Knowing what you might need for the coming year and years and which way to go, it's a game of chance. I'm 76 on an advantage plan. Two months ago I wound up in the ER and had a ERCP the same day and admitted over night. ER was covered 100%, the next day I felt much much better but I was scheduled for gall bladder surgery. Long story short, prior approval was required or the advantage plan wouldn't pay. I wound up signing an AMA (against medical advice) and since it was a nice day I walked the mile home instead of calling for a ride. I still have my old gall bladder and so far still feeling just fine. Time will tell if my old gall bladder stays or goes if it goes it will go with prior approval from my medicare advantage plan.

That's what I don't like about the insurance companies. When I had cancer in 2014 my oncologist wanted a PET scan done but the insurance wouldn't approve it until I had a Cat scan first. Two completely different tests. Having to schedule both just took more and more time. Under G or N plan I would have had the PET right away. In your case why didn't you want it removed?

Socialized system is good and bad. Good in that no one goes without health care, unlike in the US. Bad in that there are rules that make it fair to all but can mean long wait times. Most countries that have these plans also have for fee plans you can buy to ensure you can get into a private doctor right away. This is also starting here in the US in the form of concierge medical practices.

@@BuzzRetirementGarage wow that is a high deductible. And another reason to avoid the private insurance company "Advantage" plans.
@BuzzRetirementGarage2 күн бұрын
@@JBoy340a Actually we had a unique situation. After the $1500 is met we pay nothing the rest of the year. For single people it was $500. The company would pick up the difference. So if the insurance company had a 7,500 deductible, which lowered the companies overall premium, the company would pay the difference between the $1500 and $7500. Only a dozen people each year would go over the $1500 so it was a win/win for everyone.

A couple of notes: on plan N ER visits can incur a co-pay ($20) just like a doctor's visit and excess charges above Medicare approved charges can be billed, but in practice this is largely mitigated; just ask in advance if the doctor accepts Medicare rates. Regarding Plan D; some of us are subjected to IRMAA which makes Plan D quite expensive if you have no Rx and are healthy.
@BuzzRetirementGarageКүн бұрын
Fortunately, I live in one of the eight states that do not allow excess charges. Paying for D at a high cost because of IRMAA and not having prescriptions is a drawback. Thanks for the comment! Buzz

Everyone has their own preferences on this issue. As long as i can afford to not have to think about networks or approvals, I'm going with a supplement. I do not like PPOs or HMOs. I've been very blessed to hardly ever have to go to the doctor, but that will change with aging, I'm sure. And part of the reason I didn't go was because the network changed every other year or so. I'm going with N also when I sign up at end of month. I love Retirement Nerds, but the agent I'm working with is at Abt Insurance, another informative KZbinr.

I switched from Mutual of Omaha this year because of concerns about closing a book of business. It seems most companies sell supplements for a period of time , then close that book of business and start a new book of business with cheaper rates. If you're left in the old book, you could see higher rate increases. In Ohio, to switch, you need to pass medical underwriting, I decided to switch while I am still healthy.
@BuzzRetirementGarage3 күн бұрын
Things like that I'm not well versed on. I met with a finance guy this morning and he said rates also come from your income. I never knew that either.
@glennheath61504 күн бұрын
Didn’t realize how involved medical plans are in the US. In Canada we have universal healthcare so there are no monthly fees, deductibles or different plans. Healthcare is paid for through taxes paid to the federal and provincial governments. That cost is enormous and is increasing every year. Quality of healthcare is generally decreasing as costs increase with many Canadians choosing to pay out of pocket for procedures. No system is perfect. Good video Buzz.

I pay $0 for my PCP, $30 for a specialist (I see 11), and $100 for the ER. I also get a card loaded with $65/month I can spend on healthy food and OTC meds, because I'm on the diabetes (don't have) and heart disease (have) PPO plan. I have had 4 surgeries and they have all been approved no problem, and my most expensive surgery, with a 2 might hospital stay, cost me $365.

Count yourself as fortunate so far. Many horror stories about MA. It boils down to it’s great until it isn’t. With a Supplement I can go to any medical provider (that accepts original Medicare) anywhere in the country without a referral. I currently use two different area suburban Chicago medical networks, one we have found is far superior to the other. And the worse one sent out a notice to all patients they will no longer accept Aetna Medicare Advantage. Do yourself a favor and do some research on problems with Advantage plan prior authorizations and billings.

