I have also been a software architect and engineer my whole career, with more of a focus now on metabolic health, as I fit into the LMHR profile. I mostly find within the medical community an absence of the ability to change or accept new ideas,, rigidity, pharma corruption, unnecessary complexity, deeply troubling from the perspectives I am used to in my industry, Dave's simplistic presentation approach is a breadth of fresh air, demonstrating better communication style with the laymen who after all are the ones seeking the truth about improving their own metabolic health. Doctors beware, educate yourself better, the patients are coming for you now.

I had to dump my doc , He got really shitty when i refused statins , I mean really out there , Id be dead in 6 months if i didn't go on statins before i left his practice , Even had the nurse harassing the Mrss over the next 3 days on the phone . dx398 wow, watched it and he pretty much took off the gloves and one, two, out. I’m getting it that we have been bamboosald (spelling?) for so long to put our money into the pockets of the big farma. I don’t want to sound like I’m spouting corruption or anything but enough evidence builds up that you eventually think, “if there’s smoke, there must be fire.” I have stopped my statins and upon completing my blood pressure meds will not be refilling it either. This completes my doctors wishes and I feel pretty good about my progress down my LCHF journey.

Just tell your doctor no. You have 100% control over your own body. Many, most doctors have very little nutrition knowledge. They know medicines, pills and all things pharma because that is what they are taught in med school. And, in many cases, their med scripts heavily support their income. You don't need a compelling argument you just need to do your own research and prescribe the diet that is best for you.

Phillip good for you. A friend was on statins for 9 years before he had quadruple bypass !!!! I told a guy at the Mill this one day & he said he had low cholesterol & he had a double by pass. An ex co-worker on statins had a bypass 8 yrs ago. I talked to him about diet, health & exercise & he said "that's what they make medication for". !! He died this past fall of a heart attack. Too bad, nice guy. Good comments on here 1 Just found D Felman ystrdy. Merry Christmas to me !! My best present ever.

Dietary cholesterol is only found in animal products and animal by-products. Our bodies naturally make the amount of cholesterol that we need, however, humans increase the level by consuming animal products and animal by-products. Sooooo to reduce cholesterol, simple switch to a non processed vegan diet.

I have been on keto for 3 years now, usually under 5g carbs per day. I am a 5'2, 104-pound young female and mostly sedentary. I was shocked when my bloodwork came back. TC 531, LDL 350, HDL 64, TRIGS 94. My remnant cholesterol is 117. I thought I may have familial hypercholesterolemia, but both of my parents had normal cholesterol on and off keto. I don't have any major diseases that I know of. Why is my remnant cholesterol so high? I eat meat, fish, eggs, dairy, and some vegetables. No vegetable oils or processed foods. If anyone has any insight or resources, it would be much appreciated as I am slightly concerned with my bloodwork but I don't want to get off keto.

Thanks for the presentation. I am some level of lean responder with LDL - 197mg/dl, HDL - 90mg/dl, Trigl - 61mg/dl and remnant cholesterol of 2mg/dl (two). HbA1c of 4,5%. Male, 56, 5´11", 145 lbs, athletic. My doctor knows about low carb and she did not speak about statins to me.

Given that LDL is calculated; Remnant Cholesterol is equal to 20% of Triglycerides. Any deviation from this formula is due to rounding in calculating LDL. So Remnant Cholesterol and Triglycerides track perfectly.

Because of this presentation l sleep well at night while my doctor looses sleep over my high cholesterol levels. Have been on keto for a couple of years now after after suffering from hypoglycemia due to high insulin. Thankfully all my numbers have improved but my cholesterol levels keeps going up from 160, 200 to 260. At 200 my doctor prescribed cholesterol lowering medication. I didn't fill my prescription but didn't tell my doctor because he wouldn't understand.

Dave, I really liked your presentation and the simplified layout and for the most part your presentation will apply to at least 1/2 the population. Hard to say now days because so many people are sick and when you have sick organs in the body the whole game changes on everything regarding cholesterol, diet, supplements, and so on. For example, let's say the liver delivery company and/or the intestine delivery company is compromised due to poor diet, excessive alcohol consumption, etc.. Now we are digging deep and really finding out how and why no two people are alike when it comes to this sort of thing. I remember back in the mid-90s when I first started in the health industry I would give the same lowering cholesterol regimen to all my clients who had high cholesterol, LDL, etc.. When blood test results showed a drastic improvement with most clients, a few remained unchanged after 90 days or so. At that point I began to dig deeper and really learned how magnificent the human body really is. For anyone reading, keep this in mind. Disease starts in the colon and then it's all downhill from there. If the colon is functioning properly and you are having a minimum of 3 bowel movements per day - if you eat 3 times per day - then the liver will most likely be okay considering you exercise to keep the lymphatic system moving and drinking enough water with electrolytes to keep the kidneys working like they should. Basically as long as the drainage/elimination system is working then most likely all is okay and the body can function as normal - hence - do what it supposed to do when it comes to keeping cholesterol in check. One thing I do know is a problem with cholesterol is not always with diet. Our environment, genetics, and lifestyle as a whole plays a major role. Even stress! Stress drives cortisol which then drives sugar and well, you get the point because the liver acts totally different when the body is under stress. I've had diabetics drop from a 7.5 A1c to a 5.2 A1c within 3-4 months by simply giving them an adaptogenic herb like KSM 66 combined with Cordyceps to help their body deal with stress. Most people think diabetes is caused by diet and that may be true in some cases, but I'm finding that stress is the culprit these days. Keep up the good work, Dave!

