Seems bit late to reply, I guess you are using an emulator to play the game and always use save state for saving. I have encountered this problem also, and later I discovered that since I haven't ever load the in game save, so the game doesn't know which save file (among the three) to update after I beat Mildrath and comes to the The End screen, so I load the in game save once and beat the game again, then I can access Bonus Dungeon.
