To be honest it was just off Ebay. I think it was actually for a Landcruiser but I just cut out the blank mounting tab with a Dremel tool for it to fit into.

How would I remove the centre dash glove box to replace it? Is this the start of the access to it?

Yea, the LTZ is totally different because of the climate control system.