June 26, 2017 (Eating my usual diet): TC = 198 - LDL (146) - HDL (45) = Remnant Cholesterol 7. Trigs = 77. LDL particles = 898 nmol/L. Remnant Lipoprotein = 139 nmol/L. April 9, 2018 (Eating more fat): TC = 275 - LDL (206) - HDL (51) = Remnant Cholesterol 18. Trigs = 67. LDL particles - 1192 nmol/L (HIGH). Remnant Lipoprotein = 148 nmol/L. Eating more fat made my Remnant Cholesterol and Remnant Lipoprotein worse. I guess I should go back to eating my usual diet.

On my recent lipid test have category called non-hdl that is not ldl. What would be the remnant cholesterol calculation? Tot chol 236, hdl 63, trig 117, non-hdl 173, ldl 150. I am now on keto. Went off statin. Type 2 diabetic. My hdl and trig are good. Cant figure out the rest.

I've been keto for four months lost almost twenty, don't want to lose anymore. That aside I have been sick with some respiratory thing, went to doc to get checked blood panel etc. A few things stood out, my cholesterol was 380, triglycerides 63, hdl 58, ldl 309. I have done omad most of the four months and 16/8 with high carbs for three years before that. The other number of question is 1550 b12. Any thoughts on this?

Are you by any chance still slightly overweight? If going by the analogy provided, if you are eating less fats, your body will make more VLDL. If you want to lower your LDL score, next time you go on a blood test, eat a lot of fat throughout three days before your test.

I have had times when I have eaten as many grams of fat and grams of carbohydrates, in the same day, and will go right into fat burning mode because of it. It didn't make sense to me to be burning fat on days when I had so many grams of carbohydrate in the diet.Well I have asked this question in many different groups and got zero answers from anybody. But after putting the nerd brain into learning mode I have realized the answer to the question of: "Why do I burn fat when my glucose levels are elevated?" The guru types say that your glucose level has to drop and insulin has to drop before you go into fat burning mode. This doesn't seem to be true since the body will burn fats carried into the lymph system by the chylomicrons before addressing the protein and glucose that go to the liver. That is my take on what you present in your teachings. Please correct me if I am wrong...

I am very confused by the slide at 15:16 regarding remnant cholesterol. It depicts remnant cholesterol as consisting in VLDL and IDL, and those two are lipoproteins that have not yet fully delivered their loads of "energy". (Am I not right in interpreting the orange energy boxes as the triglycerides that are being carried by the "boats"/lipoproteins?). However, the Wikipedia article says that "Nonetheless, remnant cholesterol is primarily chylomicron and VLDL from which most triglyceride has been removed, such that each remnant particle contains about 40 times more cholesterol than LDL." This appears to be the contrary definition from what the slide depicts. It also seems rather more intuitive to conceive of a "remnant" as what remains of the carrier after (most of) the load has been removed.

My view is that remnant is the nomenclature used to refer to the balance of Total Cholesterol minus LDL and HDL in a regular Lipid Panel. For myself I paid for a VAP lipid panel and the remnant number was calculated for me. It is a little bit more exact than if I substract LDL and HDL from my Total Cholesterol number, but quite similar.

My LDL is very high at 238, I was doing IF 20hrs a day, with Chicken breast and eggs diet of under 500 cal for 3 days before my blood drawn. I dont know what causes it. I was 96kg some months ago and my LDL is 130 something, now I am 70kg, and I am curious why It went up when my diet is strict and I also do IF.

Watch some of David's other videos. You're likely to get a lower LDL reading by eating high-calorie with a lot of fat for 3 days before the test then fasting 12-14 hours beforehand. It's the opposite of what you'd think.

That's an entirely different subject and a long one at that. I may do a blog post on it soon. But to give a sneak preview (and one I've long talked about), I don't care if you can reduce the risk of dying by one thing unless it likewise means a reduction of death by all things (AKA all-cause mortality). Hence, I have the example of my "Cyanide Diet" where it ensures your chance of dying by heart disease is reduced by 99.999% -- but you only achieve this by dying of *something else* first. Thus, I don't compare statins for their reduction in death by CVD, I compare how they reduce death overall.

Glenn Graham a basic fact to look at is that most people died 100 years ago due to infections, then penicillin came along. Population studies done after WWII show the anomaly (or not) since low-fat diets were pushed since the 50s and now the people who are “living longer” are not necessarily from the US. I have similar experience where after stopping my statins I went to low carb high fat liefestyle by focusing on good fats like avocados, nuts etc. My ldld levels doubled my Hdl level increased 50%. I also have type 2 diabetes and inject insuling twice a day. My new lifestyle has brought my sugar levels totally under control. My problem is that I would not know when and by how much to reduce insulin injections.

