SPOILER ALERT: If my knowledge from the Light Novel is right, Phantom is actually Mio Takamiya the whole time. In my opinion, Phantom had Reine's appearance was because Mio was using the same power of Haniel (since she's the Spirit Of Origins, she created all 10 Sephiras with parts of her powers, and the transformation power of Haniel is no exception) to disguise herself to trick Kurumi (on something I'm not really sure).
