Please check anesthesia guidelines, it's given :- "These services include the usual preoperative and postoperative visits, the anesthesia care during the procedure" So intraoperative is also included.

The CPT book does not include surgical status indicators, because they are published by payers. Some versions of the code books will include the indicators as a reference, but not all. I would recommend getting them from the CMS Medicare website.
