Mixing should be done on the main deck not inside the space ; Flame Proof Lighting should be used not normal lightning.

How did flammable gasses accumulate and go unnoticed if they where doing regular gas checks?

The device used for the gas checks prior to entry didn't measure the flammable vapours. It was looking at gasses like CO, oxygen, and likely nitrogen

Gas check done at entry point. But not done when paint mixing and drying inside confined space. If gas check done during this activity the flammable LEL reading would have been detected by a multi gas meter. Forced ventilation used was not strong enough to counter the flammable gas accumulation in the confined space.

Paint mixing should be done outside of the confined space. Explosion proof lighting should be used inside confined space. Forced ventilation can be done using a more powerful blower. Gas checks to be done on more regular basis and noting when painting is in progress. A workmen inside confined space space should wear a clip on gas detector that can warn of flammable gas buildup.
