Thank you for teaching! You explain so good. I have been suffering for 22 years and I’m 48. Had back surgery and the Dr messed me up on purpose, well it was a lot of other people too. The Dr got caught and went to jail. He got to have a pain clinic since he lost his board license for what he did. He shouldn’t of had anything. God bless you for all, you do, Dr.

17 year back sufferer here and also 48 years old, had a prior 5 years of suffering from chronic appendicitis so 22 years of agony for me as well. I had a similar situation with a clinic when I traveled to Vegas out of desperation. Lot's of fraud and I think they lost their license or were shut down. I honestly have 100% PTSD from the medical system at this point. Just sickens me to the highest degree.

Your videos have brought me to tears multiple times already. The medical system has cost me everything once already and I feel like they may do it again soon if I stick with them. They keep seeing nothing that would cause the pain where I am experiencing it, acting like I am crazy. With your videos and hopefully some training with you maybe I can live again some day. 16:30 I’m not sure that the ligamentum flavum is relevant for stability. It is highly elastic, so it can’t at the same time be relevant for stability. I think it is mainly a protective sheath between the vertebrae to cover the spinal cord and to adjust the spinal canal easily when flexing/extending. 24:08 in most other posts they would say that looks like a compressed nerve root. In my mind the only difference here is that this is by ‘design’ as no abnormalities are visible on the left. 1:29:50 cervical disc pinches are extremely rare, you said that yourself a few slides ago. The nerves don’t exit quite at the disc level, and the posterior longitudinal ligament is very thick and wide at the cervicals, unlike at the other vertebrae, preventing bulges into spinal. Never mind that the cervical disc loses the nucleus in every human by age 40, so that leaking out is normal and does not cause problems. Doctors have a sub collective that tells them to blame the disc for everything cause it used to be hard to contradict them. Saying that the c disc is prone to herniation: yes it literally bisects. The lamella are weak in the corners because that’s where the normal split starts. Anatomy and physiology of the cervical spine, 1990

DC .BC Douglas Gillard : your videos are dedicated to the cervical spine are very excellent (i have not any formal education but i m read on the network (some websites) and very listen also to your lectures especially the lectures on the c spine. you do a very good job to these lectures. (also u have others but i watched them more often ) I have a question about the nuchal ligament :(what spinous process are connected to this ligament nuchal (this ligament are very important to keep the head in the normal position (avoid that the head falling down , limiting the flexion of the neck .(so what can be connected to this ligament from the spinous process of cervical (just c1 and c7 more but also the other are very important ? thanks

Hello my name is Scott Roy I got hurt like 20 years ago I had a bunch of lights fall on me that was suspended from a ceiling and a factory I could move my head left or right for 6 months went through 4 months of physical therapy and I've always had problems and they say there's nothing really wrong I have medium to generation of the bone and of the disc of C2 and I'm not sure what the other ones is I have all my records I would really like to learn a lot more to find out how much danger I probably really in I'm paying all the time and life is very difficult for me it's hard to find any doctor that knows really what they're doing I've had to do my own research and now watching this video I found out what I need to know they said I had spinal stenosis which that's definitely not it hope you get this message I would like to really find out more or at least a real doctor to go to

Hello dr. I have wuestions anout MRI and Xray. I need ur second opinion about my neck and my thoracic back problem. Are u prefer Stand up MRI and stand up Xray OR lying down Tunel MRI/Xray ?

You always want to try to get a three Tesla MRI (closed tube) and ask for 3 mm cuts with one series down the plane of the disc. Avoid open MRIs, standing MRIs, and flexion/extension MRIs for the quality is usually horrible. A good MRI should take about 50 minutes to complete. If they rush you through in 20 minutes, the quality is going to be poor.

