you have an anteverted or retroverted then the external and internal rotation will be limited so worth checking first. Regards John
