What questions do you have about the EVO ICL procedure?

I had an ICL procedure 4 months ago. However I am bothered a lot by the halos when my pupil exceed the optical zone of the lens. I have zero other complications. I would like to know if I remove the lens only due to dissatisfaction of visual disturbances, can my eye health and best correctable visual acuity be the same as pre surgery state?

@@yuanyi827 In short, yes, you can remove the ICL and your vision would return to pre-operative vision, as long as there are no complications. However, I usually recommend using prescription eye drops to help regulate your pupil size before undergoing surgery if you are generally satisfied otherwise. Your doctor should be able to discuss your options.

Thanks for your reply!! I have discussed with different doctors about my current situation. I was also prescribed with the bromindine eye drop, which works good. However I am in a bad situation now because I am not satisfied with the visual acuity either. I am a kind of person if I don’t see 20/15 I feel bad. The toric lens has -7.5D sph power and -2.5D cyl power in both sides. The lens in my right eye has rotated by 13 degrees so it gives me a -1.5D residual astigmatism with no residual spherical power. On my left eye the lens hasn’t rotated but the spherical power isn’t quite enough. I have about -0.5D residual nearsightedness. So the vision isn’t quite sharp either. Many doctors suggest me to do the rotation procedure but a doctor told me after the rotation procedure the residual astigmatism would be significantly reduced but I would have a minor residual nearsightedness about -0.5D, which is similar to my left eye. My visual acuity is about 20/30 to 20/25 in both eyes, which is bad.I could see 20/15 with glasses before surgery despite having a high power. Now I am still wearing glasses in order to see 20/15. And the halos bothers me a lot. The only good thing is that I don’t have any complications like high eye pressure or inappropriate vault. The Endothelial cell level hasn’t changed. And change of the curvature of my cornea is also negligible.(about 0.1D) So I am considering to remove the lens if I could go back to where I was before the surgery. My surgeon told me that after the removal my eye would be pretty much the same as pre surgery. I would like to know how much damage would the removal surgery do to my if it went uneventful.

@@yuanyi827 Hi, sorry you're experiencing some issues after the ICL. If the lens rotated, then an attempted rotation (or exchange) to a larger diameter ICL may be warranted, especially if your vision deteriorated to 20/30 and your vault can support it. ICL exchanges are a bit more complicated than primary insertion, but can definitely be done safely. I think some of your glare problems are due to the residual refractive error, especially if brim didn't help much. Have you tried wearing Rx contacts to correct for the myopia and astigmatism and see how it compares to glasses? If they help, then even LASIK or PRK can be performed with the ICLs in place and you'll be all set! Removing the ICLs also isn't without some risk and leaving you back at -6.5 or whatever your preop refraction is is really a shame...

There is a misunderstanding. The bromidine eye do help, it actually eliminates all the glares and halos issues. The glares and halos that bothers me is just because of the optical zone is too small. I just don’t use an glaucoma eye drop everyday in order to get rid of the halos. Could you tell me how much damage would the removal do to my long term eye health and best correctable visual acuity, if it is performed correctly or uneventfully? My lens is unfortunately EVO lens. I am also considering an exchange to EVO+, which may help the halos and resolves refractive error. Can you give some information about the comparison between the ICL exchange vs ICL removal in terms of the impact to the endothelial and long term eye health? Thank you very much!!