My problem : (Mid back/ THORACIC) got i injury from agresif Chiropractor (i feel like stubing knife in my back). I cant sleep side to side because very painful and made me unable to breathe. And also i can't lean my mid back/THORACIC against to chair/car seat/sofa as it preseses the spine, neck spine through to the head. Neck : i have tiny bulging disc in my C4-C5 C5-C6 C6-C7. Looking up, down, side it very hurts. Always tense and the pain goes down to shoulders, shoulder blades and arm. C1, C2 seems to shift and give rise to a TMJ. I went to many specialist Dr in houston TX area but all of them said everything looks fine & normal (according yo mu MRi and Xray) and they send me home again. All they do just give me drugs/meds. In this situation i am disparate to find help and answer. And finally cause anxiety, stress and depression with my chronic pain :-( I am suffering for a year, i cant work anymote, cant cook, cant take care my family anymore. Everyday is suffering :-(

dr Douglas (in time 08:56 u show ct scan in right screen very normal lordosis curve (this was filmed by man lay down (and also your mri (u show in the video (how to read MRI of c spine ) u do the MRI laying down or siting or standing ? and what do think a bout position of do the MRI SPINE ? its better to do in normal standing or sit ? thanks

Good question. I am not a fan of upright imaging. Very very rarely doesn't show anything clinically significant that is not seen in a supine MRI. And the image quality is usually substandard because they can't use anything greater then 0.75 tesla. I always recommend my clients get a three Tesla MRI (closed tube) with at least one series of cuts down the plane of the disc. You also have to make sure that the radiology center is not taking shortcuts and running you through the MRI machine and 20 minutes, which yields crummy images which are often barely of diagnostic quality. A good MRI should take 45-60 minutes to be done right.

​@@douglasgillarddcprofessoro5285yes i read the open MRI limited to 0.75 T (so it is low quality) but i ear (some D.C on lecture on stenosis tall that MRI on supine missed thinks (but this D.C it not expert to read MRI U *WHO IS CREATED VIDEO LIKE IN YOUR CHANNEL it is the high level i watched (cervical spine anatomy and MRI read i watched ) so 3 mm cut line on both sagittal and axial(what mean on : series of cuts down the plane of the disc ? i want to say that where u laying down is affected on the lost the curve ? u r very Expert (not just more than 30 years of experience doing excellent video ( See you have invested a lot on this video also other .GREAT channel u have

I am little confused. at time 1:00:00, the axial cut of cervical spine. is it a top-down, posterior-anterior view of the spinal cord? Then, should it be C7 nerve root rather than C6 nerve root? Or could you orientate the picture?

it's a S-I view (top-down) and the posterior part of the vertebra is at the bottom of the screen. The labeling is correct.

I am from India,Since last 12 years My wife is suffering from the pain at1)the base of the thumb of right hand.it is not near the wrist but at the soft area at the base of the thumb i.e. approx.1” above the wrist joint in the palm may be the muscles naming Flexire Policis breves & Abductor Policis breves(names taken from Google) 2)top of the forearm of the same hand at the exact muscles may be naming Brachioradialis /ERCL/ERCB/Supinator My question is can it be naming it as Dr De Quierviains .syndrom. but in her case it’s 1” above.the wrist joint at the palm side The special of my wife’s pain patterns is up to3 years back the pain used to start in the night when she lay down,&was better /bearable when she sits or starts walking which ruined her sleep.& confused many Doctors Now a days the pain is there for almost all day along with the pain at the right side of the neck up to the bottom of the skull & in the scaupla bone of the right side shoulder for almost full day , but gets worst at night, she has to take ‘ ‘Tramadol’ injection or tablet without which she cannot sleep.OR has to keep moving all night which is unexplainable. After watching some video she tried the exercises but the pain got worse from the neck to the thumb Dose it have any connection up to the neck?or is it the Tendonities there also Is there anything different to do for the pain at neck and scapula bone?What else can be done,Pl advice Due to the above my family is very disturbed as 12 years is a big period I have shown her to plastics/Reconstruction surgeon of Forearm he says nothing is wrong with her muscles or vains in hand. Chiropractic (5settings )adjustments made the things worst. Many X rays, MRI of spine, CT scan of brain shows some pressure on the spinal cord at c4-c5 but Dr. Says it is not as per the symptom according to MRI your left hand should pain but here’s RT hand paining,NCT also done many times shown nothing is wrong in it Please advise,or suggest something my family will be very thankful to you.
