My name is Sanjay Gupta and I am a cardiologist in York. If you would like to speak with me, you can do so via www.yorkcardiology.co.uk or my Facebook - yorkcardiology@gmail.com
Hi Guys
Today’s video is on the subject of chest pain of recent onset
On a daily basis I go to work and go on my coronary care unit to meet the patients who were admitted from the day before.
Many of them have had heart attacks and that is why they have been admitted.
And when I speak to them, I often hear patients say that they had been getting some discomfort a few days before but they thought it was indigestion or muscular or something else and therefore had chosen to ignore it off rather than seek help and I often end up berating them that they should have sought help earlier.
However when I am driving back home I often think in my own mind as to what I would do if I developed chest pains at night and if I was completely truthful it is very likely that I would do the same as my patients (despite knowing better!)
The problem is that all too often we put things off and hope they symptoms will just get better and also our healthcare system is so flawed that it can take ages to see a GP in the first instance.
So I wanted to do a video to help people make a more balanced and educated decision about when to ask for help
There are 3 things that you should take into account when you get chest discomfort:
1) The nature of the discomfort.
2) Your age
3) Whether you have risk factors such as smoking/ high blood pressure or high cholesterol
And all 3 have to be considered carefully
Lets talk about the nature of the discomfort first:
Typical Angina:
Constricting discomfort in the chest, back, shoulders, arms
Which gets worse with physical exertion
And gets better with rest or GTN spray
Atypical angina are if you only have 2 of the 3
Non angina chest pain if you have none of the following!
Risk factors:
If you have any of them, you should consider yourself at higher risk
Finally your age:
And your risks go up substantially from the age of 55 and above.

I started getting angina back In July last year. My age, 63, probably put me in a higher risk group but I had no other risks I thought and was reasonably fit. My first thoughts were, "no, it couldn't be". Only a week or so before I had been exercising quite vigorously with no problems. I had a stent inserted and have resumed a normal life. I haven't had angina since but I'll know it when I do. To me, it wasn't a truly severe pain in the chest. I'd actually bruised my ribs a few months earlier and to me that pain was much more severe. So yes, don't ignore any possible signs. I managed to escape a heart attack but I was just on the edge of having one.

Thank you Doctor from Oz for explaining these risk factors so well. I will put my GP onto this vid as I presented with stable angina symptoms two years ago which have improved sort of over time and am 68 y/o Type 2 diabetic who managed the first 15 years by diet and exercise but now on tabs with exercise. My insistence on a heart check recently had my GP 98% certain it was digestion related. The risk factors as you presented them reverses this on it's head. Thank you again and am now informed and will pass it on.

3weeks ago I have chest pain ,/angina when I'm on gym treadmill 5days in a row. But I notice when I stop walking in treadmill cools off my pain 🥺 Decided to went on ER ,then they give me 3x EKG ,8vial of blood to test and they Cardiologist told me to stay. Two days and perform a stench on me. Told me my Toponin getting higher. But free 6hours idecided to cancelled for I'm not ready for stench. They told me just call 911if got this chest paiñ again. 😲 After 2weeks pass my chest pain lower and sometimes I felt vomit for acid reflux goes up thru my chest. Then I eat some antacids and cool down. Couple day felt muchseens my angina better and my pain subsided. Still my Dr. Advice to check with cardiologist on 09/ 09/21 . What you think 🤔??

In my experience, a basic problem is with the family doctors -- who know they have an individual with family CVD risks but do not inform them of angina symptoms, which can be very subtil, as you say here. So, a person could go years with symptoms and not be diagnosed until it's too late.

Kazim Bozca Google 'pqrst' heart waves. It shows that your ST segment is raised(elevated), at that particular moment.

It all depends Kazim, I understand that you would like to have a decisive answer on your situation. Ofcourse any morphology isn't a sign of something good nor does it HAVE to be a sign of something bad. If you still feel unsure about your testresults, please contact your GP. There are too many variables to this to make any informed suggestions so I can't help you with that (nor am I a cardiologist). I doubt dr. Gupta can help you, based on just 'ST-elevation'. It is best to contact your GP or cardiologist. Best of luck to you.

Great information! Very appreciated. I'm 65 and have been blessed with excellent health, and I think much of that is partly due to a very active lifestyle for my mileage. From what I heard, you may be interested in my effort to journal what my recent experience with slight chest pain: MNEUMNIC - SIORAAA for diagnosing heart pain S - SITE of the pain : Upper chest center, about 4 fingers width above solar plexus I - INTENSITY of the pain (1 - 10) : About a 1, probably not a 2 Q - QUALITY of the pain : Feels like a dull ache O - ONSET of the pain : I’ve started noticing this pain about 4-6 weeks ago, have not seen a doctor yet (I don’t have a lot of trust of the medical community - sorry). No known trigger, except first started noticing when doing Sprint 8 on the treadmill, which also got heart rate as high as 153 bpm at the most intensive part of the Sprint. R - RADIATION of the pain : None that I can detect. It seems like a small area, maybe about the size of a half dollar. A - ASSOCIATED FACTORS : Exercise? Can’t be the only thing. Sometimes I notice the pain come on very slowly when I’m just sitting, working on a computer. Again, not a great deal of pain. A - ALLEVIATING FACTORS : None that I have discerned. A - AGGRAVATING FACTORS - None that I have discerned. Other Notes: My typical blood pressure, that I check regularly, is usually close to, or about, 110 over 65 with a typical resting heart rate of about 55 to 65. I do exercise quite regularly. Every year (for about the last 6-7 years) before spring typically, I get a health screening by LifeLine Screening. When they scan my arteries for plaque, my typical report on that test is Light to Moderate plaque. This past March 2022, my report just said light plaque. Now, interestingly, I do realize since I’ve been working out much more this year than last, I’ve also been eating much more protein, and from sources that I used to eat less often, including, honestly, much more sausage and bacon, ham, and beef. I ramped up this more meat in my diet about 6 months ago. Of course, I also eat lots of chicken and fish. I do love vegetables too. Thank you for reading... Blessings
