COVID-19 Update 63 with critical care specialist Roger Seheult, MD.
Dr. Seheult explains the theory that seems most plausible to us here at MedCram.com: That COVID-19 is primarily a disease of the endothelium (the single layer of cells that line blood and lymphatic vessels). This hypothesis helps explain the increase in cardiovascular complications from COVID-19: strokes, myocardial infarctions, thromboembolism (pulmonary embolism, DVTs, and other blood clots), as well as many critical patients who do not appear to have classic ARDS symptoms (and have relatively normal lung compliance etc. as observed by the research of Dr. Gattinoni in Italy and others). A deep understanding of this theory requires a review of some foundational biochemistry including the electron transport chain and ATP - the energy currency of the body.

you said it. for real virus has dogged mankind for long time. this is fundimental research that may go a long way at understanding extremely complex biology and virus interaction that maybe a new field of medicine/science take place that end the pandemic problem in humans and animals once and for all. This whole thing has been beyond fascinating.

@@TheOlzee Its not the "big fancy words" it is the understanding of what they mean. For many, that is beyond their level of comprehension. Thankfully, many others choose to take the time to listen, comprehend and learn. I can only imagine what would have happened to Jonas Salk if he were trying to develop a vaccine for Influenza or Polio today.

A G I have had the same experience but i think it has more to do with the fact that now, this info is tied to something that directly impacts your life, more than quality of teacher. Just a more direct interest yanno? For me at least this info now feels unbelievably important

@@mtbin2 it is interesting if the Covid19 had anything to do with that... Can that fact the Vit D plays a roll in regulating Renin and Angiotensin I. Basically meaning that the body runs out of Vitamin D faster at people who are infected by Covid19? if that's the case, makes a lot of sense to load up on those IU.. 4000IU should probably be a minimum recommended intake not the minimum www.ahajournals.org/doi/pdf/10.1161/JAHA.114.001417

All good points. Really this is all pointing to Covid presentation to be largely due to Vit D deficiency. This recent study showing statistically significant correlation with Vit D levels and severity of Covid19 symptoms basically says it all: vitamindwiki.com/Critical+COVID-19+was+19X+more+likely+if+low+vitamin+D+-+April+15,+2020 Something has to account for people not getting one darn symptom yet testing positive

As a lay person, I've never understood anything so detailed in EVERY medical description as I have your videos. YOU ARE THE BEST!! Having you teach us that are not educated in this field makes us understand this virus and what it does to the human body. There is no one out there that does it any better than you. I watched someone who tried recently...NOT!!! It didn't work, I was lost and uninterested. Thank you for explaining with your diagrams and the care that you give to us. You have made it possible, and so easy to understand. WE APPRECIATE YOU AND EVERYTHING YOU DO TO HELP US UNDERSTAND THIS VIRUS DURING THIS MOST DIFFICULT TIME IN ALL OF OUR LIVES. WE LOVE YOU TOO!

Wow. This lecture reminds me my biochemistry lessons on the nursing college. I'd been sruggling to pass the exam and I was completelly lost. If I'd had these lessons in those times... I'd be much more into the biochemistry. It is very adventurous journey to go through your lectures, doctor. Thank you for your teaching.❤️🙏🏼👍🏻 Can't wait to see another lecture.

Dr. Seheult what you are doing is extremely important in this difficult time. It reassures people who are not in the field, people who are frustrated, people around the world who lost hope. Your videos inform your peers and inspire students and other researchers.I vote that this community as well as specialists recognize you as one of, and possibly the champion of the fight against this terrible pandemic. I adore your way of pulling scientific papers and creating conversations between your observations, your interpretations, and the work of others that already went through peer reviews. Your discussions make a lot of sense. You always keep a small margin of doubt until you verify the idea. That is the way excellent scientists follow. Please continue this good work.

