Hi, in question 11 the correct code already includes interpretation, so there's no need for modifier 26. If the code didn't include interpretation then we must append modifier 26.

Hi, it's present in the CPT manual page No. 89, first paragraph of excision -benign lesions & also page No. 91 first paragraph of excision - Malignant lesions. Its given in the bracket excision(including simple closure).
