Bipolar disorder is a complicated condition as you know, where patients have periods of manic behavior. When a patient is in a manic phase, there are often psychotic features, features that are delusional, where reality is not something that they can get a grasp on. They don’t sleep, they feel grandiose, they may not be able to take advice. And sometimes even there’s a risk of hallucinations.
So if you have a patient in a manic phase with psychotic features, THC, one of the active ingredients in cannabis should be withheld. THC in patients with bipolar that are having a depressive phase, however, can help lift the spirits and help with mood. We know that it helps with major depression, and we know that it helps with the depressive phase of bipolar disorder.
CBD, on the other hand, can be a very effective antipsychotic. We know from studies in patients with psychosis, that high dose CBD in the range of 600 to 900 milligrams can work as well as Amilsulpride.
Anyway, the point I’m trying to make is that CBD works for psychosis. THC works for depression, and in patients that we treat that have bipolar disorder, and cannabis therapy. We’re careful to regulate which cannabinoids are used during which phase of their illness.
