I remember jumping out the hole whenever it would chirp every minute or two when those toxic gas detectors would go off. My coworkers would laugh at me and get mad at me saying I'm wasting time and that the device is likely not calibrated, so they made me take off the harness and let someone else in. they avoided the chirping noise as it went off and they were feeling faint so we had to reel him in, if we weren't there with the tripod lift he would have died. Lesson is, if your device is constantly chirping its very likely not broken/miscalibrated. Some gasses are odorless and constant, even with mechanical airflow

How dumb do you have to be ignore a *safety* device that you suspect is malfunctioning. Even if there wasn't any toxic gas, a malfunctioning safety device is still a major issue. Natural selection is a bitch that discriminates against people making dumb decisions.

The fact that the supervisor went in baffles me. If anyone should’ve been able to recognize a confined space hazard and how to handle it, it should’ve been them. Tragic and completely preventable loss of life for all involved

Take everything they say with a grain of salt. It's the truth you can check now. So if your supervisor is doing anything outside of what is in accord with fact and reality. Report them.

I can barely count the number of infractions committed here. 1) Didn't test the atmosphere before entry. 2) Didn't perform continuous atmospheric testing. The device to do this is about $600 and everyone has to have one. 3) No top man. The task of the top man is to continuously monitor the person in the confined space. There must be one for every person in the space. 4) No SCBA unit. Absolutely mandatory. 5) No extraction plan. There must be one specific for the space & everyone must be trained on it. 6) No A frame or other extraction equipment 7) No one is wearing a parachute harness. 8) There didn't appear to any method of emergency communication.

They also didn’t follow proper emergency procedures and they lacked proper emergency procedures as when the first worker collapsed instead of initially calling 911 and waiting for proper trained and equipped people to rescue him the other workers went into the space without proper training and equipment in an attempt to rescue him and that was how they became victims themselves.

@@jfournerat1274 I can agree with all of that except for calling & waiting for EMS & fire rescue. That is what we did 30 years ago but that will now earn you a prison sentence. You must have a confined space entry plan that is specific to that site, everyone must be trained on it and it must include an extraction plan. Lack of an effective extraction plan is negligence & if there is a fatality it is similar to criminal negligence causing death. The offender is liable to 17 years in prison which is what it should be. I work in a water supply utility and confined space entry is what we do. The laws were changed due to the number of times crap like this occurred. No job is worth dying for.

Waiting for ems and fire rescue is often not a good idea as it often takes more than 4 minutes for them to arrive and by that time they are already dead and it would be considered neglecience and worth being sentenced to prison for. That is why confined space safety training and confined space emergency rescue training is so important so that way if an emergency does happen then you will have a emergency response team that is standing close by at the work site that will be able to get there as quickly as possible and be able to rescue the person as quickly as possible. However you still need to call ems even if the person is ressurcated so that way they can go to the hospital and be properly treated especially if they had a lack of oxygen or inhaled poisonous fumes.

Always wear a harness, enter attached to a line connected to a winch on a tripod or davit. The attendant may have been able to winch the first victim out immediately.

​@@psycholaw4394 Test the atmosphere before entry: purge as required, ventilate. Each entrant to use a portable gas detector. Depending upon what you expect to encounter / results of atmospheric testing and the efficacy of purging & ventilating, you may need breathing apparatus (SCBA, Airline or Emergency Escape). Air-purifying less likely to be suitable.

I like to start with to safe. Never run any sort of saw. That's a little too much but never run a saw one time halfway if you set up to saw one board you set up just like you're sawing a 100 boards on the same spot. I believe power tool operation should be a superstition it should be a religion and it should be safe and all three of these things should be combined into a set of rules that make you nauseous when you break them. You better be tight as a piano wire when you are carving with a chainsaw and you might as well have as much protective gear as you can get because it's ugly cutting.

Thankfully, the biggest health risk there was likely co2, unless you were near a leaking sewer or there was a decent fire inside.

You would have been fine if what you’re saying is true. Large underground facility allows plenty of ventilation - it’s not the same as a confined manhole.

Like all the way to homeless people and teens destroying stuff to "vigilante" sabatoge or people stealing scrap and messing up the faculty, lots of things. And if a fire starts deep underground isn't that dangerous too idk, manmade structures that are abandoned can often be dangerous lots of gasses and things, people have found dynamite in mines and it sets off, structure collapsing

last week i got cursed on, i smelled h2s gas right away.. and was told to move into the manhole. i refused and advised him to stay away from the manhole. i did not respond to him and i just strait ahead to get my gasmeter from my van. turned it on and went back.. my meter (drager) gave a a1 alarm 2 meters near the pit and a2 near the pit. there where already 4 people standing next to it. all mad on me, but they where shocked that my meter was bleeping when i explained to went off because of h2s like i said. and i said again, come at least 10 meter away. i am not kidding. they did go 10m away but those where still discussing about how i can measure h2s. after 20 min. they asked me if i could down just to check the basics.. i called my project leader and he was really mad and told me to leave. and told them they should be ashamed and they have to pay for my waisted time.. got back to the office. and had to do my story to the others about how i choose the right things to do. and how i handled the situation. i do understand some don't understand that it is risky, and i understand some don't understand how dangerous it can be. but getting cursed on by 4 man in suits for not doing my job, while i explain why its way to dangerous is next level stuff. crazy i right away understood he was one of those, so getting my meter was the best i could do. maybe i saved his life as well. he could pass out and fall in the manhole.

You're right. Many times these accidents happen because they just "want to be done with the job quickly" and safety "wastes" time for the employer. The most important thing for a worker is to be assertive and not to get pushed into doing the job if there is something wrong. Just walk away. It's not more important than your life.

@@cubedable i've always been that guy when 10 say no. i still say yes. i don't feel that pressure, i dont care if you sceam or ask friendly, my co worker on the other hand.. i told him you stay away until i got my meter. no matter what they say oke? he agreed and i went. to my bus. he was also shocked by the result of my meter

rats die all the time in sewers. but not all sewers contain gasses. constant flowing waste water does not produce gasses. its when still standing waste water cannot get in touch with air for 3 day than it will start produce h2s gasses. for example sewage pumps that have a few miles /km of pressure pipe that start not many times the outcoming water a week later produces gas. same for pump pits. that have sludge on the bottem.. when the pump starts some raise to the surface and gasses will come out.. the reason those guys died is because they pumped away the shielding water

I can't tell you how many times they tried to put 100 thousand lbs on a truck rated for 60 thousand lbs or fake the paper work. I recall reading about comedian Eric Andre wanting to make a prank where he jumps out of a sewer and scares people. He sent a request to NYC authorities for permission to enter a manhole. The authorities informed him he would have to wear a hazmat suit because of dangerous gasses that accumulate in the sewers. They decided not to do the prank.