That's a bigger subject. There's means beyond the bloodstream (transmigration), and there's means in the bloodstream where as NEFAs (Non-Esterified Fatty Acids) they bind to albumin and either (1) are absorbed by other cells or (2) are absorbed by the liver eventually and repackaged as triglycerides into VLDLs to be sent out again (purple line)

Hi Dave - excellent video. I live in the UK and I've been on the keto/low carb diet for over and year (I've lost 28 lbs and definitely don't want to lose any more weight) and last week had a comprehensive blood test to see whether it's had any detrimental effects on my system. I had the results back today and the doctor has flagged up my cholesterol as being too high and I need to go see the nurse to talk about changing my 'lifestyle'. These are my results: serum cholesterol 7.2; serum triglycerides 0.5; serum HDL cholesterol 2.6; serum LDL 4.4; serum non HDL cholesterol 4.6; serum cholesterol/HDL ratio 2.8. I'm afraid the US system confuses me as the numbers are so different. Can you give me any advice on what my numbers really mean and whether I should be worried? Many thanks

Your HDL is 100 and triglycerides 44. TRIG/HDL = 0.44. Anything below 1.8 is good, and yours is amazingly good.

I didn't even know there is something called remnant cholesterol, and it seems mine is 13 and is good... ? Wow, thanks. I went plant based for several months. My body and workouts felt good at first, checked my lipids and glucose frequently. My LDLs went down (I thought was good), but my Triglycerides went high. And my blood pressure went up. And my body composition went toward higher fat and less muscle. Clearly I was becoming insulin resistant. I guess I'll have to be happy with high LDLs. Lol

So could my remnant cholesterol really be 5? Total 282-144 (LDL)-131(HDL)=5. My Trigs are 33 so where do they fit in the total? I have had higher HDL for years > 100 for the last 10 years but never as high as 131. Love my LCHF way of eating.

great presentation and thank you for the unbiased hard work. we need to look at how the entire system works and not just parts of it in isolation. LDLs are bad only in the setting of elevated blood glucose and suboptimal HDL and trig over a chronic period of time. Also, Ketosis is a state of health that has not been studied in the traditional realm of CVD and so the traditional significance of elevated LDL may not be pertinent in ketosis. you may have the largest body of research in this arena at this time. my 2 cents. thanks again

ViveChris if you eat cholesterol, then your body doesn't have to make it. If you don't eat cholesterol, your body produces it. Pretty straightforward. Cholesterol is critical to life and health, so our bodies constantly track and regulate how much is available.

I think your doctor can order you one for free there. But of course, the doctor may have a strong reaction if you are a LMHR given your LDL will be very high.

You can ask for a standard cholesterol test through your GP for free, it'll give measured Total cholesterol, HDL, Trigs and calculated LDL. Think I also meet the criteria for a LMHR having been LCHF for almost 10 years, my GP was hugely concerned by my last test results even though ratios were great and the 10 year risk score was 1-2%. Be prepared to argue against an offer of statins.

What is this non HDL cholesterol I’m now seeing? Why are they posting it and what’s the affect of it? What’s the concern level of this marker? Or is it another excuse for statin conversation???

Great stuff Dave, I just found out that I’ve got the ApoE 2/4 gene. My diet is high quality fats, nuts, unprocessed everything, lots of eggs, low sugar, low carbs. My Total Chol around 200, HDL is 47, Trigs are 57 and LDL-C is 130. My calcium scan score is 226 and a PET scan revealed that my heart is in good shape, with 97% blood flow. Any suggestions on diet since I have the ApoE 2/4 gene and that I have some CAD? Thanks

Enjoyed the metaphor the first time through, but I'll have to watch it again to find out how those purple boats with nothing but care packages serve as support (as in what specific things they do with that care package).

That can be a whole video on its own. But short version is that they play an immunological role in binding to pathogens and a support role for cells that need their contents by endocytosing them (engulfing the whole LDLp)

Dave....Found you and I'm rookie. I'm 74 had my wellness exam....cholesterol 173, trig 71, hdl 62, ldl 98....and get remnant choles "13"....I'm rice eater & feel good. My Doc told me that my "Risk Factor" (RF) calculated score is 15% (means 15% chance of heart attack or stroke in next 10 yrs)....and he recommends starting 20mg Crestor pill for preventive care program & possibly lower my 15% RF. Honestly, only # that would lower my RF of 15% is if I got younger age from 74-to-50 (playing with online RF calculator). Doc said my lab # are acceptable BUT RF 15% his concern & why I should start Crestor now as prevention step. If you were me what would you do ?.....sammy

Why? I am Vegan my RC is 5, I guess that is better than any of the above, no? My Trig/HDL is 1.3 good enough for me. Furthermore animals are not suffering and are not killed because of me and the planet is not as much polluted, that is a three times win, no?. I will however ask you why are you not Vegan?. Even if it would be just as healthy and there is no requirement to eat dead animal corpses and body parts. And yes my priority is health as number one as well.
