The grandfather blames Carter for not preventing this situation from happening. Had Carter notified him of Chase's drug use, they would have forced Chase into a rehab facility. It's very effective, but there could be additional factors at play that aren't included in the clip (or weren't included in the show at all). It's only effective for 30-90 minutes, so it's critical the person be taken to an ER for treatment or they could overdose again after the Narcan wears off. It also takes a big dose of Narcan to counter a strong opiate, and this was a "bad" batch of heroin - possibly laced with fentanyl? It would've been nice if they'd given us an explanation here, though.
