The mistake has been happening because of brick shuttering which never recommended for VRCC works. Poor water cement ratio effects poor consolidation. Vibrator can not be used for this kind of shuttering. Ultimately the mistake is on the part of Engineer

I would hope this structure was demolished, the Contractor fired and sued. The extremely poor construction methods on show here would result in a building crumbling in a number of years. Why not just build using load bearing masonry when the quality controls and expertise required for proper RC construction are simply non existent.