It acts as a disinfectant, since in its mode of action it turns out to be an oxidant. [1# BiologicalEfficacyList ] It is very similar to the way our own body acts, for example in phagocytosis, where an oxidation process is used to eliminate all kinds of pathogens. Chlorine dioxide (ClO2) is a yellowish gas that, to date, has not been introduced into the conventional pharmacopoeia as an active ingredient, although it is used on a mandatory basis to disinfect and preserve blood bags for transfusions. [2# Alcide studies on blood disinfection] It is also used in most bottled waters suitable for consumption, since it does not leave toxic residues; besides, it is a gas that is very soluble in water and evaporates from 11ºC.The recent Covid-19 coronavirus pandemic demands urgent solutions with new approaches. Therefore, chlorine dioxide (ClO2) in low-dose aqueous solution promises to be an ideal, rapid and effective solution. All too often, the solution is in the simplest of ways. The approach is as follows: on the one hand we know that viruses are absolutely sensitive to oxidation and on the other hand, if it works in human blood bags against viruses such as HIV and other pathogens, why would it not work organically against the coronavirus? Chlorine dioxide removes viruses through the process of selective oxidation in a very short time. It does this by denaturing the capsid proteins, and then oxidizes the genetic material of the virus, disabling it. Dr. Seheult deserves an international honor for his communication of complex biological processes to the average person. While I don’t completely understand him based on a single viewing, some of it sticks. Understanding reduces fear and his communication of these processes gives colleagues the tools to better treat patients and scientists the tools to advance research. .

Wrll, folks, I've mentioned this before...Dr. Seheult is a genius.. Had he been my profesor in university Arts and Sciences, I'd have probably leapt into science and been a fanatic. Here I am, an 82-year old Jungian therapist, former professor of literature and mythology, absolutely spellbound - learning something brand new and above all, understanding a great deal of what Dr. Seheult is describing. He is an absolutely gifted teacher - and where he finds the time to do all this is beyond me. I'm filled with gratitude for his dedication, expertise and determination that we all understand what's behind this deadly virus.I await each new episode with excitement! Imagine!!

That “Ohhh i see” moment when you said what covid19 does to ace 2 in reference to the rest of the diagram. I’m no where near medically inclined but I understand this perfectly. a logical question would be if unregulated ag2 is a key modulator of ROS , then why wouldnt an ACE inhibitor be helpful ? this would essentially block the creation of AG2 . Majority of patients with CAD , htn ckd are presumably taking an ACEI as it's first line treatment for HTN In those with CKD, DM. Additionally used in CHF. Maybe Dr Seheult can shed more light on why it would or wouldnt be helpful ?

I've read that there have been instances of high iron concentration in the bodies of patients suffering from Covid19. Can this be that because of impairment of the SOD function via the lack of ANG 1-7, Catalase is the only mitigating factor in combating Oxidative Stress? Thus, the high iron levels due to Catalase being the only game in town, so to speak. I'm a retired truck driver, so you all go easy. It's truly a credit to Dr. Seheult's concise and thorough presentation that allows a layman such as myself to comprehend the insidious nature of this disease. I'm looking forward to the Webinar tomorrow. I'll be the old, quiet guy in the back row. Cheers!

The high levels of iron, suspected to no longer be in the hemoglobin, are being absorbed by the ferritin, which could explain why the the ferritin levels might be higher. "The rationale for vitamin C in sepsis [and for that matter in COVID-19] is strong. Humans are unable to synthesize vitamin C, while animals acutely increase its production during stress to protect cells against the overwhelming production of reactive oxygen species (ROS) causing endothelial and mitochondrial injury with subsequent organ failure [3]. In animals, the endogenous vitamin C production is inversely correlated to the cortisol response. Higher cortisol responses are associated with poorer outcomes, which may be attributed to higher severity of illness, but also to vitamin C deficiency [4]. Vitamin C is our central antioxidant, which directly scavenges free radicals, prevents production of ROS, and restores other antioxidants, reducing endothelial permeability and cellular apoptosis. Furthermore, vitamin C is a neuroprotector, immunomodulator, and cofactor for synthesis of vasopressors. Increased vitamin C consumption and reduced recycling lead to abrupt hypovitaminosis C (≤ 23 μmol/l) and deficiency (≤ 11 μmol/l) as measured in 88% and 38% of the patients with septic shock [5]. Recently, the largest randomized clinical trial (RCT) of intravenous vitamin C in septic patients until now, the CITRIS-ALI trial, showed a 16.5% lower mortality in the vitamin C group compared to placebo, while the primary endpoint deltaSOFA-score after 96 h was negative [1]. Mortality curves parted from day 2, which may be due to early circulatory stabilization by vitamin C [2]. Many comments on this trial addressed the methodological problem due to the exclusion of deceased and discharged patients from the deltaSOFA-score. Given the early mortality reduction in favor of vitamin C, recalculation with assignment of the worst SOFA score to deceased patients and the best score to discharged patients will undoubtedly show less organ dysfunction. So, the results of the CITRIS-ALI study support the promising role of adjuvant high-dose intravenous vitamin C for septic patients. The CITRIS-ALI trial differed in two important aspects from the groundbreaking before-after study of Marik et al. which showed a 30% absolute mortality reduction, but had the limitation of not being randomized [2]. First, the i.v. dose of 200 mg/kg/day vitamin C (i.e., 16 g/day for 80 kg BW) was higher than the 6 g/day of the HAT-therapy. Second, vitamin C was administered as monotherapy and not in combination with hydrocortisone and thiamine (HAT-therapy, based on the hypothesis that hydrocortisone and thiamine have potentially synergistic effects with vitamin C). Although the positive results of the CITRIS-ALI study on mortality question the necessity of co-administration of hydrocortisone and thiamine when vitamin C is dosed sufficiently high, notably 66% of patients in CITRIS-ALI study received corticosteroids... The optimal dose of vitamin C is unknown, and systematic dose-finding during critical illness has not been performed. However, several facts are known. First, the antioxidant capacity of vitamin C is dose-dependent and direct radical scavenging capacity is maximal at a plasma vitamin C level > 175 mg/l (1000 μmol/l), more than ten times normal [6]. In addition, other functions like immune modulation, inhibition of bacterial replication, and neuroprotection are dose-dependent as well. Second, intravenous dosing is necessary, because enteral uptake is limited [3]. Third, intravenous vitamin C doses of 2-3 g/day are needed to normalize plasma concentrations and sustained therapy is needed to prevent reoccurrence of hypovitaminosis [7]. The dose-concentration relationship is linear, so higher doses yield proportionally higher plasma concentrations [7]. Plasma concentrations > 1000 μmol/l can be obtained with 10 g i.v. vitamin C/day [7]. In the only dose-effect study in septic patients, the higher dose of i.v. vitamin C (200 mg/kg/day) improved organ dysfunction more than the lower dose of 50 mg/kg/day. A recent meta-analysis found a reduced mortality with medium-dose vitamin C (3-10 g/day) in critically ill patients, in contrast to low (

