Q87: Respiratory failure due to acute decompensated heart failure, and we add more pressure as management with a noninvasive positive pressure ventilation? why not to use Dobutamine? unless that, the efforts here are really clear and really appreciated 🤗 Thank you so much
@WhyNot-si4pj4 ай бұрын
1:04:05 Q 116 Persistent fetal circulation - OR persistent pulmonary hypertension of the newborn (PPHN) - results in a low pulse ox in BOTH upper & lower limbs as the pulmonary perfusion is reduced & the oxygen tension in the systemic arterial circulation will subsequently be reduced . Anatomically wise ; DUCTUS ARTERIOSUS (DA) connects the PULMONARY TRUNK to the ARCH OF AORTA distal to the origins of the CAROTID & SUBCLAVIAN ARTERIES of the right side , so in case of pulmonary hypertension the flow of venous blood through the ductus arteriosus (DA) will technically deliver low oxygen tension blood to the lower half of the body . Persistent fetal circulation might also involve a PATENT FORAMEN OVALE with a subsequent right to left shift & a low pulse ox in BOTH upper & lower limbs .

Hello AJmonics I have one question from your video! For Cardiac Tamponade why is the LV preload decreased but then PCWP is increased? I thought PCWP is the same thing is L sided preload. Thank you so much and thank you for everything you do- this is the best Cardio review I have seen!!!
@AJmonics Жыл бұрын
I just JUST JUST made a video on this two days ago!!! kzbin.info/www/bejne/jpWokIuBdtqIjck Please watch it!!! I don't take such pride in my videos, but I worked pretty hard on this one to explain these factors CLEARLY. The basic idea is: imagine ENTIRELY squeezing your pet mouse - now he can't drink or pee because everything is squished up. Same thing is true with tamponade. EVERYTHING is squished up.
@AJmonics Жыл бұрын
PCWP is increased because that measures PRESSURE. PRESSURE is increased. Since pressure is increased, VOLUME is decreased. That's what LV preload is - VOLUME. Please let me know if this makes sense to you.

Thanks a dozen for the wonderful review . In Q 89, Is there an explanation why restrictive cardiomyopathy caused by amyloidosis was ruled out as a cause of right sided heart failure?
@AJmonics Жыл бұрын
You are actually totally a hundred percent correct! Restrictive cardiomyopathy caused by amyloid would have a similar presentation!! I meant to add "pericardial calcifications" to the question - which is seen in constrictive pericarditis, but not in restrictive cardiomyopathy. (I hope viewers see this post, to avoid the confusion!) Thank you May!!!
