Some of that looks like it could e easily scooped out. That suction device is not so accurate.

You tend to avoid using curettage in the ossius portion of the ear canal due to the sensitivity levels of the walls. The guy was lucky, that was looking like the start of keratosis obturans.

In addition to what @pklemming said, you see when he does go in with curettage that it pulls right through and doesn't move it at all.