My husband and I live in Georgia and we both have traditional Medicare plus a Plan G supplement with Mutual of Omaha. We are happy with the company but one thing our agent never told us is that when you originally sign up (and I think for the first 6 months), any company will accept you for a supplement plan without any medical underwriting. After that time, if you want to change companies, they will ask you medical history questions and they may impose limits/exclusions of coverage for pre-existing conditions or they could even decline to cover you at all. So it is important to choose a company that you will be happy with long term and one that is financially strong and is unlikely to go out of business. Few people reach age 65 without having some pre-existing conditions so this potentially affects almost everyone. So you're kind of a captive audience and have to hope that the annual premium increases are reasonable. We picked Plan G because we liked the peace of mind of knowing that once we reach the annual Medicare Part B deductible, we have no out of pocket expenses (except for occasional co-payments for drugs under our Part D prescription drug plan).
@SomebodySaid...3 күн бұрын
KZbinr Dr. Ed Weir says that insurance agents make the most money for themselves when they sell you an Advantage Plan and that they make the least amount of money when they sell you Plan G. He also says that Plan G is sold to you for a set amount no matter what insurance company is selling it to you.

I watched the Retirement Nerds video for Montana, the other day, and he helpfully pointed out how Advantage plans aren't the best in rural areas like ours. We don't have any real big cities/metro areas in the state, but we don't even live very close to any of the not so big cities. Heck, we're an hour from the nearest Walmart. I was already leaning toward a supplement plan, but the rural angle was something I hadn't even considered. At this point we're thinking the best bet for my husband is the high deductible G plan. He goes to the dr. every 2 years for his DOT physical and that's about it. I'm split in my opinion between that and the N plan, but he prefers saving money up front and if something came up where we had to pay that deductible, it would be a minor regret but nothing that would set us back too much. As for me, I won't be eligible for Medicare for 8 more years so I'm figuring out an ACA plan. Hubby's got about 30 days left to work!

We have learned a lot from caring for our parents for the last 10 years. My mother-in-law has lived with us for about 3 1/2 years now. She is on original Medicare with a “Medigap” plan through Blue Cross. My parents were on an Advantage plan (commonly called Medicare part C) through Humana which, according to the brokers around here, is the best one to go with. Both my parents and M-I-L’s plans paid very well but her Blue Cross Medigap plan is very expensive. I’ll be signing up in January and am leaning towards the Humana Advantage plan but will visit a broker I’ve been referred to first to make sure.

My MIL and FIL had an Advantage Plan that was an HMO and they liked their doctors, so it worked out for them. They were paying $60/month each which gave them a lower deductible. What I didn't like was my FIL broke each hip at different times and being in his 90's they kicked him out of the rehab nursing home after 30 days and he really wasn't ready. It would have been somewhere around $300 a day for him to stay. If he had an F plan, I think he could have 100 days stay before paying. Water under the bridge now. Buzz

Thanks for the info. I'm in the Cleveland area and my plan G has gone up to $152 and will probably go up in 2025. I'm going to ask my agent about plan N. I can do a lot of golfing with the money that I would save. My mother had an advantage plan. When the hospital wanted to send her to rehabilitation, they said she could go to Mt. Alverna. ( it was where my Mom's sister was). Her insurance wouldn't let her go there. They sent her to a dirty place in Parma Hts and Mom didn’t last 2 days. Get a "letter" plan if you can.

I was on three different Plan G supplement insurance plans over seven years but tired of rate increases every year, so I switched over to Plan N last June. Almost $100 a month is savings alone in that move. At 72 I am still very healthy and had no issues passing underwriting switching G plan insurers or my move to Plan N. As far as Advantage Plans go, I would only suggest those to the broke, cheap, or in the right circumstance, if you were retiring from a Mega Corp. that offered an Advantage Plan as these are generally superior from what is commercially available. Don't get cheap with your final years of life with your chosen medical insurance please.

I also went with Plan N, at 65 my premium is 92.00. If you don’t sign up for a supplement at 65, you would have to be subject to underwriting. This costs less than Plan G. But higher than the free advantage plans, but maximum out of pocket will be lower when health issues arise. The premiums will go up every year, so be prepared for that.

