I thought another advantage of a sliding genioplasty was it provides length to the chin as well as projection. It also decreases the depth of the mental-labial fold.

I think it depends on how much the bone is advanced, surgeon's preferences, cosmetic goals and the patient's anatomy. When I had my sliding genioplasty, my surgeon used a bone chip graft to fill the gap and edges. For reference, I had a 3mm horizontal and 1mm vertical advancement. One of the things my OMF surgeon discussed was the greater the advancement, the more preferable it is to use a bone graft so that the osteocytes have more of a scaffold for bone to grow into and remodel. If I had needed a greater advancement than what I had, my surgeon said he would need to take a bone graft from my rib. Something to keep in mind about a SG is that when the chin is cut and advanced forward, there is now a bony stepoff which can make an already deep labiomental fold appear deeper, which can be an aesthetic concern to consider depending on the anatomy of the chin. Bone grafts placed in the "step off" can also help soften the fold somewhat by preventing the overlying soft tissues from collapsing into the created space. I'm not a doctor giving medical advice, but I don't mind sharing my experience so that others can have an informed discussion with their own doctors.

Technically it takes time for the bone to fuse back together, so after 12 months it would be solid as before or get to the point. the bone is held by a plates and screw.

bone movement due to trauma to the chin would be just as likely for someone who had a genioplasty as it is for someone who hasn't, because new bone growth fills the space between the previous position and new position! on the other hand it's very common for chin implants to shift overtime even without a cause, and they actually tend to weaken the chin bone (bone erosion overtime due to the friction). with regards to functional strength: overall a genioplasty is definitely the best way to ensure that your chin is just as strong functionally as it was before, because as your bone heals, the chin reattaches to the surrounding facial muscles and adapts to its new position. an implant is just placed into the chin - no adaptation whatsoever, it's generally quite obvious that something was placed since the implant doesn't move with your facial movements because it's not attached to any muscle.

Nooooooooo please don't go there, I beg you on my life, he ruined mine twice, I am now left with a very tight lower lip, I can't smile at all, he did not explain any of the potential side effects genioplasty may have, you donot get enough time with the surgeon, patients are rushed for appointments and usually go for surgery with incomplete information, outof 4 people with whom i interacted with went to surgery with me had to return for revision, I am scarred for life now, I hope I will find some solution to it otherwise I won't be able to smile/laugh in my life at all.
