I love that I can come back to these videos as I'm studying in my 2nd semester of CRNA school! Thanks Sara! Just to clarify for others, the tensilon test is administering an acetylcholinesterase inhibitor medication. This means you're inhibiting (blocking) the acetylcholinesterase enzyme whose sole function is to breakdown acetylcholine (ACh). By blocking the breakdown of acetylcholine, you allow the presynaptic neuron time to release more acetylcholine into the neuromuscular junction (NMJ) and allowing the ACh to bind to the postsynaptic neuron and communicate with the muscle. Whereas before there was little to no ACh in the NMJ. If someone was having a cholinergic crisis and you introduce this same medication into the body you'd be blocking the acetylcholinesterase enzyme and just flooding the already flooded NMJ with ACh and so you wouldn't see any improvement like Sara said.
