How Do You Treat a Choking Adult? New 2020 AHA / ILCOR Guidelines | CPR Certification Institute

Updated for new 2020 AHA / ILCOR guidelines. Healthcare Provider for the Conscious and Unconscious Choking Adult Victim lesson by the CPR Certification Institute (Authorized Health Ed Solutions Training Center). Mark Dzwonkiewicz, the lead instructor from Health Ed Solutions, demonstrates the proper response to a choking victim and explains the physiology behind the Heimlich maneuver. He touches on the following updated AHA and ILCOR guidelines:
📌 Conscious Victim:
👉🏻 Explain what you're going to do
👉🏻 Perform the Heimlich maneuver until the obstruction comes out or the patient becomes unconscious
📌 Unconscious Victim
👉🏻 Activate EMS
👉🏻 Open mouth and try to pull the out obstruction if visible
👉🏻 Perform CPR - Every time before providing a breath into the victim, check to see if the obstruction has loosened up and can be pulled out

PLEASE inform yourself, this advice may be OUTDATED. A recent KZbin video from Mass General Brigham (Boston, MA highly respected hospital system) on adult choking protocol explicitly says to 1. Ask victim if they can cough to remove obstruction and if not, 2. Proceed with FIVE blows to the victim’s upper back (between the shoulder blades) with the heel of your palm, and if unsuccessful 3. Continue with abdominal thrusts, then continue to alternate between those two. If victim loses consciousness, continue with CPR. This American Heart Association video DOES NOT DISCUSS the back blows! My husband just took an AHA first aid course on 5/10/24 taught by a paramedic following AHA protocol and there was NO MENTION of back blows. This is all to say that my mother died a couple weeks ago b/c her partner WAS NOT AWARE of back blows (even though he had also just taken an AHA CPR (etc.) class a month or so prior to this incident). Because he wasn’t aware of back blows he did not attempt back blows, even after abdominal thrusts did not work (he couldn’t get the right grip on her body to perform them properly). Had he attempted back blows maybe, just maybe, she would have survived! Shame on AHA for not teaching first aid in a way that takes all options into consideration. I get that in some circumstances back blows may not be appropriate (if you are not able to make sure victim is leaning forward, e.g.).

I need to understand something because my mother just passed away after a choking incident. Why do you not teach about five blows to the back (between the shoulder blades with the heel of your hand)? Other organizations ARE teaching this. If my mother’s partner had tried the back blows and not just the abdominal thrusts (which did not work) she might have had a chance.

Back blows people - read more about BACK BLOWS as another option, the FIRST option if victim cannot cough, before you proceed to the Heimlich. Five blows to the upper back area between the shoulder blades. If it doesn’t free blocking object, proceed to Heimlich, then alternate between the two until loss of consciousness. PLEASE INFORM YOURSELF. You might just save someone’s life.