I hope you will post the webinar so that I can listen to it later. I would love to hear it because I have followed all of your posts in order to figure out how to deal with my own personal health situation. (Bad as it gets, high risk, elderly, multi-generational household). I don't feel ok about taking up a space which might go better to a medical professional. Because of your posts, we are doing everything we can to reduce risk, my favorite post what the 'what I do when I get home to not bring the virus into my house'. It makes me feel better everything I bleach wipe my light switches. Zinc is daily....

This whole lecture series is doubly important. 1) It serves as a historical record of the disease in terms of actual progression and in terms of understanding. If we'd known this in January... but you have to 'earn the learn', unfortunately. Terrible though this is, we are at least fortunate to face this in 2020, rather than the poor doctors with their techniques and understanding who had to face Spanish Flu in 1920. 2) It's so clear as explanation. If medical school were completely online with videos, I think I might be able to graduate; otherwise, so much goes over my head the first time. So glad to be able to watch this. Awesome, this lecture took me back to metabolism class as a Food and Nutrition major in college. I love the names of superoxide dismutase, glutathione peroxidase, especially, their important effects on oxidation of cells /free radicals. Catalase included of course. Thank you!

Thank you Doctor Seheult for the immense work you are putting into assembling all this information in a digestible form, ON TOP of your full-time job fighting this horror at the point it is most desperate. I'm simply soaking up these postgrad level studies right on the leading edge of biochemistry and the mechanisms of infection and damage from a disease, which you selflessly provide free to us and the whole world. I'm miraculously managing to follow them, despite a background (In IT, so at least I'm used to logical thought and ongoing education) far removed from the medical field (although I've always been science nut, a trait inherited from my grandfather). The closest I've ever been to studying medicine was understanding my own sicknesses and treatments (unfortunately many and varied), basic first aid (I used to do voluntary work as a motorcycle road race marshal, so have been the available body to assist in some trauma management) and studying for my flying exams. You manage to explain in ways which simplify the complex and even allow me to make predictions of the direction this is leading in. I may be smart (my medical records say I am), but without an explanation presented logically, I'm lost! I think there are probably far more smart people out there who simply have not been given the chance to show it than is commonly believed. You are providing that logical explanation, and in a way that even those outside the medical field can follow! Definitely worthy of the Faraday prize, and if and when it is turned into a paper for peer review and published (which it should be - you are making logical steps which tie so many other papers together and then apply them in a new way), it could easily become the definitive text on how to manage and treat the virus effectively, and be worthy of the Nobel prize for medicine, given the global desperation for such a management protocol. But first we've got to beat this little bastard of a virus, and thank you for your work in the trenches actually applying all this to those who need it most.

