I had CMC Arthroplasty with flexor carpi radialis transfer on October 19, 2022, right hand. I’m right hand dominant. My cast was applied so tightly that there was difficulty and a lot of manipulation to get it off; I just about wretched and passed out. 5 months later, my hand is not healing well. It is noticeably smaller than my left and the entire hand feels inflamed with soreness and noticeable, stiff ligaments or tendons (?) showing through on the palm. I had an x-ray - bones look great. Onto an MRI and the next steps. As an aside, it will be an entire month of tolerating the current pain and inflammation before I could get an MRI appointment. Don’t take this surgery lightly. There’s a reason why you may receive a nerve block lasting about a week for this surgery.

This is different from the other ways I've seen to do this surgery. One of those does not use the tendon at all, it uses sutures and "buttons" (I think it's called "suture bridge"). It seems that the recovery time/difficulty varies with the type of surgery used. This video is old; this particular procedure may or may not still be used. Anyone who needs CMC joint surgery should try to talk to more than one surgeon about how it would be done, what is the recovery time, is full movement possible afterward, etc. before deciding what to do. Having a usable thumb is key to having a usable hand. Don't make this decision lightly.

this guy is simply genius and the technique he explained looks extremely solid. the geometry is explained amazingly and involving scaphoid. there has to be a scientific paper for this. can you share? is this just lr without ti but with suspension? what if the tendon slips away in the new pivot point? say i moved my thumb to z-axis (you move that thumb towards my ipad screen that i am looking? or basically i lift the thumb way up) why not having 2 pivot points? is there any risk that cannot get loose with carrying heavy things?

Thank you for explaining. OR, RN, however hands not my specialty. DJD, bilaterally, as well as missed Lyme disease, need bilateral surgery. Pain as you stated is chronic. Good explanation, and why part of tendon is transposed. Thank you for very informative surgical demonstration

Robert, also a RN and very afraid to "pull the trigger" on this (worried of inability to perform procedures at bedside, picking up pills etc.). I did steroid injections 4-5yrs; initially approx every 6mos. The last 8mos was horrible - as they quickly lost effect. The last couple didn't last more than 6-8wks. Lrti on 2/14/22...and with pain meds weaning at 3wks already dramatically less pain. No lies, it's still painful, and the wait to load and strengthen it is long [my surgeon says 6wks]. So while still not functional, if I don't overdo it is less painful. I no longer had a choice, so I took the plunge. Hoping with compliance and PT I will be pleased with my results. Best wishes to you, and thanks again doc. Your props provided further explanation I was wanting that explains the new anatomical positioning (and thus limitation in ROM/movement d/t new placement). It satisfied my piqued curiosity between Therapy visits.

@@TransplantNurse Thank you, finally took plunge after multiple back( epidurals, Dacron, rhizotomy. Left hand now on second cast. Sutures out next week, another cast, etc. Then the (R) CMC arthroplasty. L5-S-1 shot. Left knee, talk of unicondylar knee.only after Stress test. Never ending. Hoping to get some relief . Time will tell. T score -2.3. I did it without narcotics. Axillary, then supraclavicular rescue block( no versed or anything.) Watched the guided ultrasound. @@TransplantNurse Just realize, sharing is personal, but if I , you can help 1 other, worth it. Misdiagnosed as male breast cancer led to post Lyme disease until 2 step Western-Blot done. Add DJD, osteoarthritis & soon fully osteoporootic, you sharing provided inspiration. Only we know the multiple steroid injections.Back, hands( bilaterally) and now Synvisc 1 in (L) knee. Is unfortunately, what it is. Regards 💕

I have my surgery next week on the 29th. 😬 I'm a little nervous about it. I'm going to ask my doctor, which I should have asked him in the office had I seen this earlier, what method he's going to use??? I've watched 3 different procedures for this surgery and I like this one best! Wish me luck that the quickest healing procedure will be used! 🙏😳

I call bullshit on the no 'DNA propensity' for CMC Joint problems. Why do some people that expose the joint to as much load as many sufferers did, not end up with the joint degenerating? Why the selectivity as to who will develop the condition? Why 3 times more women sufferers than men? Medical views tend to be leaning towards arthritis degeneration having a strong genetic metabolic link to connective tissue breakdown..ie ligaments, tendons muscle fascia (Herniae) and not simply just 'overuse' Sure, Surgeons know how to operate..but that's about all they know. :)

this is not the 'anchovy' procedure - that term refers to making a ball out of residual tendon and stuffing the void left by the trapezium

Why can’t you just go in and scope the joint like you would do for a knee that’s bone on bone as a precursor to arthroplasty? Just buy me a few years…clean that joint out a bit…
