As for tier 1, supposed to deal with rockets, it too failed several times, causing damage and casualties.

Efficacy of Air Defence Systems The Russians (and the Yankees and Europeans agree) that all current air defence systems have only two levels of efficacy: 0% or 50%, and nothing in between. In other words, either you miss (i.e., 0% efficacy) or you need TWO interceptors to kill one incoming rocket (i.e., 50% efficacy). So, 92% efficacy is a misnomer. It is only 50% efficacy. This does not include those interceptors that chicken out and return (this is awesome, even the Patriots couldn’t do that). That’s the first important point. The Second important point is the time takes to reload…. In most cases, it takes 20 minutes to do so. Therefore, if the enemies swamp their rockets within 20 minutes (Ukraine and Gaza) the air defence system cannot re-load in time to launch a 2nd wave of interceptors. (NB: You can see this in Ukraine with the Shaheed-136). So, let’s look at the numbers: If you have 10 batteries (each battery covers only 50 sq km… small area!!!) with 20 interceptors in it, you can shoot down only: Efficacy Factors: @ 50% efficacy = 50% x 20 interceptors x 10 batteries = 100 hits. If the enemy fires 1000 rockets, then it is only 10% efficient. The third and most important point is the Cost Factors: 200 interceptors @ USD150k each = USD30,000k or USD30.0 million…. Vs 1000 rockets @ USD8k each = USD8,000k. this is 30,000/8 = 3,750 times….. or 0.027%.... (8/30,000) If you Fire 5,000 rockets per day the enemy will go begging China to borrow money in Yuan within one month…. That’s defeat on the Economic front. kzbin.info/www/bejne/rp6rk5hnjdeXoZo And the Patriots (costing some USD4.00 million each?) have the same efficacy problem whilst ‘defending’ Saudi from the Houthi drones…… Aramco just suffered a big destruction…. Good analysis, with one correction, though: "have the same efficacy problem whilst ‘defending’ Saudi from the Houthi drones" They are not "Houthi" drones, but launched by the Yemeni armed forces allied with Ansarullah, a broad-based political alliance that goes well beyond the small Houthi ethnic group.

As earlier comments point out, Iron Dome is quite expensive, but it's also not very effective. Think of the rockets fired by Hamas as bullets fired from a pistol, and Iron Dome as analogous to a bulletproof vest. In this analogy, Hezbollah's arsenal consists mostly of rifles, against which a bulletproof vest is ineffective.