Buzz, I just switched from Plan G to Plan N. My Plan G had gone to $198/month, my Plan N is $111/month (age 69 in NC). From what I understand about Plan G is that when you move from your employer insurance to Medicare and you decide on a Medigap policy, Plan G, has to insure you without any medical review. Hence more people with medical conditions that go on to Plan G have more claims and the rates rise more per year. Plan N, I had to answer medical questions to get approved. Plan N does as you say require a co-pay for Dr. office visits and does not pay excess charges.(I usually go to the Dr office 1 time a year for the Medicare check up). All the information I've have seen about the excess charges are that is is very rare that medical services charge excess charges. My Plan D with Wellcare also went to $0. I think the insurance commissions are made on the Medicare Advantage plans of the Medigap letter plans.
@BuzzRetirementGarage3 күн бұрын
I never heard that about the medical questions for plan N. Interesting. Thank you for the comment! Buzz

"It's Free..." Yes, most people don't realize that the Medicare supplement insurance agents work on commission. but they are paid by the companies, NOT the customer. Also, Medicare Advantage plans pay the agents MUCH MUCH MORE in commissions, which is why many agents push those plans over supplement plans.

Skipping out on Part D can be a bad idea as the penalties accrue by month, not by year. Sucks to be paying into a meds plan when you use $0 meds but that's the socialist way. Medigap Plan G covers Medicare Part B excess charges, while Plan N doesn’t. If a health care provider is legally permitted to charge more than Medicare’s approved amount and does so, what’s left over after Medicare’s approved amount is an excess charge.
@BuzzRetirementGarage2 күн бұрын
Correct but in eight states including my state of Ohio excess charges are not permitted. Also, Brian from Medicare 365 put out a video today about N and G and said he's not heard of anyone charging the excess fee. Thank you for your comment. I appreciate it. Buzz

If you are going to a health group of doctors, or a hospital it's all or none for excessive charges. They don't allow one in the group to do excessive charges. Also Medicare penalizes those those who do excessive charge. So the doctor don't get the full 15%. They have to bill you directly, they cannot go through Medicare to bill you. Check those in private practice, physiologist, psychiatrists etc. Simply ask every every new doctor before you see them. Also check all forms signed as a new patient as well. Private practice can also be private pay as well. Meaning they don't accept any insurance.

So, for me and my wife, $185/each for B. No way to reduce that one. $92/month for plan N. Drug plan zero monthly cost this year. I'm working on dental and vision costs. When you get your book, read through the plans and then google them for specifics. An insurance broker would be worth talking to.

Our system here is so much easier. Citizens are all covered by Medicare and it's paid for by a small tax on everyone who is working/paying tax. This is the public system and treatment is mostly free. The other system is the private system where you pay for health insurance, which can be quite expensive, and can include extras such as Dental, Optical etc etc. The issue with the public system is that you may have to wait quite a while for treatment for things they deem as elective and waits for things like a knee replacement can take quite a while. Going private will likely mean you get treated much more quickly. My wife had a back op. a few years ago and was booked in to have it done at a very nice private hospital within a few days.

Advantage plans are great if you are generally healthy and don't travel within the U.S. much. When you begin having health problems and need treatment(s) is when people start seeing the downside of the advantage plans. You are putting decisions about your health and needed treatments into the hands of a for-profit company and thus you're likely to run into more obstacles of getting treatment plans approved. You save now, but pay more, and have more difficulty getting treatment in later years with the advantage plans.

Depending on the person and frailty, If you need physical therapy or chiropractic or both which can be multiple visits you might break even on G assuming you sprinkle in a GP twice/4 times a year, cardiologist probably 2 a year , opthomologist once, maybe some other specialties initial visit then a followup or two but , just normal average doctor visits for a mildly healthy person probably wouldn't get you there , although I've seen folks go to PT months and month on end 2 visits a week especially stroke and even some pelvic floor PT , same thing with chiro 2 visits a week for months. So those folks...it's peace of mind that everything is covered "Carte Blanche" style., I think G covers excess charges (if some doctor goes higher than standard reimbursement) and N doesn't although that's pretty rare.

I didn't realize Medicare released the 2025 premiums Friday. What is the difference between an Advantage Plan and Supplement? I assume a supplement is known as MediGap. The problem with an HMO versus standard Medicare B is that they require a referral to see another Dr which is a problem in an emergency. It might be cheaper but it could be bad if you need a second opinion.

I think the cons for the advantage plans are if you live in a rural area. Then you are limited on doctor choices. You do have a higher deductible. check with an agent, I think your work insurance has to comply with the changes to the drug coverage (inflation reduction act now capping MOO for drugs to 2000 a year) and if not in compliance they end up penalizing you later when you sign up. That's the only thing I'd check on.
