why we do retraction cord option in the last step ? what is the reason of it ?

3 things I do differently: - I use a metal matrix band to retract the gingiva rather than rubberdam and metal clamp. You can angle the matrix band as you desire and then lock it in place with interproximal wedges - I use flowable composite for maximal surface smoothness at the gingival margin. I never ever touch the equigingival or subgingival part with anything rougher than a polishing bur to prevent irregular or rough surfaces. - I never ever blow air on the primer or bonding. This is a very common mistake that should absolutely be avoided. It is advisable to use a clean microbrush instead to soak up the excess primer and/or bonding agent. This technique gives you much better control over where the primer and bonding are and aren't going, rather than blowing it all over the place. Other than that, nice presentation and thank you for sharing this.

Interesting approach! I never thought to use a matrix band for Class V. We're taught in dental school to blow air on the adhesive in order to evaporate the solvent, which subsequently increases bond strength.

@@jdk1313 The solvent will evaporate by itself if you give it 20-30 seconds, but on the condition that you don't use too much of the primer agent. That's why I use a second (dry) microbrush to soak up the excess primer. By doing that you solve the issue of having too much primer and you also give enough time to the solvent of the primer to evaporate

Interesting, thanks for sharing your alternative approach. I have used the metal matrix bands for Class Vs and while I personally don't prefer them, they can certainly be useful! We will agree to disagree on point 3, as there is a wealth of data supporting the efficacy of an air-stream evaporation method rather than just wiping away excess adhesive. Do you have any high-quality evidence you can share to support this method?

@@mentaldental The method I use is largely due to personal preference, though I'm sure that your method would outperform my method in a controlled study environment. The problem I have with this method is that it is harder to reliably reproduce in practice, giving you less control over where the primer and/or bonding agent is and isn't going. I've talked a lot about this with Bart Van Meerbeek and Marleen Peumans (KU Leuven) who both have done a lot of studies on adhesive dentistry. They both apply the same technique you use, but always taught me that you should never blow air directly at the primer and/or bonding agent, but rather use gently air flow over the cavity to fasten the evaporation process.

For foreign students,After clearing INBDE,you have to apply for dental school for DDS,which is like 2&half year course,after that you have to appear for licence exam in particular state where you want to practice in USA,then and then you can practice there

We never recommend beveling dentin, because the resin-dentin bond is not as predictable as the resin-enamel bond and the force of polymerization shrinkage can cause a gap for secondary caries.

Because in the majority of cases, Class V caries or non-carious cervical defects will involve the CEJ. Even if not, by the time you make the prep that enamel layer at the gingival margin is SO thin (remember the anatomy of a tooth in cross-section) that any amount of beveling will put you into dentin.

Why are mesial distal walls divergent? and where to place retention groove and why?

Mesial and distal walls should be more parallel, but the occlusal and gingival walls should be ever so slightly divergent to avoid having unsupported enamel rods. Retention grooves are 0.2mm inside DEJ to provide resistance to buccal displacement.

Drilling in my opinion is terrible cause I don't see how destroying the natural tooth structure and enamel is a good idea your just making that small chip bigger and covering it up, couldn't you just apply the filling? And isn't their a way to restore the enamel obviously not completely but just good enough so the tooth doesn't need to be drilled esstianly making it a healthey tooth?