I don't know if the criteria for the Nobel prize would apply here, but certainly a Faraday Prize is ABSOLUTELY in order. The Royal Society of London Michael Faraday Prize is awarded for "excellence in communicating science to UK audiences". Named after Michael Faraday, the medal itself is made of silver gilt, and is accompanied by a purse of £2500.

@@ELee-zv5ud Yes, you are right. In the eligibility criteria (see link below) the tenth point is: 10. The prize is open to citizens of the British Isles or a Commonwealth country, or those who have been ordinarily resident and working in the British Isles or a Commonwealth country for a minimum of three years immediately prior to being nominated. Ive learned SO much from these updates, thank you eternally. Even though I’m fascinated by the sciences, I actually failed secondary school (high-school) science. My only failing grade! My academic & professional background is creative (& communications), so it’s be a stretch is a stretch for me to understand every detail, but the way it’s explained I’m able to follow and come away so much more informed. Been watching for the last month and it’s SO exciting and satisfying when you refer to past videos that I keenly watched on repeat to try & learn more & more. It’s making me want to go into medicine/biology/something! Thank you again 🙏🏼

This was a lot of good information for a layperson like me. Seems we need something to protect Ace 2 before it impacts the rest of the process. Of course i know nothing, but I appreciate these lessons.

In response to your post, "Seems we need something to protect Ace 2 before it impacts the rest of the process" (good thought) Monoclonal Antibodies targeted to bind to the virus's ACE2 latch,Preventing the Virus to bind to ACE2 , have been developed by Centivax and Distributed Bio LLC and a few other company's. Centivax's Antibodys ( www.centivax.com/monoclonals ) if all goes well, can be distributed as early as Sept.

Excessive ROS production is indeed part of the inflammatory process. What complicates it is that LOW levels of ROS, from sources such as the mitochondria and NADPH oxidase, are part of normal cellular physiology, serving a signaling role, switching activities of key enzymes on and off by reversible redox reactions. Hydrogen peroxide is heavily involved in such normal signaling. Under normal conditions, the levels of ROS are kept under control by SOD/catalase and the glutathione redox system he mentions here. But if these cellular antioxidant systems' capacity to maintain ROS at physiological levels is exceeded, then you get oxidant stress. And that can trigger inflammation. Even inflammation plays a normal role in host defense against pathogens, and as an initial step in the the repair process after an injury, and as part of that normal response, there are steps to tamp that inflammation back down once it's no longer needed. Like excess ROS, it's when inflammation gets out of control that problems arise. Finally, inflammatory cells themselves, e.g. macrophages, neutrophils, actually do their thing by generating ROS--it's part of how they attack invading pathogens. Unfortunately, other things can trigger an inflammatory response besides an invading pathogen, and if that inflammatory response becomes dysregulated, diseases can result. Most of the big diseases plaguing Western society these days--cardiovascular disease, cancer, arthritis, inflammatory bowel disease, type 2 diabetes, autoimmune disorders, ischemia-reperfusion injury, etc. all have ROS-associated inflammation at their root. This has been recognized for decades now, and it is a huge area of research interest in biomedicine.

Sarah TJ inflammation is a single type of oxidative stress due to the immune response activating neutrophil and macrophage cells which kill pathogens by producing ROS.

Melatonin is endothelium protective and a potent antiinflammatory, boosts the body's natural antioxidant system such as catalase, SOD and glutathione, is a highly potent antioxidant on its own that can scavenge ROS & RNS, is protective of mitochondria and mitochondria activity, increases ATP production, helps prevent reduction of ANG 1,7 and is a potent antiinflammatory among many other potential health benefits! Please do a report on melatonin as it might relate to Covid-19!

The brilliantly intelligent doctor! You have connected the dots , it made so much sense. This also demonstrated the relationship between metabolic health and infectious diseases. A lifestyle which lowers oxidative stress on the body (eg. keto diet and intermittent fasting) would counteract these oxidatve stress, or at least lowering the baseline, thus putting the body in a much better position in responding to the disease. THANKS! I am your fan !

Their ace2 makeup is different as well. Blacks are not to take ace 2 inhibitors.

Also many white people are deficient in Vitamin D as well. In winter anyone in northern places -all of Canada, northern US until you get really far south, northern Europe, Russia etc In the summer you have to have 15 daily minutes of enough skin e.g., arms and leg exposed (without sunblock) lol. Most Australians are Vitamin D deficient because they were encouraged to use sunblock all the time ! So everybody take a Vit. D pill.
